MCFP- SPRINGFIELD- FEBRYARY 8, 2010
 
At approximately 10:38a.m., an inmate from another unit assaulted another inmate. The OIC called for assistance and the two inmates were separated and taken to lockup. The inmate that was assaulted received some bruises to his facial area. There were no staff injuries reported.

FCI- ELKTON- FEBRUARY 4, 2010
 
An officer was conducting a shake down of an inmate’s locker. While shaking down, the officer found a leafy substance in an envelope. The inmate arrived at the area and made a lunging gesture towards the officer in an attempt to grab the substance. The officer was required to use physical force to move the inmate away from him as he activated his body alarm. The inmate was placed in the Special Housing Unit. There was no known injury that occurred to the staff member.

At 2:00p.m., the tower officer advised that a group of gang rival inmates appeared to be squaring off against each other on the rec yard. The compound officer requested additional staff to report to the rec yard. Responding staff observed an inmate bleeding from the face area. He was immediately escorted off of the rec yard to health services. A review of the Vicon security camera revealed one inmate striking another inmate with closed fist punches and knee kicks to his upper torso and face area. The inmate was medically assessed and escorted to SHU without incident.

At 12:50p.m., an inmate threw urine in the face of an officer as he was making rounds in the Special Housing Unit.
 
**At 09:10p.m., an officer was searching an intoxicated inmate in the Lieutenants complex. The inmate became disruptive and struck the officer with his elbow. The officer received a contusion on his arm.

At approximately 8:32 p.m., an officer requested additional staff report to K-Unit. The officer observed an inmate lunge towards him in front of the upper tier officers station. The officer perceived the inmate lunged at him with a weapon after he heard something he perceived as metallic fall to the lower tier. The inmate’s attempt to assault the officer failed when another inmate stepped between them and wrapped his arms around the inmate preventing the assault. The inmate then escorted the other inmate back to his cell. Responding staff secured the unit as other inmates within the unit were loud, unruly and resentful of being locked down. The inmate was removed from the cell, medically examined, and placed in the Special Housing Unit.

USP- CANAAN- JANUARY 30, 2010
 
At approximately 11:30 a.m., the e-1 unit officer notified the operations lieutenant of a possible fight in his housing unit. The unit was lock down and upper body searches were conducted. Two inmates were found to have injuries both inmates were taken for medical assessments. At 1:15 pm one the inmates was taken to outside local hospital for injuries he sustained from the fight. There was no staff injuries to report.

At 11:10 p.m., an inmate in SHU covered his window and broke the sprinkler head in his cell. As a SHU officer was attempting to clear the water from in front of the cell door, the inmate threw a liquid substance which smelled of feces on him. It hit the officer in his arm and leg.

At 05:27 p.m., a fight was announced in A-2 Unit. Several inmates were fighting and staff responded. As an officer was attempting to pat search an inmate on the top tier of the unit, the inmate became combative and grabbed the officer by the arm. The inmate then pushed the officer towards the top tier railing, while simultaneously reaching for a weapon which was hidden in his waistband. Chemical agents were utilized on the inmate to stop the assault, and the inmate was then restrained and the weapon recovered.

At approximately 8:10p.m., an officer working the mental health unit found an inmate unresponsive. His hands was tied and a ligature was around his neck. Two inmates were locked up for the homicide.

At 6:05p.m., while an officer was searching a cell in A-1 (SMU)
unit, an inmate entered the cell with her and punched her in the side of the head. The officer was referred to an outside hospital for treatment. A sharpened weapon was recovered from the cell after the incident.

FCI- OXFORD- JANUARY 25, 2010
 
An inmate in an unit was found with injuries consistent with an assault. A second inmate was identified as being involved. Both inmates were examined by medical staff, then placed in the Special Housing Unit. Inmates in the unit were placed on cell restriction until mass interviews were conducted. There were no staff injuries reported.

USP-1 is on lock down, due to inmate found unconscious and stabbed in the recreation yard.

FCI- EL RENO- JANUARY 22, 2010
 
At approximately 6:00 an inmate was assaulted in a housing unit. The inmate was transported to a local hospital for his injuries. The institution was locked down and 11 inmates were identified as possible participants in the incident and were placed in SHU.

