MCC- NEW YORK- AUGUST 30, 2010
 
A staff member was assaulted while uncuffing an inmate inside of the recreation cage in SHU. The inmate pulled the officers arm through the slot which caused the officer hand to be injured. The officer was taken to the hospital. The inmate was placed in a cell.

At 7:25a.m., a cell fight involving two inmates occurred. Responding staff were able to separate the 2 inmates. There were no staff injuries to report.

At 8:01a.m., assistance called was made for in the recreation cages for an assault. Staff had to use less than lethal munitions to gain control of the situations and to separate the inmates. There were no staff injuries to report.

At approximately 7:55p.m., an officer announce a fight on the recreation yard. Two inmates attacked one inmate. The fight ended before staff arrived. Upper body checks were initiated, and three inmates were found to have markings consistent with a fight. The inmates were medically assessed and taken to SHU.

At 5:43 p.m., assistance was called for a cell fight involving two inmates. Responding staff were able to separate the 2 without the use of less than lethal. There were not staff injuries to report.

At approximately 11:30a.m., a female staff member was assaulted by an inmate while standing in the mainline inside the food service. The male inmate put his hand on her leg. The inmate was escorted to SHU without further incident.

FCI- TALLADEGA- AUGUST 24, 2010
 
An officer got choked out by a SMU inmate. There were two inmates in the cell with eight staff members prepared to go conduct a force cell move. The inmate that choked the staff member out was lying on his bunk and all of a sudden attacked the officer. The officer was sent to the Emergency Room.

At 8:20p.m., a medical emergency was announced in the laundry area. An inmate was bleeding from the neck area. Staff responded and locked down the laundry area. The inmate had been stabbed in the neck area numerous times by another inmate. The inmate was transported to an outside hospital were he was later pronounced dead from his injuries. Institution was placed on lock down the FBI was notified . There were no staff injuries to report.

At approximately 12:15p.m., the control center announced a body alarm in a housing unit. Three (3) inmates were involved. It was a gang related incident. There were no staff injuries to report.

At 7:19a.m., a fight was announced in food service. An inmate on inmate fight occurred. Staff separated both inmates and escorted them to SHU. There were no staff injuries to report.

At 9:44a.m., a fight was announced in SHU. An inmate on inmate cell fight occurred. There were no staff injuries to report.

At 7:05a.m., a fight was announced on the compound. It was an inmate on inmate fight. Staff arrived and separated the inmates. The inmates were escorted to SHU. There were no staff injuries to report.

At 12:22p.m., a staff member was assaulted by an inmate. The inmate spit on a staff member. Staff responded and the inmate was restrained.

At 7:50a.m., a fight was announced in SHU. Two inmates were fighting in a cell. Both inmates were separated. There were no staff injuries to report.

At 1:34p.m., staff called for assistance for a two on one assault in the recreation cages. Responding staff were able to restrain and separate the three inmates and return them to their cells. There were no staff injuries to report.

At 9:18a.m., staff called for assistance for a fight involving three inmates inside a recreation cage. Responding staff were able to separate the inmates and return them to their cells with no further incidents. There were no staff injuries to report.

FCC- COLEMAN USP 2- AUGUST 16, 2010
 
An officer was escorting an inmate through the scan machine to the strip search area when the inmate became aggressive. The inmate struck the officer one in the face and scratched him in the face. The officer is okay just a little sore.

At 3:30p.m., three gang members assaulted one rival gang member. All three of the inmates were placed in SHU. There were no staff injuries to report.

At 8:45a.m., a body alarm was announced in the R&D area. A female inmate attacked a staff member. The inmate’s shoulder bumped the officer so the officer pushed the inmate to the wall to gain control . The inmate was able to scratch the officer in the eye and face. The inmate continued to fight with responding staff, a staff member received injuries to her arm while fight with the inmate. The inmate was placed on the floor and placed in handcuffs.

A fight occurred with 25-35 inmates involved in a housing unit. This fight was a gang related fight. There were no staff injuries reported.

At 12:36p.m., staff called for assistance for a cell fight. Staff members were able to separate the two inmates and place them into different cells.

USP- POLLOCK- AUGUST 13, 2010
 
A staff member was assaulted in his office by an inmate. Other inmates helped restrain the assaulting inmates until enough staff arrived to gain control of the inmate. The staff member was assaulted pretty bad but was not hospitalized.

At 8:20a.m., officers called for assistance for a fight involving two inmates in the recreation cages. Staff separated the inmates and escorted them back to their block.

At 4:28p.m., a cell fight was announced in SHU. It was an inmate on inmate fight. Both inmates were separated without further incidents. There were no staff injuries to report.

At 8:41a.m., an assistance call was made for a fight in the recreation cages. Four inmates were in the cage although it seems only two inmates were involved. Staff was able to separate the inmates and all four were escorted back into their unit. There were no staff injuries to report.
**At 5:34p.m., staff responded to a call for assistance for a cell fight. Responding staff were able to restrain and separate the two inmates. There were no staff injuries to report.

USP- BIG SANDY- AUGUST 10, 2010
 
A fight occurred in the dinning hall and inmate was stabbed numerous times. Approximately one minute later another assault occurred in a housing unit. One inmate was transported to the local hospital. The institution was locked down for one day.

A body alarm was activated due to a fight in progress on the recreation yard. Responding staff isolated the incident and the inmates suspected of being involved were removed from the area. There were approximately 75 inmates involved. There were no staff injuries to report.

An officer was assaulted by an inmate . The officer hit his body alarm and continued to defend himself until enough staff arrived to control the inmate. The officer was taken to the hospital for stitches.

USP- BIG SANDY- AUGUST 8, 2010
 
An inmate assault occurred in a housing unit. There were two inmates on one inmate. The inmate was beat with a lock in a sock. The inmate was transported to the local hospital.

FCI- TALLADEGA- AUGUST 5, 2010
 
TALLADEGA — An inmate at Talladega’s Federal Correctional Institute was pronounced dead early Thursday morning after being involved in an altercation with another inmate Wednesday.

Bobby Wayne Cowley, 32, “was pronounced dead at a local hospital” at 3:40 a.m., according to Charles Sansom, executive assistant to the warden at FCI.

Cowley was serving a 60-month sentence for possession of a firearm in furtherance of drug trafficking,” he said. “The next of kin have been notified (and) the death is currently under investigation.”

Cowley was sentenced to prison by the federal court for the Northern District of Texas. According to the online federal inmate locator, he was due to be released April 9, 2012.

Sansom declined to comment on Cowley’s cause of death or any details of the altercation leading up to it.

At approximately 11:00 am, while working in SMU an officer asked another officer to do a round on a range. The officer arrived at a cell and a inmate was yelling that his cell mate needed medical attention. The officer looked into the cell and observed the inmate on the floor with blood surrounding him. The inmate was then taken down for medical assistance.

