FCI- EDGEFIELD- DECEMBER 31, 2008
 
A fight broke out between gang members, at approx. 7pm, during the 10 minute move. The number of inmates involved is unknown, but no staff were involved. One inmate was sent to an outside hospital.The FCI is currently on lockdown status, and visitation was suspended. Lockdown termination date is undetermined/undisclosed at this time.

At 1:55 p.m., staff called for assistance in a housing unit due to an inmate being down. Staff arrived in a cell. An inmate was face down in the cell with a pool of blood around his head. Another inmate was standing at the door. The inmate was ordered to submit to hand restraints and complied. The call door was opened and one inmate was escorted off the range. The other inmate was assessed and placed on a gurney and transported to the Health Services Unit. The investigation continues.

At 10:40 a.m., staff observed two inmates striking each other in the facial area with both fists in the slaw library, assistance was called. Both inmates were ordered to the floor but refused. When enough staff arrived the inmates were separated, restrained, and the inmates was escorted to Heath Services without further incident. Both inmates were medically assessed and placed into different cells.  All notifications were made. Incident reports for fighting and refusing orders were initiated.

FCI- FLORENCE- DECEMBER 29, 2008
 
Staff were alerted that an inmate was found dead in his cell.  This incident appears to be a gang related homicide.  The cell area where this ocurred was considered a crime scene.  The unit had been vacted by the Agency during the shift in which this incident occurred.  The institution remains on lockdown, pending investigation.  No staff were injured.

USP- LEE- DECEMBER 28, 2008
 

At approximately 1:05 p.m., recreation staff observed an inmate bleeding from injuries to his head and face.  A review of the video surveillance indicated at 12:49 p.m., while in the gym, two inmates were involved in a physical altercation where both inmates were striking each other with fists to the head and upper torso area.  Both inmates were medically assessed and placed in the SHU without further incident.  One inmate was subsequently transported to the local hospital for treatment of his injuries.  An investigation continues. 

USP- LEE- DECEMBER 27, 2008
 

At approximately 7:07 p.m., Staff announced a fight in a unit, when he observed, two inmates striking each other repeatedly with fists to the head and upper body.  Both inmates ignored staff orders to stop fighting.  Responding staff separated both inmates and placed them on the ground to regain control of the situation.  Both inmates were restrained and escorted out of the unit. The inmates were medically assessed and placed in SHU without further incident.  The investigation continues. 

 

USP- MARION- DECEMBER 27, 2008
 
At approximately 8:02 p.m. an officer was attempting to remove home made alcohol from an inmates locker in the inmates cell.  The inmate then approached the officer and begin arguing with the officier.  As the officer began applying restraints on the inmate, the inmate attempted to strike the officer with his right hand.  Another officer who responded was then able to assist the officer in applying the hand restraints.  As other officers arrived the inmate continued to resist, and then spit in an officer's face. The inmate was then escorted to health services without any further incident.

At approximately 11:30 a.m., an inmate entered the Unit Officer=s station bleeding from his nose and mouth area.  The Unit Officer secured the inmate and notified the Operations Lieutenant.  Upon the arrival of responding staff, the inmates were secured in their cell and upper body searches were conducted.  Staff reviewed recorded video surveillance and identified two inmates as the assailants.   All of the inmates involved were restrained, medically assessed, and admitted into the Special Housing Unit without further incident.  The assaulted inmate was subsequently transported to an outside hospital for treatment due to nature of his injuries. Notifications were made and an investigation continues.

 

**At 2:39 p.m., Recreation staff called for assistance in Recreation upon observing two inmates involved in a physical altercation.  Specifically, staff observed both inmates throwing punches at one another with their fists.  Staff ordered both inmates to cease their disruptive behavior, in which they complied.  Upon the arrival of responding staff, both inmates were restrained, medically assessed, and escorted to the SHU without further incident.  The investigation continues. 

FCI- GREENVILLE- DECEMBER 25, 2008
 
While attempting to place an inmate back into his cell after rec, the inmate became disruptive after noticing that his cell had been searched. The inmate then pulled the Operation Lieutenants hand through the
tray slot cutting the top of his hand.

USP- ATWATER- DECEMBER 24, 2008
 
At approximately 3:00pm all the inmates in the Special housing decided to cover their window to prevent the taking of count. At 3:30pm SHU was unable to conduct count and the inmates refused to uncover their windows. At that point calculated use of force teams were prepared 25 staff involved. They went from cell to cell using gas and sting balls to extract the inmates and place them in ambulatory restraints. There were no serious staff injuries reported. The use of force teams stopped going in on inmates at around 2am in the morning. The good verbal
for the 3:30pm count was cleared at 2:45am on December 25, 2008. The warden refused to activate the DCT Teams for the disturbance and continued to run the institution even though there was not a clear count. Over 35 inmates total were placed in ambulatory restraints. This all started due to the inmates not receiving their Christmas packages.

USP- LEE- DECEMBER 24, 2008
 

At approximately 3:35 p.m., staff observed smoke coming from under a cell.  Staff could not see into the cell due to a towel in the window.  For the inmate safety, staff entered the cell and placed the inmate in hand restraints.  He was escorted to another cell.  A Use of Force Team was authorized to place the inmate into ambulatory restraints due to his disruptive behavior.  Upon the arrival of the Use of Force Team, the inmate was moved to a holding cell to be visually searched and placed into hard ambulatory restraints. The Inmate was medically assessed and notifications were made.

At approximately 8:55 a.m., a housing Unit Officer activated his body alarm after observing an inmate throw a dust pan at another inmate.  The inmate then grabbed a broom handle and took a defensive stance.  Responding staff arrived and both inmates complied with staff orders to submit to restraints.  A review of the video surveillance identified two inmates approaching one inmate from behind and began striking him in the head with closed fists. The two inmates briefly chased the other inmate to the second tier before he (the one being chased) grabbed the broom handle.  One inmate then departed the area.  Upon the arrival of responding staff, the inmates were separated, restrained, and medically assessed.  All of the inmates involved were admitted into the Special Housing Unit without further incident.  The FBI was notified and an investigation continues.

FCI- GREENVILLE- DECEMBER 23, 2008
 
While attempting to restrain an inmate for routine cell
rotations, an inmate pulled a staff members arm through the tray slot, leaving a bruise and knot on the officers arm.