USP- LEE- JANUARY 21, 2010
 
At 3:44 PM a unit officer announced a fight in his unit with weapons involved. As responding staff entered the unit the assault stopped and the inmate was secured in a tv room. The victim was restrained and removed from the unit to the unit team area. The victim was then transported by stretcher to the health services area. At 4:21 PM the victim was transported to an outside hospital where he was pronounced dead at 4:55 PM. Institution remains on lock down status pending outcome of mass interviews.

FCI- BEAUMONT- MEDIUM- JANUARY 17, 2010
 
At 7:30a.m., an officer was assaulted by an inmate with a book. The inmate was escorted to S.H.U.
 
**At 11:00a.m. 26 inmates refused to leave the recreation cages in SHU. The inmates held the recreation cages closed for several hours before complying with staff orders to exit the cages.

USP- LEWISBURG- JANUARY 17, 2010
 
At 3:13p.m., a cell fight broke out in F Block. Gas was used to separate the inmates. Both inmates were removed safely and medically assessed. There was no staff injuries reported.

FCI- BEAUMONT- MEDIUM- JANUARY 16, 2010
 
At approximately 6:00p.m., an inmate in SHU threw an unknown liquid substance on an officer.

USP- LEWISBURG- JANUARY 16, 2010
 
An inmate was placed into ambulatory restraints for assaulting a staff member in G Block. There was no other staff injuries reported.

FCC- COLEMAN- USP 2- JANUARY 14, 2010
 
At approximately 9:30a.m., an unit officer was monitoring the movement when he told an inmate he cannot enter his unit. The inmate quickly jumped on the staff and they were wrestling to the ground. The officer was taken to the hospital and pending more information. Another staff member received head injuries as well. He was the first responder and he suffered some type of head trauma. The officer was also sent to hospital.

At approximately 10:35 a.m., 200 hundred inmates that were in the gymnasium were observed grouping up by staff. These inmates were placed in the gym due mass shake downs being conducted in c-1 and c-2. The inmates were ordered by staff to lay on the ground and the inmates would not comply. At 10:50 a.m., flash bangs and chemical munitions were deployed in the gym to take control of the dangerous situation. All inmates were then flexed cuffed and decontaminated by health services. Body checks were conducted inmates were then locked down. There was no staff injuries to report and the institution is still on lock down status.

At approximately 12:30p.m., A fight broke out on the recreation yard, among several inmates. The was no contraband found but several inmates were blooded. There was no staff injuries reported.

USP- CANAAN- JANUARY 13, 2010
 
A call for assistance was announced at approximately 11:15 am. A fight broke out on the compound in front of the housing units 2 inmates on 1. One inmate was stabbed multiple times. The inmate that was stabbed was taken by ambulances to outside hospital. There was no staff injuries reported.

A fight broke out in the recreation yard between internal gangs. Staff fired two percussion grandes to quell the fight. The institution was placed on lock down status until 01-13-10.

FCI- MIAMI- JANUARY 12, 2010
 
At approximately 12:50a.mm, a radio call came from an officer, stating that he needed a couple of officers to respond to his housing unit. An inmate was beginning to get angry and acted like he wanted to fight. As officers tried to restrain the inmate the inmate became combative and swung at one of the officers. The officers took down the inmate and placed him in arm restraints. The inmate was placed in SHU. There was no staff injuries reported.  

MCFP- SPRINGFIELD- JANUARY 12, 2010
 
At approximately 5:15p.m., in the upstairs TV room on ward 10-G, two inmates were watching TV. Another inmate came in and turned the channel without asking the others. The channel changer and one of the inmates watching TV had words. The one inmate went to his room and then came back and hit the channel changer in the side of his head. The channel changer ran out of the room and the other inmate that had been watching TV with the first inmate, came to the aid of the channel changer and hit the first inmate. The inmates began fighting. The first inmate eventually goes back to his room and the second inmate follows him and again begins to fight with him. All three inmates were locked up. The channel changer just had some red marks on his face where he got hit. The first inmate did not get injured and the second inmate had 2-3 small lacerations to the head and a dislocated thumb.