FCI- GREENVILLE- AUGUST 4, 2010
 
At 7:00p.m., approximately 40 inmates were involved in a fight in the recreation yard. There were no staff injuries to report. At least one inmate suffered from puncture wounds to the abdomen. The institution is locked-down and SIS is reviewing the video. The word is we will be conducting mass interviews and shakedowns.

At 10:50 a.m. a call for staff assistance was announced in SHU. An inmate attempted to assault a staff member by spitting on him at which time the staff member took control of the inmate and placed him on the floor till more staff arrived on the scene. There were no staff injuries to report.
 
**At 11:05 a.m., a fight was announced in SHU. It was an inmate on inmate fight in a recreational cage. Both of the inmates were separated without further incident. There were no staff injuries to report.

At 10:58a.m., staff responded to an assistance call for two inmates fighting in their cell. Responding staff were able to separate the two inmates and place them in separate cells.

FCI- HERLONG- JULY 31, 2010
 
At approximately 4:10p.m., the Lieutenant announced a cell fight in R&D, after just arriving at the institution from a bus run. Two inmates of different gang affiliation were fighting. The inmates were separated, medically assessed, and escorted to SHU.

USP- ATWATER- JULY 31, 2010
 
At 7:11p.m., there was a use of force team assembled in the special housing unit, due to two inmates hitting a officer with the food trays and holding the trap. The officer was medically assessed and only had minor bruising to his hand and abdomen. There were nine use of force team members due to there were three inmates in the cell. There were no other staff injured. The inmates were placed in precautionary restraints due to the inmates refusing to submit to hand restraints.

A large fight broke out between gang rival inmates. More than a hundred inmates were involved and some used weapons. Lethal munitions were used in order to get the inmates under control. All inmates were placed under control and put back into their cells without any further incidents. There were no staff injuries to report.

The institution has had three gang related confrontations involving over 200 inmates each time. The first incident was at 6:30p.m. on July 29, 2010. The second confrontation was at 9:00a.m. on July 30, 3010. Followed by another one third evening watch on July 30, 2010. There were no staff injuries to report.

At 11:30a.m., a fight was announced in an unit with weapons involved. Three inmates were airlifted to the trauma center. The institution was placed on lock-down status.

At 7:54a.m., staff called for assistance for a two on one assault outside recreation cages. There were no weapons involved. Responding staff removed the two assailants. The victim was taken to an outside hospital. There were no staff injuries to report.

FCI- HERLONG- JULY 27, 2010
 
At approximately 6:50a.m., an inmate in SHU broke the fire sprinkler head in his cell.

**At approximately 2:00p.m., SHU announced a fight in a cell. One inmate attacked his cell mate striking him about the face and body with closed fists. When the inmate was ordered to cuff up the attacking inmate complied. The inmate victim had to be taken via ambulance to the hospital due to broken ribs and a punctured lung.

At 9:14a.m., a call for assistance was announced for a cell fight. Staff separated the two inmates and placed them in separate cells. One of the inmates refused staffs orders to relinquish to the restraints. There were no injuries to staff.

USP- CANAAN- JULY 24, 2010
 
At 8:40p.m., a fight was announced in a housing unit. An inmate threw a hot substance into the face of another inmate causing burns. He then continued to punch and kick the inmate who was down on the ground. Two more inmates became involved in the fight and one inmate had a shank and stabbed the inmate that was burned multiple times in shoulder area. All of the inmates were separated and the housing unit was placed on lock down. There were no staff injuries to report.

At 12:00p.m., an assistance call was made for a cell fight. It was a cell fight involving two inmates. Gas was used to gain compliance of the inmates. There were no staff injuries to report.

USP- TUCSON- JULY 22, 2010
 
At approximately 9:00p.m., another officer was assaulted by an inmate. The inmate attempted to strangle the officer. The institution did not lock down. The officer is back at work.

USP- LEWISBURG- JULY 21, 2010
 
At 6:30p.m, a use of force team was assembled for a calculated use of force for 2 inmates who were barricaded in their cell. Chemical agents were utilized and staff was able to go in and retrieve the inmates. There were no staff injuries to report.

FCC- OAKDALE- JULY 20, 2010
 
An inmate in a shower requested to speak with the Lieutenant. The inmate then struck the Lieutenant in the left front pocket with a shank. The Lieutenant had a note pad in the pocket so the shank didn’t puncture the skin. The inmate was placed in SMU.

FCI- HERLONG- JULY 20, 2010
 
At approximately 9:50p.m., a unit officer was locking down his unit when he came across an inmate whose head was covered in blood and was acting incoherent. This inmate also had other markings consistent with fight. The officer called the Operations Lieutenant, and the inmate was escorted to the Lieutenants office. Upon review of the video, it showed the inmate was assaulted by two inmates. The assaulted inmate then staggered into his cell. Another inmate then went in after him and came out approximately 2 minuets later. The inmate was medically assessed then taken by ambulance to an outside hospital due to extensive head trauma.

At 8:04a.m., staff responded to a block inside the recreation area for a fight involving three inmates. Staff members were able to separate the three inmates and remove them from the area without further incident.
**At 8:15a.m., staff responded to the recreation yard for a fight involving two inmates. Officers had to use gas, a pepper ball gun, and a flash bang to get the inmates to stop fighting and submit to hand restraints. Staff then separated the two inmates. There were no staff injuries to report.

At 5:54p.m., assistance was called by the control center for a cell fight involving two inmates. Responding staff applied hand restraints and separated the inmates without further incident. There were no staff injuries to report.

At 1:26p.m., a fight was announced in SHU. It was a one on one inmate cell fight and both inmates were separated. There were no staff injuries to report.
 
**At 3:51p.m., another fight was announced in a SHU recreation cage. It was also an inmate on inmate fight and both inmates were separated. There were no staff injuries to report.

USP- TERRE HAUTE- JULY 18, 2010
 
An inmate on inmate assault with weapons occurred in a housing unit. One inmate was sent to a local hospital for treatment of the stab wounds.

USP- TUCSON- JULY 16, 2010
 
At approximately 6:15a.m., staff responded to a body alarm in the housing unit. The female officer in the unit was attacked by an inmate in a unit offericer's office and he attempted to strangle her. She was separated from her body alarm. She screamed and other inmates in the unit responded to help her and got the inmate who was attacking her away from her. The officer is back at work.

At 4:55p.m., a fight was announced in a housing unit. It was an inmate on inmate fight and both inmates were placed in SHU. There were no staff injuries to report.

**At 5:47p.m., a cell fight was announced in SHU. It was an inmate on inmate fight and both inmates were separated. There were no staff injuries to report.

An inmate assaulted another inmate in a housing unit by throwing liquid on him.

At 7:37a.m., the control center announced assistance was needed in an outside recreation cage for a fight involving three inmates. Two inmates attacked one inmate. When staff responded, the inmates ceased fighting and allowed staff to restrain them. The three inmates were removed from the cage, medically assessed and returned to their cells.

**At 9:00a.m., staff responded to an assistance call for a cell fight involving two inmates. Staff applied hand restraints and the inmates were separated.

***At 10:39a.m., staff responded to an assistance call for a cell fight involving two inmates. Staff applied restraints and separated the inmates.