A riot broke out during the pm meal. The riot was due to gang relations. The instutution was placed on lockdown. The food service and lower housing were trashed but no staff was reported injuried. Six inmates were taken out to the hospital, three returned from the hospital. Staff responded from the other institutions during the incident, and a staff recall was initiated.

At 12:20 p.m., staff gave an inmate an order to leave the holding area in Health Services and he refused to comply.  The inmate continued to refuse staff orders to leave.  As staff began to escort the inmate from the Health Services Department, he let his body go limp to impair staff=s ability to continue to escort him.  Staff began to place the inmate back on a bench, he attempted to pull away from escorting staff and was placed against the wall to gain control of his disruptive behavior.  The inmate continued to resist staff by jerking his body away from staff.  The inmate was then placed on the ground to cease his disruptive behavior.  The inmate was restrained and medically assessed.  Due to the inmate’s behavior and statements he had made, the Psychology Department was notified.  The inmate was then placed on suicide watch.

At 12:35 p.m., staff observed an inmate striking another inmate with his fist to the head and upper torso area in a cell in a Housing Unit.  Staff radioed for assistance and ordered the inmate to cease his actions in which he refused to comply.  Upon the arrival of responding staff, the cell door was opened and staff restrained both inmates without further incident.  Both inmates were medically assessed and placed into different cells.  The FBI was notified and an investigation continues.  

 

**At  7:16 p.m., staff observed two inmates striking one another with their fists in the facial area in a cell.  Staff radioed for assistance and ordered both inmates to cease their disruptive behavior, in which they refused to comply.  Upon the arrival of responding staff, the cell door was ordered open.  Staff had to physically separate both inmates and had to place one inmate on the lower bunk to gain control of his actions.  Both inmates were separated, restrained, medically assessed, and placed into different cells without further incident.    

FCI- GREENVILLE- DECEMBER 20, 2008
 
While feeding the unit their noon meal in Special Housing, a Staff memeber was assaulted by an inmate throwing a cup of unknown liquid substance striking the officer in the right leg and upper torso.

FCI- OTISVILLE- DECEMBER 19, 2008
 
While clearing the 300 range dorm in an unit. A staff member observed an inmate run into the bathroom. The staff member followed the inmate into the bathroom and observed him attempt to flush items down the toilet. When staff attempted to retrieve the item the inmate turned around and placed his hands on his upper torso and pushed him against the wall causing his right elbow to strike the wall. The Inmate was restrained and placed in SHU.

At approximately 5:38 p.m. an inmate was escorted to the Lieutenant=s Office to be visually searched.  The inmate refused to comply with staff orders and refused to submit to restraints.  As staff attempted to apply the restraints, the inmate attempted to strike one staff member in the face with his fist.  At this time, another staff member radioed for assistance and placed the inmate on the ground.  While staff was attempting to gain control of the inmate, he struck staff with his elbow, grabbed the staff member=s arm, and attempted to grab his groin area.  Upon the arrival of responding staff, the inmate was restrained and escorted to the Special Housing Unit without further incident.  The inmate and the staff members involved were medically assessed. The FBI was notified and an investigation continues.

FCC- COLEMAN USP 2- DECEMBER 17, 2008
 
During Morning Meal an Officer was shaking down with the Lt. by his side, while attempting to shake down an inmate the inmate turned around and punched the officer in the chest and face, both individuals were fighting on the hallway floor until assistance arrived. The officer is doing fine with no major injuries to report.

At 12:20 p.m., staff observed an inmate at the laundry exchange window speaking loudly in Spanish and waving his arms around.  Staff ordered him to exit laundry, in which he complied.  As staff was securing the door to laundry, the inmate attempted to reopen the door and then slapped a staff member across the hand.  Staff called for assistance and the inmate was placed on the floor to gain control of his disruptive behavior. The inmate was restrained, medically assessed, and placed in the Special Housing Unit without further incident.  The FBI was notified and an investigation continues.

 

**At  2:00 p.m., staff ordered an inmate to submit to hand restraints to be placed into a different cell, in which he would not comply.  A Use of Force Team was authorized to place the inmate into ambulatory restraints due to his disruptive behavior.  Upon the arrival of the Use of Force Team, the inmate submitted to restraints.

 

***At 5:57 p.m., as staff walked by a range holding cell in a housing Unit, they observed an inmate striking another inmate with his fists to the upper body area.  Staff radioed for assistance and ordered both inmates to separate and lay down on the floor, in which they complied.  Responding staff restrained both inmates and removed them from the holding cell without further incident.  Both inmates were medically assessed and placed into different cells.  The FBI was notified and an investigation continues.

 

****At 7:00 a.m., an Officer radioed for assistance upon observing two inmates involved in a physical altercation.  Specifically, staff observed an inmate with another inmate in a choke hold while attempting to throw him off his back.  Upon the arrival of responding staff, both inmates were separated, restrained, and escorted to Health Services for medical assessments. One responding staff member was medically assessed and received a minor injury responding to the incident.  Both inmates were admitted into the Special Housing Unit without further incident.  An investigation continues.

 

 

 

USP- LEE- DECEMEBER 15, 2008
 

At approximately 11:42 a.m., staff called for assistance upon observing two inmates fighting in the building courtyard.  Specifically, staff observed one inmate striking the other inmate in the face and upper body area with his fists.  While the one getting hit grabbed the other around the waist and wrestled him to the ground.  Responding staff separated, restrained, and medically assessed.  Both inmates were escorted to the Special Housing Unit without further incident.

At approximately 3:05 a.m., while staff conducted the 3:00 a.m. count in a Housing Unit, they approached a cell, which is occupied by two inmates.  Staff observed both inmates had injuries consistent with a physical altercation.  The Operations Lieutenant was notified and reported to the Special Housing Unit. Both inmates submitted to hand restraints and were removed from the cell without incident.  Both inmates were medically assessed and placed into different cells. An investigation continues.

 

**At 6:11 p.m., a housing unit Officer called for assistance upon observing two inmates striking another inmate with their fists to the head and upper body.  Upon the arrival of responding staff, the inmates were separated, restrained, and medically assessed.  All of the inmates involved were admitted into the Special Housing Unit without further incident.  The FBI was notified and an investigation continues.