FCI- EL RENO- JANUARY 11, 2010
 
At approximately 6:00p.m., there was an assault on the recreation yard. An inmate who had just arrived on the bus was assaulted. He was transported to a local hospital and is currently in the I.C.U. There were no weapons noted that were used in the assault. There were no staff injuries in the assault. The institution was placed on lockdown and the lockdown was lifted on Jan 12, 2010 at approximately 9:30 AM

USP- LEWISBURG- JANUARY 11, 2010
 
At 8:37a.m., an assistance call was made in the west recreation yard. Two inmates were attacking another inmates. The inmates were separated, medically assessed, and returned to their cells. There was no staff injuries reported.

**At 9:09a.m., a body alarm was sounded in the Z block Recreation area because an inmate assaulted staff. Responding staff restrained the inmate and he was placed into 4 point restraints. The staff member received minor injuries to his face.

***At 12:33p.m., a body alarm was hit in the G block third floor because an inmate assaulted staff. The inmate was placed into ambulatory restraints. There was no other staff injuries reported.

FCI- TEXARKANA- JANUARY 9, 2010
 
During the evening watch, the D-unit officer was making rounds and observed an inmate in the D-2 restroom covered in blood. It appeared the inmate was kicked in the facial area several times and his facial area was also smashed using a hard object. His bones from his face were broken in several different places that the inmate was transported two hours from the institution to a specialist. There was no staff were injured.

At approximately 11:30a.m., there was a fight in the segregation unit between two inmates who were celled together. It was an isolated incident and no staff were injured.

USP- CANAAN- JANUARY 6, 2010
 
At 7:10a.m., an inmate could not clear a medal detector at which time staff started to pat search the inmate. He became combative he was placed on the ground and was found to have a shank hide in the sleeve of his jacket. The inmate was placed in special housing unit. There was no staff injuries reported.

FCC- COLEMAN USP 2- JANUARY 6, 2010
 
An inmate was walking beside the operations lieutenant and grabbed her butt. Other staff members responded and placed the inmate in SHU.

**At approximately 12:30 p.m., lock down was again ordered due to fences going down and inoperable. The stun lethal fence at both institutions has gone down on a daily basis , thus interrupting the daily running of the institution. Programs, recreation and other activities have been shut down due to fence problems. Which, in turn, creates a hostile work environment for our officers who have to deal with irate inmates who are confined to there unit without any activities to keep them occupied.

At 3:35p.m., an assistance call was made for a cell fight in E Block. Both inmates were examined by medical staff. There was no staff injuries reported.

FCC- COLEMAN- USP 2- JANUARY 5, 2010
 
At approximately 9:30 p.m., staff called for compound to stop by H-2 unit because an inmate refused to step into his cell for lock down. The compound officer arrived and pat searched the inmate. As inmate was going into his cell he turned around and slapped the officer in the face. The officer immediately wrestled with the inmate and placed him in hand restraints.

FCC- COLEMAN- MEDIUM- JANUARY 5, 2010
 

The FCI Medium is currently locked down due to a fight among two rival gangs. A total of 8 inmates were fighting in front of food service for the evening meal. Two of the inmates were sent to local hospital with serious stab wounds.

FCI- TEXARKANA- JANUARY 4, 2010
 
At 7:20 a.m., an inmate was assaulted by another inmate in a housing unit due to a gambling bet. R & D staff heard some loud noises above their work area and notified the Lt's office.  Lt's responded to the area where the noise was heard and found one inmate laying on his bunk covered up with his sheets. The inmate was kicked in the facial and upper body area. The institution was on lock down until inmates were counted and interviewed. The institution lock down was lifted at 10:30a.m.

FCI- BENNETTSVILLE- JANUARY 4, 2010
 
At 12:10 p.m. a fight occurred on the recreation yard involving six inmates on two inmates, with weapons involved. Two inmates were sent to an outside hospital for injuries. One inmate had multiple stab wounds and the second  inmate had head trauma and a broken nose and jaw.

At 9:00p.m., an assistance call was made for a cell fight in the special housing unit. Both inmates were examined by medical staff. There was no staff injuries reported.