USP- ALLENWOOD- JULY 13, 2010
 
An employee at was blindsided by an inmate as he was walking through a doorway and punched in the face. The employee received minor injuries. The inmates was then put in restraints and placed in SHU. USP-Allenwood had just returned to normal operations from a two week institutional lock-down the day prior after several staff had been assaulted in a pre-meditated incident.

MDCC- NEW YORK- JULY 13, 2010
 
As an inmate was being removed from the strip cage, he became argumentative with the escorting officer and refused to be escorted. The inmate tried to walk past the officer without being escorted and when the officer tried to grab onto the inmate, the inmate elbowed the officer in the rib area. Two other officers then gained control of the inmate and escorted him to his cell without further incident.

At about 5:17p.m., assistance was called for a cell fight with two inmates fighting. Responding staff ordered the inmates to cease fighting and to cuff up. Both inmates submitted, were medically assessed and placed in separate cells. There were no staff injuries to report.

At approximately 9:00a.m., an inmate assaulted a staff member by making inappropriate comment toward the staff member. As staff was telling the inmate to leave the area, the inmate slapped the staff on the hands and left. The inmate was sent to SHU.
**At 10:30a.m., an officer was conducting rounds in the housing unit when he saw an inmate talking on a cell phone. The inmate tried to flush the phone down the toilet but the officer grabbed the phone and wrestled with the inmate. The officer got the room secure when another inmate came and hit the officer in the ribs. The officer then fell to the floor causing him to sprain his ankle.

At 9:43p.m., a fight was announced in a housing unit. There were five inmates fighting. The inmates were hitting each other with chairs. One inmate had a shank and stabbed another inmate multiple times. The unit was placed on lock down status. There were no staff injuries to report.

At approximately 7:50 p.m.., staff called for assistance after observing several inmates fighting with weapons on the softball field.

USP- LEWSIBURG- JULY 6, 2010
 
At 3:10a.m., A cell fight occurred. Both inmates were sent to an outside hospital. One inmate died and one inmate was treated and released.

FCI- HERLONG- JULY 6, 2010
 
At approximately 8:25a.m., the control center announced a body alarm in a housing unit. The unit officer had ordered an inmate out of a cell for a pat search. While attempting to pat search the inmate another inmate ran over and got between the officer and the first inmate. The second inmate then started assaulting the unit officer by pushing him, at which time the first inmate ran away. The first responding staff member saw the first inmate running by him and gave chase into a cell. The first inmate then began assaulting the responding staff member. Enough staff arrived and were able to place both inmates on the ground and in restraints. The
inmates were then escorted out of the unit, medically assessed and placed in SHU.

At 6:08p.m., a fight was announced in a housing unit. Responding staff separated the inmates and escorted them to SHU. There were no staff injuries.

FCI- LORETTO- JULY 6, 2010
 
At approximately 12:04p.m., during the afternoon mean an inmate became disruptive. The inmate refused orders of the staff to be handcuffed and transported to SHU. When staff attempted to physically secure the inmate he became assaultive. After staff obtained control of the inmate, he was placed in restraints and had to be physically carried to SHU by six staff. Some of the staff received abrasions.

At 2:24p.m., control announced a fight in front of a building. One inmate was chasing another with a weapon. Responding staff had to separate and restrain both inmates. Staff recovered the weapon. One inmate was taken to an outside hospital due to his injuries. There were no staff injuries to report.

**At 3:09 p.m., while making rounds in a general housing unit, staff observed what appeared to have been a physical altercation in a cell. Staff secured the cell with three inmates inside. The unit was secured and upper body searches conducted and a victim was identified. Four inmates involved were placed in SHU. There were no staff injuries to report.

***At 8:22p.m., a fight was called outside the dining hall. Responding staff physically separated and restrained the inmates. There were no staff injuries.

Staff discovered an inmate on the recreation yard with head injuries consistent to that of being in a physical altercation. The inmate was sent to an outside hospital due to the extent of his injuries. There were no staff injuries to report.
 
 

FCI- BIG SPRINGS- JULY 2, 2010
 
At approximately 8:30 a.m., a compound officer found blood on the fence from the razor wire to the ground in the recreation yard. A total yard recall was called and emergency count was conducted. All inmates were accounted for, and no injuries were found. At 9:00 a.m., the local police were called, and a test was conducted on the blood. It tested positive for human blood. The blood was tested 6 more times, and in every case was found to be human. The FCI is on lock down until further notice, due to an escape attempt.

At 2:06p.m., a fight was announced in front of a building. Seven inmates were fighting. Responding staff had to physically separate all involved inmates. After this incident three sharpened weapons were taken from these inmates. There were no staff injuries to report.

FCC- TERRE HAUTE- JULY 1, 2010
 
An inmate was placed in the SHU for assaulting a staff member during the conduct of a pat search.
 
**An inmate was pronounced dead at local hospital. The inmate had been in the hospital since his involvement in a physical altercation with another inmate in the FCI SHU on 6/29/10. The inmate was strangled during the altercation.

USP- LEE- JULY 1, 2010
 
At 10:33a.m., a fight was called in special housing. One inmate was assaulting another in a cell. Once sufficient staff were present the door was opened and staff had to separate the inmates. One staff member was injured during the incident requiring treatment at outside hospital.

**At 7:19p.m., a fight was called in a housing unit courtyard between two inmates with weapons involved. Staff separated the inmates and escorted them to SHU. There were no staff injuries to report.

**At 9:03p.m., a fight was called in a housing unit courtyard between two inmates. Staff separated the inmates and escorted them to SHU. There were no staff injuries to report.

FCI- MARIANNA- JULY 1, 2010
 
At 7:25 p.m., a gang related disturbance occurred on the recreation yard. Eleven staff members reported minor injuries. Seven inmates were sent to local hospitals for treatment. One inmate suffers from life threating injuries. The institution was placed on lock down status.

USP- LEE- JUNE 30, 2010
 
At 6:28a.m., an unit officer announced a fight in the housing unit. Three inmates assaulted one inmate. Once enough staff were present the unit was locked down and upper body searches were conducted . One staff member received injuries requiring outside medical treatment.

At 6:19p.m., two inmates attacked one on the recreation yard. The victim was laying on the ground while two assailants were kicking him in the head/face area. The assailants had to be placed on the ground by responding staff and then were restrained. There were no staff injuries to report.

At 7:10p.m., a fight was announced in a housing unit. Three inmates were fighting with each other. They were separated and secured in cells. After responding staff arrived they were restrained and taken to SHU. There were no staff injuries to report.

At approximately 7:20a.m., an officer announced a fight in a SHU cell. One inmate was restrained, the other was not. The unrestrained inmate was hitting the restrained inmate with closed fists about the head and body. When sufficient staff arrived for immediate use of force the restrained inmate was unconscious on the floor of the cell. The cell door was opened and the attacking inmate was placed on the floor and restrained. The inmates were separated and medically assessed.