At approximately 7:05 p.m., a Unit Officer called for assistance upon observing an inmate laying on the floor of a cell, with blood on his head and face.  Another inmate was standing at the sink in the cell washing blood from his hands.  Upon the arrival of responding staff, both inmates were restrained, medically assessed, and admitted into the Special Housing Unit  without further incident.  The FBI was notified and an investigation continues.

MCFP- SPRINGFIELD- DECEMBER 13, 2008
 
An inmate used his wheel chair to break out his cell windows.  The flying glass cut the face of two staff members.  Both staff member went downtown to the local hospital.

At approximately 2:19 p.m., a housing Unit Officer called for assistance upon observing an inmate assaulting another inmate in the sallyport of the unit.  Specifically, one inmate struck the other inmate with his fist to the upper body and a struggle ensued.  Staff ordered both inmates to cease their actions, in which they refused to comply.  Upon the arrival of responding staff, both inmates were placed on the ground to gain control of their disruptive behavior. Both inmates were separated, restrained, and medically assessed.

FMC- LEXINGTON- DECEMBER 12, 2008
 
At approximately 12:00a.m., 3 Staff were sent to a Housing Unit to conduct a pat search on and inmate who was suspected of having a cell phone.  During the pat search a cell phone was discovered.  The inmate started to run from Staff and during the scuffle one Staff Member hurt their right wrist.  Inmate was placed in SHU. No other Staff injuries were reported.

At approximately 9:50 a.m., a Calculated Use of Force Team was authorized due to two inmates refusing staff orders to submit to restraints in order to facilitate a 21 day cell rotation in the a housing Unit.  Upon arrival to the cell confrontation avoidance procedures were initiated and proved successful.  Both inmates submitted to restraints and were removed from the cell.  Both inmates were placed into hard ambulatory restraints due to their disruptive behavior and medically assessed.

At 6:20a.m., a body alarm was activated in inmate laundry.  As officers proceeded to the inmate laundry to see an inmate who was in the center of the laundry room, thrusting his arms toward another inmate, as if he was attempting to stab the other inmate. The defending inmate had a plastic, government issued chair in his possession, attempting to keep inmate away from him. As officers approached the inmate from behind, the Laundry Officer stated that the inmate had a weapon in his possession. An officer then restrained the inmate. The weapon was retrieved by the Laundry Officer and the inmate was subsequently placed in hand restraints. An officer sustained a small puncture wound to his middle finger of his left hand from the weapon when he restrained the inmate. Health Services conducted a  medical assessment,  the wound was cleaned, and a tetanus shot was administered.

At 11:30 p.m, an inmate being escorted in the Special Housing Unit was able to slip out of his handcuffs.  The inmate then assaulted and officer by punching him directly in the face.  The inmate was then restraint with no further incident.  The officer received minor injury.

At approximately 9:02 p.m., a housing Unit Officer called for assistance upon observing two inmates involved in a physical altercation in the common area of the Unit.  Specifically, staff observed two inmates striking each another in the head and upper torso with their fists.  Staff ordered the inmates to cease their actions, in which they refused to comply.  Upon the arrival of responding staff, both inmates were separated, restrained, and medically assessed.  Both inmates were placed in the Special Housing Unit without further incident.  

 

**At 11:40 a.m., staff was picking up food trays on a range in a Housing Unit. After retrieving the food trays from one cell an inmate placed his arm outside the wicket.  After being ordered to remove his arm, the inmate was ordered to submit to hand restraints.  Upon placing the restraints on his right wrist, the inmate attempted to pull the restraints into his cell.  Staff restrained the inmate’s right arm and removed the restraints.  The inmates arm was placed back into the cell and the wicket was secured.

At approxmately 6:10am, an inmate fight occured in a unit between two inmates with weapons. One inmate was stabbed several time. No staff injuries were reported.
 
**At approxmately 7:10am, an inmate fight
broke out between three inmates. Weapons were involved, two locks on a belt were used. No staff injuries were reported.
 
 

FCC- FORREST CITY MEDIUM- DECEMBER 9, 2008
 
During noon meal, A fight/assault occured in front of commissary, at least 4 inmates were sent to shu. No staff injuries were reported.

At approximately 3:22 p.m., the Control Center announced a body alarm in a Unit due to an inmate assault in progress.  Specifically, as staff opened the front door to the Unit to allow an inmate entrance into the unit, an inmate approached him and began striking the other inmate in the face and head with his fists.  Staff ordered him to cease his assault and he complied.  Responding staff restrained both inmates and removed them from the unit.  Both inmates were medically assessed and placed in the Special Housing Unit without further incident.  The FBI was notified and an investigation continues.

FCI- OTISVILLE-  DECEMBER 8, 2008
 
Around 6:45a.m., a fight between two gangs started in the dining hall. This was followed by a body alarm in a housing unit, then shortly after another body alarm in another unit.  The results of these fights was that three inmates were sent out to outside Hospitals, one was flown to by helicopter. One Inamte returned around 3pm and there were no reports on the other two.
 
 

FCC- FORREST CITY MEDIUM- DECEMBER 8, 2008
 
An inmate on inmate assault occured in housing unit. During the lunch meal, an inmate assaulted another outside of food service starting several other fights, at least 10 inmates were sent to shu. A bus of new inmates arrived at 10pm, then a fight in r&d occured at least 3 inmates involved. No staff injuries were reported.

At approximately 11:25 a.m., Housing staff called for assistance upon observing an assault in a cell.  Specifically, staff observed one inmate striking another inmate in the head and body with a closed fist.  Staff ordered the inmate to cease his actions, in which he refused to comply.  Upon the arrival of responding staff, both inmates were ordered to separate, in which they complied.  Both inmates were restrained, medically assessed, and placed into different cells without further incident.  The FBI was notified and an investigation continues.

USP- LEAVENWORTH- DECEMBER 2, 2008
 
At 12:40am the administration released about ten gang related members from SHU. The staff members knew it was not going to go well so the lieutenants were watching the cameras keeping an eye on rival gang members as they were released into the recreation yard. At 1:03 the rival gang members launched there assault. The tower officer fired four less lethal rounds and the altercation ended. Four inmates ended up with puncture wounds. There were no staff members reported injured. The institution was placed on lock down and the institution plans to send 40 of the gang members out on a bus and everything should reside to normal.

An Officer hit a body alarm in laundry because an inmate was being stabbed, staff respond. Another Officer hit a second body alarm in the food service for another inmate being stabbed, all staff respond.  These incidents are gang related and believes this is not over yet. No staff injuries were reported.
 

FCC- COLEMAN USP 1- DECEMBER 1, 2008
 
At approximately 8:50 PM, during a move from the recreation yard, staff were conducting random pat
searches on the inmates.  Upon searching an inmate  staff felt what appeared to be a homemade weapon in the inmate's right pocket.  At which time the inmate broke away from staff's control.  The inmate was immediately placed on the ground as additional
assistance was called. The Tower Officer activate the emergency announcement system and fired one concussion round from the Remington shotgun.  The inmate was placed in hand restraints as staff confiscated and secured a homemade weapon, approximately 8 ½ inch long, sharpened to a point.   The inmate was medically assessed and escorted to the Special Housing Unit, pending further investigation.

At approximately 3:36 p.m., staff called for assistance in a housing Unit upon observing an inmate striking another inmate to the upper body area with his fist.  Upon the arrival of responding staff, the inmates were separated and restrained.  Both inmates were removed from the holding cell, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

 

** At 11:16 a.m., staff observed an inmate take a razor blade out of his mouth and began to slash his right arm. Staff ordered the inmate to cease his actions, in which he refused to comply. Staff radioed for assistance. Upon the arrival of responding staff, the cell door was ordered open. Staff attempted to secure the razor blade and restrain the inmate as he attempted to bite the Lieutenant in the leg and was grabbing at the Lieutenant's groin area.  Staff secured the weapon and restrained the inmate.  The inmate was moved to the medical treatment room for a medical assessment.  Upon being escorted back to a holding cell, the inmate attempted to break away from staff and was placed on the ground to gain control of his disruptive behavior.  The inmate was then escorted to the holding area without further incident. Psychology staff was notified for an assessment of the inmate. The FBI was notified and an investigation continues.

At approximately 5:38 p.m., staff called for assistance in a housing Unit upon observing two inmates involved in a physical altercation. Specifically, staff observed two inmates striking one another with their fists to the head and upper torso areas. Staff ordered both inmates to cease their actions, in which they complied. Upon the arrival of responding staff, both inmates were restrained, medically assessed, and escorted to the Special Housing Unit without further incident.

USP- LEE- NOVEMBER 29, 2008

At approximately 11:55 a.m., a housing Unit Officer secured a cell due to detecting an odor of homemade intoxicants. An inmate then approached the officer and asked to allow him to get his jacket out of that cell, which was his assigned living quarters. The officer allowed the inmate into the cell to retrieve his coat and at this time inmate the inmate attempted to dispose of the intoxicants he had hidden inside his cell by pouring the liquid into the toilet. The officer gave the inmate a direct order to stop his actions. At this time, the inmate attempted to flee the cell, striking the officer with his left shoulder. As the officer began to place the inmate on the wall, they both fell to the floor. The inmate got to his feet and ran to the upper tier. The officer radioed for assistance and followed the inmate to the upper tier shower where he dumped the intoxicants and attempted to push past the officer again. The officer ordered the inmate to lie face down on the floor, in which he complied. Upon the arrival of responding staff, the inmate was restrained and escorted from the unit. The FBI was notified and an investigation continues.

USP- LEE- NOVEMBER 28, 2008

At approximately 8:55 p.m., a housing Unit Officer called for compound to come to the Unit, due to observing an inmate with blood on his face and clothes. Staff arrived at the Unit and escorted the inmate from the Unit. At approximately 9:00 p.m., another Unit (different Unit) Officer called for compound to come over to his Unit upon observing two inmates bleeding from the head. As staff began to restrain inmate one of the inmates he started screaming and attempted to strike staff with a fist. Staff immediately placed the inmate to the ground, where he continued to resist. As staff were attempting to place the inmate in restraints, an unidentified inmate threw a plastic bowl from the top tier, striking staff. As the Lieutenant ordered the less-lethal munitions be brought to the unit, another inmate came running down the stairs and attempted to intervene. The inmate was immediately placed on the ground, where he continued to resist staff. Other inmates started to run down the upper tier towards the stairs in an attempt to get involved, but stopped when met with responding staff. As staff arrived with the Less Than Lethal munitions, the inmates started returning to their cells. At this time, an inmate exited his assigned cell, and refused to return. Staff approached him with the L8 and gave him several orders to return to his cell, in which he refused to comply. Staff then fired one ALS foam baton round, striking the inmate in the torso, in an attempt to gain his compliance. The inmate continued to refuse to return to his cell until a second and third round was discharged, also striking him in the torso. The remainder of the unit was secured without further incident. All inmates involved were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 3:45 p.m., an Offier called for assistance after observing two inmates involved in a physical altercation.  At approximately 4:30 p.m., a review of the security system was initiated.  Inmate #1 was observed initiating the altercation by striking inmate #2 with a Homemade weapon in front of a cell that was adjacent to the inmate telephones.  Inmate #2 was observed responding to the attack by pulling his own homemade weapon and striking inmate #1with the weapon.  Approximately, thirty seconds into the altercation, inmate #3, inmate #1 cell mate, was observed with another homemade
weapon striking inmate #2 in the back.  The altercation continued with all three inmates continuing to throw weapon strikes until responding staff entered the unit. All three inmates were observed returning to their cells. Subsequent physical examinations of the three inmates revealed inmate #2 suffered several puncture and lacerations to his head, neck chest and back.  Inmate #1 suffered a laceration to the right side of his neck.  Inmate #3 received no injury in the altercations. Inmate #2 was taken to Local Hospital for additional treatment

At approximately 2:59pm, during open movement, a staff member called for assistance in the main corridor when he observed inmate #1 and inmate #2 hitting each other with closed fists. Upon staff arrival, both inmates were separated, restrained and medical assessments accomplished.

At approximately 9:00 p.m., staff called for assistance in a housing Unit upon observing two inmates involved in a physical altercation. Specifically, staff observed two inmates striking one another with their fists to the head and upper torso areas. Staff ordered both inmates to cease their actions, in which they refused to comply. Upon the arrival of responding staff, both inmates were again ordered to cease their actions and get on the ground, in which they complied. Staff separated and restrained both inmates. Upon conducting a pat search of one of the inmates, staff discovered a combination lock attached to a belt. Both inmates were removed from the unit, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

alarm hit in the housing unit There were two inmates housed in the same cell involved in a fight. Minor injuries were sustained by both inmates.  None of the responding staff were injured.