At approximately 6:55p.m., officer’s were escorting two inmates to the institution’s library. Once the inmates were placed inside the library law and the door was shut, one inmate slipped off his handcuffs and started assaulting the other inmate.

At approximately 12:43 p.m., an unit officer observed an inmate covered with blood in the common area of the housing unit. The Unit Officer immediately called for assistance, and responding staff began securing inmates in their cells. Medical staff arrived and escorted the inmate to the Heath Services Department for medical assessment. The inmate was medically assessed and sustained blunt force trauma to the head. Upper body searches were conducted on all inmates in the housing unit and three inmates were escorted to the Lieutenants Office as possible assailants in the incident. The inmate was transported to the local hospital due to the extent of his injuries. The inmate was pronounced deceased at 3:00 p.m. The FBI was notified and an investigation continues.

At approximately 10:25 a.m., an officer called for assistance after observing an inmate striking another inmate, with his feet and closed fists. Upon the arrival of responding staff, the inmate was observed choking inmate the other inmate. Responding staff separated the inmates and medical assessments were conducted. The assaulted inmate sustained multiple head trauma and bruising on his forehead, temple, and neck. The inmate was transported to the local hospital for further treatment.

An inmate told an officer that he had been assaulted by another inmate. After reviewing video it was determined that the inmate had been in an altercation with another inmate. After being medically assessed the inmate was taken to an outside hospital for more treatment. There were no staff injuries to report.

At 5:14p.m., recreation staff announced a fight on the recreation yard. There were approximately 30 rival gang members engaged in a fight with weapons. Staff orders to stop fighting went unheeded. Chemical munitions were deployed, and approximately 10 minuets later the inmates laid down to be restrained. The inmates were separated, medically assessed and placed in SHU.

At 6:56a.m., control announced a body alarm activation in SHU for a fight in one of the recreation cages. The inmates complied with staff orders to cease fighting and cuffed up. The inmates were separated and medically assessed. There were no staff injuries reported.

At approximately 6:35 a.m., staff observed an inmate being assaulted by two inmates with closed fists and an eight inch homemade knife. The incident was secured by responding staff and all inmates involved were escorted to Health Services for medical assessments. One inmate sustained a puncture wound to his left leg. The other inmates sustained no injuries. All inmates were placed in Administrative Detention.

Approximately 100 inmates were involved. There were no staff were injured. The institution is on locked down status until further notice.

At 3:41p.m., a staff needs assistance call came from a housing unit. Staff witnessed an inmate exit a cell with blood on his clothes and enter another cell. Responding staff secured both the victim and the assailant without incident. There were no staff injuries to report.

At approximately 2:30a.m., an officer in SHU radioed control about two inmates that were intoxicated and fighting in a cell. Multiple orders were given to the inmates to cuff up, but the inmates didn’t comply. The Operations Lieutenant deployed non-lethal munitions at both inmates with negative results. Two use of force teams were assembled, and confrontation avoidance was initiated with negative results. Chemical munitions were again used at 7:45a.m. with negative results. The inmates were given a final order which was refused. At 7:50a.m., the two use of force teams made entry into the cell, restrained and separated the inmates, who were then decontaminated and placed in holding cells for medical evaluations. There were no staff injuries to report.

**At 4:34p.m., there was a fight between two inmates with weapons involved on the compound. The institution was locked down for an emergency count and the following day it was let back up to normal operations.

At approximately 1:15 p.m., staff observed an inmate with visible injuries. Staff also observed a large amount of blood and a homemade weapon on the floor outside of an unit. The unit was immediately secured, and the inmate was escorted to Health Services were he was medically assessed. Due to the severity of his injuries, the inmate was transported to an outside hospital for further medical treatment. A review of the recorded video revealed the inmate was assaulted by a group of inmates with weapons. All of the inmates were sent to SHU.

At approximately 12:41 p.m., staff requested assistance after observing two inmates assaulting one inmate. Responding staff separated the inmates and restrained the inmates without further incident. The assailants were medically assessed and escorted to SHU. The other Inmate was escorted to Health Services where he was treated for a laceration to his right ear and multiple contusions and abrasions to his head and face.

At approximately 3:28 pm, staff observed an inmate assaulting another inmate with a prison-made weapon on the recreation yard. In an effort to prevent serious injury, the tower officer activated the emergency response system which brought an end to the attack. One inmate was placed in restraints and sent to SHU. The other inmate was sent to an outside hospital for the severity of his injuries.

At approximately 7:13 p.m., staff requested assistance after observing several inmates fighting on the recreation yard. One inmate was armed with a lock affixed to a belt. As a result, the Tower Officer activated the emergency announcement system which was deemed ineffective. As a result, one aerial dispersion round was deployed which brought an end to the fighting. The inmates were separated by staff, placed in restraints, and escorted to SHU. No serious injuries were reported.

At approximately 1:20p.m., a call for assistance was called in a recreation area. There was a fight between four inmates containing weapons. One inmate was sent to a hospital for puncture wounds. All other inmates were treated by medical staff and released.

At 3:52p.m., SHU staff called for assistance after two inmates started fighting in their cell. One inmate had stabbed the other. The victim was on the top bunk while the assailant stood in the door and would not show his hands to staff. Chemical munitions had to be used. After the third time munitions were used the assailant then laid on the floor and staff entered the cell and restrained both inmates. There were no staff injuries to report.

**At 10:28p.m., a call for assistance came from a housing unit. Three staff members pulled an inmate form his assigned cell after receiving a drop note that the inmate had a weapon. During the pat search of the inmate the inmate became disruptive when staff discovered the weapon. The inmate tried to maintain possession of the weapon and in doing so injured two staff members. Staff were able to take the weapon before the inmate could assault them. The inmate was restrained and taken to SHU. The two staff members received injuries that required treatment at an outside hospital.

At 6:05p.m., an inmate could not clear a metal detector and an officer attempted to pat search the inmate. The inmate took off running on to the compound. The inmate then threw a shank on the roof of the facility. The inmate was then restrained and escorted to SHU. There were no staff injuries to report.

At 8:28p.m., a fight was announced by control in a housing unit. Staff witnessed one inmate throw a broom at two other inmates. The two retaliated with weapons and assaulted the first inmate. One of the inmates held down the first inmate while the other stabbed him in the head and neck area several times. After responding staff were on the scene the inmates separated and ran back to their assigned cells. While securing the unit staff found the weapon used in a trash can. All involved inmates were taken to SHU. There were no staff injuries to report.

At approximately 7:20a.m., an unit officer observed an inmate bleeding profusely from several wounds on his head. The inmates were secured in their cells while upper body visual searches were conducted. Two inmates were identified as the assailants. Both inmates were examined by Medical staff and placed in SHU. The assaulted inmate was transported to an outside hospital for treatment.

At approximately 10:05a.m., staff requested assistance when an inmate jumped up from the chair during a medical screening in Health Services and attempted to charge toward the examining physician in an aggressive manner. The escorting officer immediately placed the inmate on the floor to gain control of her. Upon arrival of additional staff, the inmate was placed in ambulatory restraints and escorted to SHU where she was medically assessed without further incident.