Upon the arrival of responding staff, both inmates had to be physically separated and placed on the ground to gain control of their disruptive behavior. Both inmates were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. An investigation continues.

At 3:30p.m., a fight broke out with three inmates. The three inmates had weapons that left lotz of blood on the floor of the unit.  No injuries of staff were reported.

At 5:50 p.m., recreation staff radioed for assistance on the Recreation Yard upon observing an inmate being assaulted by two inmates. Specifically, They punched the first inmate repeatedly with their fists, to the head and upper torso area, knocking him to the ground and continuing their assault. The automated voice system was activated, ordering all inmates to get down on the ground. The inmates refused to comply with the order and continued their assault on the inmate. Perceiving a threat to responding staff and inmates, two Tower Officers fired a total of six ALS hard bang rounds from their less lethal 12 gauge shotguns. Upon the arrival of responding staff, the inmates involved had to be physically separated and placed on the ground to gain control of their disruptive behavior. All inmates were restrained and escorted from the Recreation yard.

**At 6:14 p.m. and in the exact same location, another inmate was assaulted by two additional inmates. Specifically in this incident, two inmates punched another inmate repeatedly to the head with their fists. The automated voice system was again activated, ordering all inmates to get down on the ground. Three Tower Officers fired a total of five ALS hard bang rounds to gain control of their disruptive behavior. Responding staff had to physically separate and place the inmates on the ground to gain control of their disruptive behavior. The inmates were restrained and escorted from the Recreation Yard. All inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

USP- LEE- NOVEMBER 22, 2008

At approximately 5:58 p.m. a Unit officer called for assistance upon observing an inmate staggering up the unit stairwell, bleeding from the right side of his face. Responding staff restrained the inmate and escorted him to Health Services for a medical assessment. The inmate smelled of a strong odor of intoxicants and refused staff orders to submit to a breathalyser. Video surveillance was reviewed and revealed that the inmate had been assaulted in another Unit building courtyard by two inmates. Specifically, the two inmates began striking the first inmate with their fists to the head and upper torso area. As the first inmate fell to the ground, they continued their assault by kicking him to the head and upper torso area. All of the inmates were medically assessed and placed into the Special Housing Unit without further incident. The FBI was notified and the investigation continues.

At approximately 10:20 a.m., an Officer was opening the block after the 10:00 a.m. count and observed injuries on the inmates in a cell.  Both inmates were subsequently interviewed, medically assessed, and placed into Administrative Detention. No other injuries have been reported.

An inmate gang related disturbance occured. The Agency was forced to disburse gas cannisters in order to get this situation under control.  Multiple inmates were injured and had to be treated for injuries sustained in the disturbance.  Several staff responders were injured while responding.

At approximately 8:00 am, the Special Housing Unit staff were removing an inmate from his cell, to move him to the shower.  After being placed in handcuffs from behind, the cell door was opened. Immediately upon the door being opened, the inmate turned and lunged at an officer, spitting body fluids in his face. The inmate was immediately controlled by staff and placed in restraints.  The inmate continued to be disruptive, yelling profanities at staff and flailing with resistance while being escorted to a cell. After being placed in additional restraints the inmate continued to yell profanities at staff. While being medically assessed the inmate answered the medical staff with profanities also. No staff received injuries during this action.

Upon the arrival of responding staff, the inmates ceased their disruptive behavior. An inmate feigned a seizure, when the officer went to assist him, the inmate agressively attacked him and punched the officer in the face. The Inmate was then restrained and placed in the Special Housing Unit.

Staff ordered the window to be uncovered, in which the inmates complied. As the window was being uncovered, they observed two inmates striking one another with their fists to the head and upper body area. Upon the arrival of responding staff, the inmates were ordered to cease their actions and submit to restraints, in which they complied. Both inmates were medically assessed and placed into different cells. An investigation continues.

Specifically, staff observed two inmates striking one another in the head and upper body areas with their closed fists. Both inmates were ordered to cease their disruptive behavior, in which they refused to comply. Upon the arrival of responding staff, the cell door was ordered open. Staff had to physically separate and place both inmates on the ground to gain control of their disruptive behavior. Both inmates were restrained, medically assessed, and placed into different cells without further incident. An investigation continues.

At approximately 8:14am, a fight between three inmates  broke out . One inmate was severely stabbed in the head, neck and both lungs and was sent out to a Trama Hospital. No staff injuries to report. U.S.P. Big Sandy is locked down, as of November 14, 2008.

At approximately 12:26 p.m., the Control Center announced staff needs assistance on the recreation yard. Specifically, staff observed an inmate assaulting another inmate by striking him in the facial area with his right fist. One inmate fell to the ground and inmate other inmate began striking him several times to the head and upper body with his left knee. Staff ordered inmate the to cease his actions in which he refused to comply. Two Tower Officers dispensed two ALS Hard Bang rounds each, perceiving a threat to the safety of responding staff and inmates. Responding staff placed both inmates on the ground. Both inmates were restrained, medically assessed, and placed into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At 5:30 p.m., the Control Center announced staff needs assistance on the recreation yard volleyball court. Specifically, staff observed two inmates striking one another in the face and head area with closed fists. The Tower Officer activated the automated voice annunciator, ordering the inmates to lay down on the ground, in which they refused to comply. Two Tower Officers dispensed a total of five ALS Hard Bang rounds, fearing for the safety of responding staff and inmates. As responding staff arrived, both inmates complied with staff orders to get on the ground. Both inmates were medically assessed and placed into the Special Housing Unit without further incident. An investigation continues.

USP LEWISBURG-  NOVEMBER 13, 2008
 
At approximately 10:25 a.m., inmate #1 reported to an officer with what appeared to be numerous stab wounds.  A review of video surveillance revealed at 10:19 a.m., inmate #2 stabs inmate #1 with a homemade weapon on the stairwell.  Inmates #3, and #4 join inmate #2 and chase inmate #1 to the third floor  and continue to stab inmate #1 numerous times.  Inmate #1 was able to flee from the three assailants and alert the block officer.  Inmate #1 sustained numerous puncture wounds resulting in a punctured lung and was transported to a local hospital for further treatment.  An investigation continues.

At approxamently 7:05pm, there was a fist fight involving 2 inmates. One inmate was knocked out cold and possesed a
6" inch medal weapon (Knife). No staff injuries to report.

A gang related disturbance broke out.  Numerous inmates were reported injured.  Inmates were breaking glass out of windows to use for weapons. This  Institution was placed on lockdown.

At approximately 7:10 a.m., while staff were conducting a pat search on an inmate, he turned in an aggressive manner and struck staff in the chest area. Staff called for assistance and inmate was placed on the ground, restrained, and was taken to Health Services. An injury assessment was conducted on inmate and he did not sustain any injuries. The Lieutenant sustained an abrasion to the left hand the officer sustained pain to the left shoulder.

At approximately 6:20 p.m., while the Special Housing Unit staff was observing inmates on the recreation yard, an inmate threw an unknown liquid substance, striking the Officer in the upper left side shoulder and chest area of his body.  The inmate and the staff member were medically assessed. The inmate was placed in hand restraints, and will remain in the Special Housing Unit pending further investigation.

At approximately 11:40 a.m., an Officer observed inmate #1, with a cut on the bridge of his nose. The officer called for additional staff and responding staff secured the inmate. Staff searching inmate #1 and found a 6 inch plastic homemade weapon tied to the front of his pants. Video surveillance, combined with inmate interviews, revealed inmate #2, punched inmate #1 in the face.

At approximately 6:55 p.m. an inmate, was escorted to the Special Housing Unit for the use of intoxicants. As staff began to place the inmate into the recreation cage, he turned toward staff in an aggressive manner. Specifically, the inmate lunged towards staff in an attempt to knock them down. The inmate was then taken to the ground by staff to gain control of his disruptive behavior. FBI was notified and the investigation continues.

FCI- PEKIN- NOVEMBER 9,2008
 
FCI Pekin is on lockdown due to a gang related disturbance in three different areas of the institution on Nov 9.  No staff injuries are reported.  300-400 inmates involved in this disturbance. One inmate was reprted to be in ICU.

USP- TERRE HAUTE- NOVEMBER 7, 2008
 
Two staff members were assaulted.  An inmate became disruptive and punched a female staff member, knocking her unconscious.  Another staff member responded, and he was also knocked unconscious by the inmate.  The inmate was then restrained by other staff.  Both staff were taken to the local hospital, treated and released. 

USP- MARION- NOVEMEBR 4, 2008
 
An officer observed two inmates fighting adjacent to the corridor steps. He announced the fight over the radio.  Soon after the fight was in progress 4 other inmates jointed in and all 5 inmates where kicking and punching the one inmate several times before staff
could respond and break up the fight. The 5 inmates are gang suspects. All inmates where taken to the institutional hospital for medical evaluation and then were taken to SHU.  No staff injuries were reported.

At approximately 12:06 p.m., the Control Center announced staff needs assistance near the outside of the Corridor entrance. Specifically, staff observed an inmate strike another inmate to the right side of the head with a fist. Responding staff separated and restrained the inmates. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 12:55 p.m., staff radioed for assistance in a housing unit upon observing two inmates in a cell engaging in a physical altercation. One inmate was holding the other inmate in a side head lock, who was also bleeding from the facial area. Upon the arrival of responding staff, the inmates were separated, restrained, medically assessed and admitted to the SHU without further incident. The FBI was notified and an investigation continues.
 
**At 4:57 p.m., the Control Center announced staff needs assistance in a housing Unit. As staff responded to the unit, they began securing the inmates into their assigned cells. One inmate, was still in the shower after all other inmates had been secured in their cells. Staff ordered the inmate to stop his shower and return to his cell. The inmate became belligerent as he was being escorted to his cell. Upon arriving at his cell, he cursed at staff and slammed the cell door. Based on the inmates actions during an institution emergency, staff opened the unsecured door and ordered the inmate to submit to restraints. The inmate again became belligerent and as staff attempted to place him in restraints, he turned and attempted to strike staff with his elbow. Staff immediately placed the inmate onto his bunk and applied restraints. The inmate continued to resist staff until hand and leg restraints were applied. The inmate was medically assessed and admitted into the Special Housing Unit without further incident.
 
***At 7:43 p.m., an inmate, was observed by Compound Officers exiting a unit after the movement had ended. Staff ordered the inmate to return to his unit and he refused, continuing to walk towards the exit gate. Staff repeatedly ordered the inmate to stop, which he eventually complied. As staff were attempting to perform a pat search on Stoddard he spun towards the officer. Immediate force was utilized and was placed on the ground. The inmate continued to struggle until responding staff placed him in restraints. The inmate was medically assessed and placed into Administrative Detention.

At approximately 11:08 a.m., staff in the Special Housing Unit heard noises consistent with a physical altercation coming from a cell on the range. Upon arrival at the cell staff observed two inmates grappling in the cell’s floor. SHU staff radioed for assistance. Specifically, staff observed one inmate on top of another inmate holding him down. Both inmates were ordered to cease their actions and submit to hand restraints, in which they refused to comply. Upon the arrival of additional staff, the cell door was opened. Staff had to physically separate both inmates. The inmates were restrained, medically assessed, and placed in separate cells without further incident. The FBI was notified and the investigation continues.

**At 11:25 a.m., staff in the Special Housing Unit radioed for assistance upon observing an inmate assaulting another inmate, in a cell on the range. Specifically, staff heard noises consistent with a physical altercation. Upon arrival to the cell staff observed an inmate on top of the other inmate striking him with his fists to the head and torso area. Both inmates were ordered to cease their actions, in which they refused to comply. Upon the arrival of additional staff, the cell door was opened. Staff had to physically each inmates. Both inmates were restrained, medically assessed, and placed in separate cells without further incident. The FBI was notified and the investigation continues.

At approximately 9:13 a.m., the Special Housing Unit Recreation Officer radioed for assistance upon observing an inmate, assaulting another inmate in a recreation cage. Specifically, one inmate began striking the other inmate in the face and upper torso with his fists. The inmate feel to the ground and inmate attacking inamte continued to assault him by kicking and striking him with his fist in the upper torso area. Both inmates were ordered to cease their actions, in which they complied. Upon the arrival of additional staff, the recreation door was opened. Both inmates were restrained, medically assessed, and placed separate cells without further incident. The FBI was notified and the investigation continues.
 