 

**At approximately 10:55a.m., staff discovered an inmate with his SHU cell window covered. He was also using his mattress to barricade himself in his cell. Confrontation was initiated with negative results. The inmate was given a final order to come to the door and submit to hand restraints with negative results. Two 2-second bursts of OC chemical agent was dispersed from the MK-21 with negative results. The Use of Force Team entered the cell and placed the inmate in hand and leg restraints. He was decontaminated, medically assessed, and placed in ambulatory restraints.

 

**At approximately 11:15a.m., staff observed an inmate smearing feces on the walls of his assigned cell. The inmate refused staff's orders to submit to hand restraints. A Use of Force Team was assembled and confrontation avoidance proved ineffective. The inmate was given a last order to submit to hand restraints, to which he complied. After being placed in an observation cell, the inmate manipulated his hand restraints, moving them from the back to the front. The inmate was then placed in progressive ambulatory restraints. Medical assessments were conducted and an incident report was written.

 

**At approximately 12:00p.m., an officer observed two inmates exit a cell and fall to the floor fighting and punching each other. Staff ordered the inmates to stop fighting and they complied. Responding staff separated the inmates, applied restraints, and escorted them to SHU with no further incident.

 

**At approximately 2:45 p.m., while conducting rounds in SHU, staff requested assistance upon observing an inmate attempting to asphyxiate herself by tying strips of clothing around her neck. The inmate refused staff orders to cease her actions. Responding staff immediately entered the cell, removed the strips of clothing from the inmate’s neck and placed her in ambulatory restraints. The inmate was escorted to Health Services and medically assessed. The inmate sustained a contusion to the left side of the neck. The inmate was subsequently placed on suicide watch.

 

**At approximately 3:59p.m., an inmate was escorted from R&D for placement in his assigned cell, upon which time he was placed in the first floor shower as a temporary holding area. The inmate then stated he would not go into a cell with another inmate and verbalized threats of violence towards any inmate he was placed in a cell with. The Warden was notified and authorization was received to place inmate in ambulatory restraints due to his aggressive and assaultive behavior. At 5:01p.m., confrontational avoidance procedures were initiated with positive results. At 5:23p.m., the inmate was placed into ambulatory restraints. A medical assessment was performed on the inmate with no injuries noted.

 

**At approximately 9:10p.m., the Operation Lieutenant was notified that an altercation between two inmates had occurred in front of a unit. A preliminary investigation revealed that one inmate assaulted the other inmate by ripping a large portion of her hair from her scalp, pulling her head into the bench, and striking her in the face with a closed fist. Both inmates were escorted to health services for a medical assessment and placed in temporary holding pending an investigation.

At approximately 7:49 a.m., staff observed an inmate assaulting another inmate. The assault occurred in a general housing cell. Both inmates were placed in hand restraints and placed in SHU. Both inmates were transported to health services. One inmate had to be sent to an outside hospital for further medical treatment. On June 15, 2010 the inmate was pronounced dead at the outside hospital due to his injuries.

At approximately 8:30 a.m., SHU staff observed several inmates striking each other with closed fists in the recreation cage. Responding staff ordered the inmates to stop and submit to restraints. The inmates complied and were removed from the recreation cage without further incident. All three inmates were medically assessed and returned to their assigned cells. An investigation continues.
 
**At approximately 12:40 p.m., staff in SHU observed several inmates assaulting another inmate. Specifically, staff observed one inmate strike another inmate with a closed fist to the facial area, which caused the inmate to fall to the ground. Two inmates then began to strike and kick the inmate with closed fists and their feet. Staff also observed another inmate stomp the inmate in the face/head area. All inmates submitted to hand restraints and were removed from the cage. The other inmate was unable to stand or submit to hand restraints, so staff entered the cage and he was restrained and placed in a wheelchair. He was escorted to the Health Services Unit, where he was treated for large contusions on the right side of his face and head, multiple small abrasions on his scalp and knees, and a laceration on the left side of his upper lip. The inmate was escorted back to SHU without incident. The other inmates were treated by the on-duty Registered Nurse and sustained no injuries. One staff member sustained minor injuries as a result of the incident.
 
**At 1:04 p.m., an inmate refused to submit to the application of hand restraints to be removed from the Special Management cell and be placed in SHU. The Warden was notified and authorized the use of calculated force to remove the inmate from the cell. At 1:09 p.m., after confrontation avoidance proved ineffective, the Lieutenant introduced the Use of Force Team to the cell. Upon staff entering the cell, the inmate jumped on the back of the a team member and struck him in the lower right jaw. The team was able to gain control of the inmate and place him in restraints. The inmate was escorted via van to the SHU and medically assessed. The inmate received contusions of the face, neck and scalp requiring minor first aid. The restraints were removed from the inmate and the team departed the cell without further incident. The staff member was medically assessed and received a small laceration to his lower lip requiring no medical attention.
 
**An inmate was observed in his cell with a homemade noose placed around his neck. The inmate was ordered to submit to restraints and moved to an area where he could remain under constant staff supervision. Psychology was notified and conducted an assessment. The determination was made that the inmate had no suicidal ideation but was attempting to achieve secondary gain. The inmate was subsequently placed in soft ambulatory restraints without incident.

**At approximately 1:50 p.m., an inmate approached the Unit Officer and informed him that he could not be in the unit anymore. The Unit Officer called for the Compound Officer to escort the inmate to the Lieutenant's Office. The inmate told the Lieutenant he had just been released from SHU to general population, and upon arriving to the Unit, he was disciplined by a gang. The inmate was medically assessed and placed in SHU without further incident.

**At approximately 3:15 p.m., an inmate, approached an Unit Officer and reported that he had been assaulted. The inmate was secured and escorted to the Lieutenant's Office. The inmate was medically assessed, treated, and placed in SHU without further incident.

**At approximately 7:15 p.m., Recreation staff called for assistance on the recreation yard after observing several inmates fighting by the basketball court. The Tower Officer activated the amplified verbal warning system and made an announcement on the megaphone at which time, the inmates disregarded. The Tower Officer fired two less-lethal rounds, at which time the inmates complied with staff orders. The inmates were then placed in hand restraints and escorted to SHU without further incidents. Medical assessments were conducted on the inmates sustaining minor injuries. One staff member sustained minor injuries as a result of this incident.

**At 7:55 p.m., a SHU Officer activated his body alarm when he observed an inmate, cutting himself on the arm with a sprinkler head he had broken. Responding staff ordered the inmate to submit to hand restraints, to which he complied. Staff escorted the inmate for a medical assessment, psychological evaluation, interview, and photographs. He was secured in SHU without further incident.

**At approximately 7:58 p.m., an Officer announced a fight on the recreation yard. Two inmates involved in a physical altercation. Responding staff escorted both inmates to the Lieutenants' Office. The inmates were medically assessed and placed in SHU without any further incident.