**At 7:01 p.m., staff radioed for assistance in the  Corridor upon observing one inmate assault another inmate. Upon the arrival of responding staff, the inmates were separated and restrained without further incident. Both inmates were medically assessed and admitted to the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

An inmate brushed an officer because he refused to go through the metal detector. The officer hit his body alarm. No further incident were reported. The inmate was charged with minor assault.

At approximately 1:30  a fight broke out in the yard, it was reported by various staff member that approximately 15 inmates were involved. The fight was gang related. SIS locked up approximately  20-25 members of the gang that caused this fight.

At approximately 5:56 a.m., an Officer called for assistance after observing several inmates involved in an altercation on the first floor range.  The inmates quickly dissipated after given orders to cease their actions.  A review of video surveillance revealed inmate 1, entered a cell where inmate 2 was waiting, and they engaged in a fight.  As inmate 1 exited the cell, inmate 3, assaulted inmate 1 by tackling him and then punching and kicking him in the upper body.  Inmate 2 then exited the cell and began punching inmate 1 with a lock in his clinched hand.  Inmate 4 also attempted to participate in the assault by throwing punches in the direction of inmate 1.  Inmate 1 received minor bumps and bruises  Inmate 2 sustained five puncture wounds to the right leg, two puncture wounds to the left leg, one puncture wound to the left arm, and an abrasion on the abdomen.

At approximately 12:35 p.m., Recreation staff observed an inmate attempting to retrieve something from one of the drains on the Compound. The Compound Officers located the inmate and escorted him to the Lieutenant's Office for a visual search. The inmate refused staff orders to submit to a visual search. Staff then ordered him to submit to hand restraints, at which time he took an aggressive stance towards staff and clenched his fists. Staff immediately called for assistance as the inmate began swinging his fists, striking several staff members. Staff had to physically place the inmate on the ground as he continued to assault staff until he was restrained. The inmate was medically assessed and placed into the Special Housing Unit without further incident. The FBI was notified and the investigation continues.
 
**At 11:18 a.m.,a housing Unit Officer radioed for assistance upon observing two inmates, in a physical altercation with weapons involved. Specifically, one inmate struck the other inmate with a lock attached to a belt to the chest, head, and arms several times. While the other inmate struck back with a closed fist to the head and upper chest area. One inmate was later found to be in possession of a five inch piece of metal sharpened to a point at one end, which he dropped to the floor. Due to the inmates disruptive behavior, staff placed one inmate against the wall and the other inmate on the floor to gain control of their actions. Both inmates were restrained, medically assessed, and placed in the Special Housing Unit without further incident. The FBI was notified and the investigation continues.
 

***At 1:53 p.m., the Control Center announced staff needs assistance in a housing Unit. Specifically, staff observed an inmate with blood covering his face and torso area along with injuries consistent with being involved in a physical altercation. The unit was secured and a review of the VICON recorded video surveillance revealed a fight had taken place between two inmates. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 4:38 p.m., the an Officer called for assistance after observing inmate **** exiting a Block with blood on his head. Inmate **** was escorted to Medical Services by responding staff. Inmate **** admitted to assaulting inmate *****. Inmate **** sustained between fifty and sixty stab wounds to the back from the base of the neck, 5 to 6 stab wounds to the chest, multiple stab wounds to the abdomen, minor stab wounds to both upper arms, and a shallow puncture wound in the left temple area with a goose egg. Inmate **** was transported to a local hospital for further treatment.

At approximately 6:00 p.m., a housing Unit Officer called for assistance upon observing two inmates fighting. Specifically, the inmates were striking one another in the head and chest area with their fists. Upon the arrival of responding staff, both inmates were placed on the ground and restrained. As staff began to move one of the inmates, a homemade ice pick type weapon was discovered lying underneath him. A review of the video surveillance revealed one inmate chasing the other inmate and throwing a liquid substance on him. At this time, it appeared one of the inmates pulled a weapon from his waistband and both inmates began fighting. Both inmates were medically assessed and placed into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 10:30am an inmate grabbed the chain that was attached to the OIC's keys as she was attempting to shut his food slot.  The OIC was able to break free but fell down on the floor. The officer was not injured.

The institution was locked down for several hours due to an institutional emergency in regards to the fight that happened on Tuesday 10-21-08. The ** and the ** wanted to know which inmate gave a weapon to the ** inmates on Tuesday’s assault. Staff rounded up a total of 21 inmates that were ID as ** and ** and escorted to R&D where they were stripped out and placed into restraints (martin chain,cuffs and leg irons) and interviewed by staff. After the interviews the institution was taken off lock down.

At approximately 12:34 p.m., the Control Center announced staff needs assistance in a housing Unit stairwell. Specifically, staff observed one inmate, strike another inmate, in the back of the head with a closed fist. The one inmate attempted to elude the other inmate; however, the attacking inmate pushed the other inmate down on the steps, striking him in the back with his right fist and kicking him in the right side with his right foot. Upon the arrival of responding staff, the inmates were separated and restrained without further incident. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.
 
**At 5:27 p.m., staff in the Special Housing Unit radioed for assistance upon observing an inmate, assaulting another inmate, in a cell. Specifically, as one inmate retrieved the food trays from the food slot, the other inmate began striking the other with his fists to the head and torso area. Both inmates were ordered to cease their actions, in which they refused to comply. Upon the arrival of additional staff, the cell door was opened. Staff had to physically separate each inmate. Both inmates were restrained, medically assessed, and placed in separate cells without further incident. The FBI was notified and the investigation continues.

FCI- BEAUMONT (MEDIUM), OCTOBER 22, 2008
 
Ten staff were injured in a gang related disturbance which occurred on the recreation yard.  While responding staff were securing the recreation yard a large group of gang specific inmates (most of the same inmates involved in the assault) attacked 5 staff members after one inmate refused to obey orders to get on the ground.  Its estimated that approximately 25 gang related inmates were involved in the assualt on staff.  Injuries to the staff ranged from a concussion to cracked ribs to cuts.

At approximately 9:20 a.m., while making rounds a Unit, the Unit Officer ordered an inmate, to clear his cell window. The inmate refused to comply with the order to clear his window. The inmate then got up and shut the cell door on the Unit Officer’s foot. The cell door was immediately secured and the Lieutenant was notified. The inmate was medically assessed and escorted to the Special Housing Unit without further incident. The staff member was medically assessed, receiving minor injuries to his foot. The FBI was notified and the investigation continues.
 