**At approximately 11:30 a.m., while staff were delivering food trays to inmates in SHU. Staff observed both inmates striking each other with closed fists in the chest and upper body areas. Staff immediately gave the inmates direct orders to stop fighting, in which they complied and submitted to hand restraints. Both inmates were medically assessed and placed in SHU.

**At approximately 11:37 p.m., an unit officer observed an inmate cutting himself on the right arm. The officer called for assistance and responding staff reported to the cell. Upon staff observation that the inmate was physically harming himself, immediate Use of Force was used to control the inmate. The inmate was medically assessed and place on suicide watch without further incident.

At 2:29 P.M., the Unit Officer responded to a duress alarm. While investigating the duress alarm, the unit Officer observed two inmates striking each other with their fist to the upper torso, head, and facial area. The unit Officer notified control of the fight and location via radio, then gave both inmates a direct order to cease their actions. Responding staff placed the inmates in hand restraints and escorted them to Health Services where they were treated for minor injuries.
 
**At approximately 7:05 p.m, a call for assistance was announced in Health Services. Specifically, staff observed an inmate take his left fist and hit another inmate on the left temple. Staff ordered the inmate to cease his combative actions, and he complied. The inmates were submitted to hand restraints and were separated. The aforementioned inmates were placed in the Administrative Detention pending investigation.
 
***At approximately 10:18 p.m., the Control Center announced a fight in an Unit, after the Unit Officer activated his body alarm. Specifically, the Unit Officer heard noise coming from a cell. He responded to the cell and observed an inmate strike another inmate, in the face with a fist. The inmate retaliated striking the other inmate in the back of the neck with his fist. The Officer gave several orders for the inmates to stop fighting, to which the inmates complied. Responding staff separated the inmates, placed them in hand restraints and escorted them to SHU. The inmates weremedically assessed, photographed, interviewed, and secured in SHU without further incident.
 
****At approximately 10:20 p.m., an Unit Officer activated their body alarm in response to two inmates fighting in the TV room. Two inmates were observed exchanging punches with closed fists. The Officer ordered both inmates to separate but with negative results. Upon the arrival of additional staff, both inmates were separated. Both inmates were medically assessed and escorted to SHU without further incident.

At approximately 11:17 am, two inmatesbegan striking another inmate with closed fists to the head and upper torso, after he was placed in the SHU recreation cage. The inmates did not comply with staff's orders to stop and get on the ground. Once enough responding staff were present the recreation door was opened and all three inmates were separated, placed on the ground, and restrained. The inmates were medically assessed and placed back in their cells without further incident.
 
**At 9:03 p.m., the Unit Officer announced a fight in the unit after observing several inmates fighting with other inmates. Responding staff observed the inmates involved in the fight striking one another in the head and torso areas with sharpened metal prison made weapons and combination locks attached to institution issued nylon belts. The inmates did not comply to the direct orders given by staff to cease their actions, and the Operations Lieutenant authorized the deployment of less-lethal munitions in the Unit. A total of 22 inmates were placed in SHU. One inmate was transported to an outside hospital for treatment due to the severity of his injuries.

At approximately 3:23p.m., a call for assistance was made by an officer who observed staff attempting to secure an inmate during the recall move for the north side. Specifically the officer was monitoring the recall move for the north side when an inmate attempted to enter the recreation yard, an officer stopped inmate The officer instructed him to submit to a pat search, once the officer began the pat search of the inmate, he felt something in his left pant pocket, he asked the what was in his pocket, the inmate replied, my glasses". The inmate then turned and attempted to flee from the officer. The officer with the assistance of another officer placed the inmate on the ground and applied restraints. Another officer assisted the Compound officers and applied leg irons to the inmate. A search of the inmate’s pocket revealed a 6 inches long black hard plastic homemade weapon, sharpened on one end and wrapped on the other end with black shoe string for a lanyard. The inmate was escorted to health services for a medical assessment and escorted to the special housing unit without further incident. An investigation continues.

At approximately 8:30p.m., a fight was announced in a housing unit between multiple inmates with weapons. This was a gang related incident between approximately 5 different gangs and approximately 18 inmates were involved. One inmate was taken to the outside hospital due to having multiple puncture wounds. There were no staff injuries and the institution was placed on lock down until after the holiday to be re-evaluated once the administration gets off of their holiday break.

An officer was assaulted by an inmate. The inmate punched the officer in the neck. The inmate was restrained and placed in SHU. The officer was not seriously injured.

At approximately 1:55 p.m., five inmates escorted aninmate to the Compound Officer. The inmates told the inmate to "check in". The inmate refused and pulled out what appeared to be a homemade weapon while stepping towards the inmates. Staff called forassistance, and the inmate ran from the scene. While staff were following the inmate, he turned towards staff with weapon in hand and began to run. Staff gave the inmate several orders to cease his actions, to which he complied and laid on the ground. Hand restraints were applied. The inmate was removed from the area. He was medically assessed and placed in SHU without further incident. Staff conducted a search of the direct area of the yard, the weapon was not recovered.

An inmate in SHU assaulted staff on two occasions by throwing a liquid substance on them. The inmate was placed on a sack lunch for 5 days for his continued disruptive behavior which continued throughout the shift.

Eight inmates assaulted another inmate in a housing unit at the USP. During the response by staff, one of the inmates bit a case manager on the finger. The inmate who was assaulted was taken to the local hospital for treatment of his injuries.

At approximately 6:54 p.m., the Center Hall Officercalled for assistance as an inmate ran past Center Hall towards the Rear Corridor. The Rear Corridor Officer approached the inmate and observed a combination lock attached to a belt in the inmates right hand. The Rear Corridor Officer ordered the inmate to drop the weapon. The inmate refused staff orders and was placed on the ground by responding staff. Hand restraints were applied and the inmate was subsequently escorted to Health Services where an injury assessment was completed. The inmate was noted to have received no injuries during the incident. The inmate was escorted from Health Services to SHU where he awaits the disciplinary process.
 

At approximately 7:20 p.m., staff monitoring the Visiting Room surveillance cameras, observed a female visitor place an item into a potato chip bag and give the bag to the inmate who swallowed the contents of the bag. The inmate was removed from the Visiting Room and placed in Dry Cell status. Local authorities were notified and the female visitor consented to a search of her vehicle. During the subsequent search of the vehicle, an orange colored balloon containing marijuana was located and an unmarked container containing numerous Xanax tablets were found. The female visitor was arrested and taken into custody by the local police department. On May 29, 2010, the inmate provided a stool sample which contained one orange colored balloon containing four Xanax tablets. The FBI was notified and an investigation continues.

At approximately 2:20p.m., staff observed an inmateacting as if he was intoxicated. Staff called for compound officers to escort him to the Lieutenants' Office. As staff escorted the inmate he became argumentative and began to pull away from staff. The inmate attempted to aggressively turn on staff. Staff placed the inmate on the ground to gain control of his disruptive behavior. Responding staff applied leg restraints, placed the inmate in a wheelchair, and escorted him to the Lieutenants' Office. The inmate was breathalyzed, medically assessed, and admitted into SHU without further incident.  The FBI was notified and an investigation continues.
 

**At approximately 2:30p.m., staff was conducting linen exchange in SHU. As staff approached and opened the food slot to exchange linen with an inmate the inmate became argumentative with staff. The inmate then placed his arm in the food slot and refused staff orders to remove it. When staff attempted to secure the food slot, the inmate struck the staff member in the forearm with an unknown object that he had in his right hand. Staff immediately gained control of the inmate=s arm and placed it back in the cell. The food slot was secured without further incident. Both staff and inmate were medically assessed. The F.B.I. was notified and the investigation continues.

They were escorting an inmate back into his cell when the inmate jerked away from the officer as the officer attempted to uncuff him thru the tray slot, therefore breaking the cuff key off inside the handcuffs. Gas had to be used on this inmate to get him to comply with orders from staff.

A staff member was attempting to restrain a disruptive inmate. The inmate became combative and resisted against being restrained. The inmate turned around on the staff member catching the staff members hand in the cuffs and began running away. A call for assistance was initiated and responding staff were able to subdue the inmate after a short chase. The inmate was placed on the ground and restrained. The inmate was moved to the SHU. The staff member was treated at a local hospital and suffered injuries to his hand requiring it to be placed in a cast.

There were no staff injuries to report. The institution was placed on lock down status.

At approximately 6:25a.m., an unidentified inmate entered into a housing unit. He was dressed with gloves and a mask. He proceeded to assault another inmate with a pipe. Responding staff arrived and medical staff began working on the inmate. The inmate had been stabbed several times and was transported to a local hospital by an ambulance. The inmate was pronounced dead later in the morning. The institution was placed on lock down status. The FBI was called and an investigation continues. There were no staff injuries to report.

The unit officer was inspecting the inmates radio and the inmate lunged for the item, knocking the radio out of the officer's hands. The inmate was placed on the ground with the assistance of another unit officer. The inmate had hidden a 2.5 inch razor blade from a utility type knife, inside his radio holder. The inmate was medically assessed and escorted to the Special Housing Unit without further incident.

At approximately 9:00p.m., while the unit officer was locking down the unit for the 9:30pm count he heard a shuffle below him. He noticed several inmates acting strangely and called for staff to assist him in locking down. Through the video subservience system it was identified that 2 inmates were in a heated argument and were pushing each other. One inmate went back to his cell and grabbed what appeared to be a weapon. When responding staff arrived the inmate got rid of the weapon. Both inmates went to SHU with no staff injuries.

At 9:08a.m., a fight was announced on the recreation yard. Four inmates were involved in a fight. Three inmates jumped on one. Two towers fired less lethal munitions with negative results. Responding staff physically separated the inmates. There was no staff injuries to report.

At approximately 4:50p.m., an officer stopped an inmate who was failing to identify himself to the officer. The inmate began combative when he turned and swung an arm at the officer. The inmate then took a combative stance and jumped as if he were going to strike the officer. A call for assistance was initiated. The inmate then assaulted the staff member and was taken to the ground. This was the same officer who was assaulted the day before. The officer sustained minor injuries, again. The inmate was identified and placed in SHU. The unit was placed on lock down until interviews could be conducted the following day.

At approximately 2:00p.m., a staff member was assaulted by an inmate when he attempted to pat the inmate down. The inmate was restrained and escorted to SHU. The officer was not injured.

At approximately 4:50 p.m., an officer was making rounds through a housing unit. He came upon an inmate in a cell acting suspiciously. When he attempted to search the inmate, the inmate became combative and attempted to flee the cell. The inmate assaulted the officer and managed to get out of the cell and throw something to another inmate. The officer initiated a call for assistance. Both inmates were identified and restrained by responding staff. Marijuana was found in the area where the assault occurred. The staff member received minor injuries and continued to work through his shift. The unit was placed on lock down until interviews could be conducted the following day.

The assailants refused staff orders to stop and had to be physically removed from the inmate being attacked. All three inmates were placed in SHU after this incident. At approximately 9:00p.m., an inmate came into an unit officers office with a bloody face and other marks consistent with a fight saying he needed to "check in." Upper body checks were initiated and another inmate was found to have markings consistent with a fight as well. Upon review of video a third inmate was identified as an attacker. Three inmates were escorted to the Special Housing Unit without further incident. This was gang related due to the victim not wanting to hang
out with a particular gang anymore.

One Staff member injured his back while trying to restrain one of the inmates.

After hearing rumors of a food strike, staff started conducting interviews with inmates. This escalated into an inmate curing at staff. Staff members attempted to detain the inmate when another inmate got involved and a physical confrontation began between staff and two inmates. The institution was placed on lock down status. At 6:30p.m., an inmate was assaulted on the recreation yard. The inmate was medically assessed and placed in SHU.

At 3:21p.m., a fight with weapons was announced between two groups of gangs. There was no staff injuries reported. There was 3 inmates involved in the altercation.

**At 4:30p.m., there was a fight between approximately 30 inmates with weapons involved. The institution went on lock down and one inmate went to the local hospital for his injuries. There was no staff injuries reported. The next day the institution went back to normal since it was a isolated incident.

At approximately 7:50a.m., the unit officer asked an inmate to clean his room and told him that he needed to start taking showers. The inmate squared off on the officer and then assaulted him, hitting him in the face with his fist causing a scratch on the officer's nose. A body alarm was activated and staff responded. The inmate was taken to lockup and was not injured.

Staff arrived secured the area locked the unit down and and escorted the five inmates to SHU. The other inmate was escorted to medical and sent out to a local hospital. ***At 8:12 p.m,. an immediate use of force was used in Special Housing Unit during the move because an inmate assaulted a staff member. One staff member broke his ankle and leg when the inmate bucked and landed on the officers leg. The staff member was sent to outside hospital for treatment.

At 1:30p.m., an intoxicated inmate from another unit went into an officer’s unit and hit her in the head area several times. After she was hit she backed up and hit her body alarm then put distance between her and the inmate until additional staff arrives to help her. The inmate then went after the responding staff. The staff member received a slight concussion and is doing fine. The inmate was moved to another institution. The institution was also placed on lock-down status until futher notice.

An officer was conducting a shake down of an inmate’s locker. While shaking down, the officer found a leafy substance in an envelope. The inmate arrived at the area and made a lunging gesture towards the officer in an attempt to grab the substance. The officer was required to use physical force to move the inmate away from him as he activated his body alarm. The inmate was placed in the Special Housing Unit. There was no known injury that occurred to the staff member.

At 2:00p.m., the tower officer advised that a group of gang rival inmates appeared to be squaring off against each other on the rec yard. The compound officer requested additional staff to report to the rec yard. Responding staff observed an inmate bleeding from the face area. He was immediately escorted off of the rec yard to health services. A review of the Vicon security camera revealed one inmate striking another inmate with closed fist punches and knee kicks to his upper torso and face area. The inmate was medically assessed and escorted to SHU without incident.

**At 09:10p.m., an officer was searching an intoxicated inmate in the Lieutenants complex. The inmate became disruptive and struck the officer with his elbow. The officer received a contusion on his arm.

At approximately 8:32 p.m., an officer requested additional staff report to K-Unit. The officer observed an inmate lunge towards him in front of the upper tier officers station. The officer perceived the inmate lunged at him with a weapon after he heard something he perceived as metallic fall to the lower tier. The inmate’s attempt to assault the officer failed when another inmate stepped between them and wrapped his arms around the inmate preventing the assault. The inmate then escorted the other inmate back to his cell. Responding staff secured the unit as other inmates within the unit were loud, unruly and resentful of being locked down. The inmate was removed from the cell, medically examined, and placed in the Special Housing Unit.

At approximately 11:30 a.m., the e-1 unit officer notified the operations lieutenant of a possible fight in his housing unit. The unit was lock down and upper body searches were conducted. Two inmates were found to have injuries both inmates were taken for medical assessments. At 1:15 pm one the inmates was taken to outside local hospital for injuries he sustained from the fight. There was no staff injuries to report.

At 11:10 p.m., an inmate in SHU covered his window and broke the sprinkler head in his cell. As a SHU officer was attempting to clear the water from in front of the cell door, the inmate threw a liquid substance which smelled of feces on him. It hit the officer in his arm and leg.

At 05:27 p.m., a fight was announced in A-2 Unit. Several inmates were fighting and staff responded. As an officer was attempting to pat search an inmate on the top tier of the unit, the inmate became combative and grabbed the officer by the arm. The inmate then pushed the officer towards the top tier railing, while simultaneously reaching for a weapon which was hidden in his waistband. Chemical agents were utilized on the inmate to stop the assault, and the inmate was then restrained and the weapon recovered.

At 3:44 PM a unit officer announced a fight in his unit with weapons involved. As responding staff entered the unit the assault stopped and the inmate was secured in a tv room. The victim was restrained and removed from the unit to the unit team area. The victim was then transported by stretcher to the health services area. At 4:21 PM the victim was transported to an outside hospital where he was pronounced dead at 4:55 PM. Institution remains on lock down status pending outcome of mass interviews.

At approximately 9:30a.m., an unit officer was monitoring the movement when he told an inmate he cannot enter his unit. The inmate quickly jumped on the staff and they were wrestling to the ground. The officer was taken to the hospital and pending more information. Another staff member received head injuries as well. He was the first responder and he suffered some type of head trauma. The officer was also sent to hospital.

At approximately 10:35 a.m., 200 hundred inmates that were in the gymnasium were observed grouping up by staff. These inmates were placed in the gym due mass shake downs being conducted in c-1 and c-2. The inmates were ordered by staff to lay on the ground and the inmates would not comply. At 10:50 a.m., flash bangs and chemical munitions were deployed in the gym to take control of the dangerous situation. All inmates were then flexed cuffed and decontaminated by health services. Body checks were conducted inmates were then locked down. There was no staff injuries to report and the institution is still on lock down status.

At approximately 12:50a.mm, a radio call came from an officer, stating that he needed a couple of officers to respond to his housing unit. An inmate was beginning to get angry and acted like he wanted to fight. As officers tried to restrain the inmate the inmate became combative and swung at one of the officers. The officers took down the inmate and placed him in arm restraints. The inmate was placed in SHU. There was no staff injuries reported.

At approximately 5:15p.m., in the upstairs TV room on ward 10-G, two inmates were watching TV. Another inmate came in and turned the channel without asking the others. The channel changer and one of the inmates watching TV had words. The one inmate went to his room and then came back and hit the channel changer in the side of his head. The channel changer ran out of the room and the other inmate that had been watching TV with the first inmate, came to the aid of the channel changer and hit the first inmate. The inmates began fighting. The first inmate eventually goes back to his room and the second inmate follows him and again begins to fight with him. All three inmates were locked up. The channel changer just had some red marks on his face where he got hit. The first inmate did not get injured and the second inmate had 2-3 small lacerations to the head and a dislocated thumb.

At approximately 6:00p.m., there was an assault on the recreation yard. An inmate who had just arrived on the bus was assaulted. He was transported to a local hospital and is currently in the I.C.U. There were no weapons noted that were used in the assault. There were no staff injuries in the assault. The institution was placed on lockdown and the lockdown was lifted on Jan 12, 2010 at approximately 9:30 AM

At 8:37a.m., an assistance call was made in the west recreation yard. Two inmates were attacking another inmates. The inmates were separated, medically assessed, and returned to their cells. There was no staff injuries reported.

**At 9:09a.m., a body alarm was sounded in the Z block Recreation area because an inmate assaulted staff. Responding staff restrained the inmate and he was placed into 4 point restraints. The staff member received minor injuries to his face.

***At 12:33p.m., a body alarm was hit in the G block third floor because an inmate assaulted staff. The inmate was placed into ambulatory restraints. There was no other staff injuries reported.

During the evening watch, the D-unit officer was making rounds and observed an inmate in the D-2 restroom covered in blood. It appeared the inmate was kicked in the facial area several times and his facial area was also smashed using a hard object. His bones from his face were broken in several different places that the inmate was transported two hours from the institution to a specialist. There was no staff were injured.

At approximately 11:30a.m., there was a fight in the segregation unit between two inmates who were celled together. It was an isolated incident and no staff were injured.

He became combative he was placed on the ground and was found to have a shank hide in the sleeve of his jacket. The inmate was placed in special housing unit. There was no staff injuries reported.

An inmate was walking beside the operations lieutenant and grabbed her butt. Other staff members responded and placed the inmate in SHU.

**At approximately 12:30 p.m., lock down was again ordered due to fences going down and inoperable. The stun lethal fence at both institutions has gone down on a daily basis , thus interrupting the daily running of the institution. Programs, recreation and other activities have been shut down due to fence problems. Which, in turn, creates a hostile work environment for our officers who have to deal with irate inmates who are confined to there unit without any activities to keep them occupied.

At 3:35p.m., an assistance call was made for a cell fight in E Block. Both inmates were examined by medical staff. There was no staff injuries reported.

At approximately 9:30 p.m., staff called for compound to stop by H-2 unit because an inmate refused to step into his cell for lock down. The compound officer arrived and pat searched the inmate. As inmate was going into his cell he turned around and slapped the officer in the face. The officer immediately wrestled with the inmate and placed him in hand restraints.

Lt's responded to the area where the noise was heard and found one inmate laying on his bunk covered up with his sheets. The inmate was kicked in the facial and upper body area. The institution was on lock down until inmates were counted and interviewed. The institution lock down was lifted at 10:30a.m.

At 12:10 p.m. a fight occurred on the recreation yard involving six inmates on two inmates, with weapons involved. Two inmates were sent to an outside hospital for injuries. One inmate had multiple stab wounds and the second  inmate had head trauma and a broken nose and jaw.

At 9:00p.m., an assistance call was made for a cell fight in the special housing unit. Both inmates were examined by medical staff. There was no staff injuries reported.