**At 1:00 p.m., an inmate placed his arm out of the wicket of his assigned cell. Staff ordered the inmate to submit to restraints in order to secure the wicket. As staff attempted to place restraints on the inmate, he reached out with his right arm in an attempt to assault staff. At this time, the inmate again reached out with the same arm in another attempt to assault staff. Staff were able to secure the wicket without further incident. The inmate and the staff member were medically assessed with the staff member receiving injuries to his left and right hand. The FBI was notified and the investigation continues.

USP- BIG SANDY- OCTOBER 21, 2008
 
At approximately 9:01pm, Staff called for assistance on a Unit. Responding staff ran into an inmate refusing to be restrained, due to being intoxicated. While this was going on another inmate ran down the stairwell in the unit and began assaulting the unit officer with a weapon. An Officer was stabbed 5 times with the weapon by the inmate who ran back up the stairs after assaulting the Officer. The unit was finally locked down while the Officer was rushed to Medical Center. The Officer sustained two, 3 inch stab wounds to the head area and three stab wounds to the lung and back area. The Officer had Blood streaming from the head wounds and was taken to a local hospital for treatment. There were 3 inmates involved.  U.S.P. is locked down at the time.

USP- COLEMAN-I OCTOBER 21, 2008
 
Two staff were assaulted by inmates. One staff member was stabbed by what was described as a sharpened pexiglass type weapon approximately 15 times.  That staff member was airlifted from the insitution to an outside hospital.  The second staff member was punched with a closed fist and received minor injuries.  The institution was locked down pending investigation.

At 11:11 a.m.,  a large scale fight occurred on a housing unit range.  Responding staff were able to contain and control the situation.  Approximately 59 inmates were escorted to the Special Housing Unit without further incident. During the altercation a staff member was punched in the upper torso and received medical attention. The Institution was placed on lock-down to investigate the incident.

FCI- OTISVILLE- OCTOBER 21, 2008

At 10:40 am 7 ** inmates were ordered to the compound from SHU. They did not want to go to the compound because they don’t get along with ** . Staff told Management not to release the inmates on the compound because there will be a fight between the gangs . Management released the inmates to the compound anyway. At around 12:00 pm the same day a body alarm was sounded in one of the housing units for a fight involving several inmates with weapons. One inmate was stabbed several times and needed medical attention the other inmates had minor injuries.

At approximately 11:25 a.m., an inmate, threw a cup of feces and urine on staff as they were feeding on a housing unit range, striking the staff member on the right leg. To avoid being assaulted again, the staff member backed away from the wicket, prior to it being secured. The inmate then threw feces and urine on the walls and floor of the range. Notifications were made and authorization was granted to assemble a Use of Force Team. Confrontational avoidance was successful. A medical assessment was conducted, and the door was secured without further incident. The investigation continues.

At 7:30 p.m. 4 staff were assaulted by inmates. Some inmates were able to break cell doors off of hinges in the Special Housing Unit they were then unsecure on SHU Ranges.  It was reported that the staff assaulted received minor injuries.  The institution remains on lockdown status. The inmates were also reported making gas suits out of mattresses in SHU.

At approximately 7:20 a.m., a Tower Officer observed four inmates, assaulting another inmate, with their fists on the recreation yard. The Tower Officer notified Control, via radio, and activated his bi-lingual warning annunciator, ordering inmates to the ground. The inmates disregarded the order and continued striking and kicking the one inmate with their boots. Fearing for the safety of responding staff and the inmate safety, the Tower Officer discharged two ALS Hard Bang and two Fin Stabilized rounds from his designated less lethal, Model 870, Remington, 12 gauge shotgun; however, the assault continued. The Tower Officer then discharged one 5.56 mm round from his M16A2 rifle, firing into the designated pit at the base of his tower. Responding staff separated and restrained the inmates without further incident. (Separate incident with different inmates, gang related) As staff were escorting the inmates from the recreation yard, a housing Unit Officer announced a fight in a Building courtyard. Specifically, two inmates were fighting with one another. Responding staff ordered them to cease and separate; however, they refused to comply. Both inmates were placed onto the ground and restraints were applied. All inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

FTC- OKLAHOMA CITY- OCOTBER 17, 2008
 
At approximately 11:10 pm when an Officer was attempting to house an inmate coming into the Unit from R&D. The inmate already housed in the cell did not want the new inmate coming from R&D placed in his cell. The Officer pulled the inmate out to talk to him and the inmate punched the Officer in the face and kept assaulting the Officer till responding staff was able to arrive (next door unit officer eliminated 10pm to 6:00am), one of the responding staff members who responded received an injury to his elbow. The Unit Officer sustained minor injuries to both hands, forearm, and chest area. A responding Lieutenant involved in the incident also experienced some numbness in his shoulder.

At approximately 4:55am, an inmate grabbed medical staff's arm while he was receiving medical care.  The Unit officer had to forcefully remove the inmate's hand from the staff memeber's arm.  The staff member or the inmate were not injured.

A serious fight  broke out involving 6-7 inmates who were believed to have assaulted one inmate. The inmate went to the hospital for his injuries.

FCI- TERMINAL ISLAND- OCTOBER 16, 2008
 
An officer was one of three staff members that were escorting an inmate from his cell to the recreation area of the SHU. As the inmate came out of the cell he head butted the officer in the face and kicked another staff member in the groin area. The third staff member managed to gain some control, and the inmate was put back in his cell.  The officer  sustained a fractured nose and deviated septum.

15-20 inmates.  Homemade weapons were involved, requiring one inmate to be taken to the outside hospital by ambulance for treatment due to stab wounds.  The institution locked down until approximately 12:30 pm 10-16-08.

At approximately 8:02 p.m., two inmates, were in housing unit holding cell. Staff observed one of the inmates reach up and pull down on the sprinkler head, causing it to break. Notifications were made and a calculated use of force team was authorized. Confrontation avoidance was initiated and proved successful. Both inmates voluntarily submitted to restraints. Medical assessments were conducted without further incident.

FCI- OTISVILLE- OCTOBER 12, 2008

At 8:00 pm an inmate assaulted a staff member while exiting his housing unit. The officer was doing random shake downs in the unit and took nuisance contraband from the inmates cell. The inmate was upset that the officer took his unauthorized items from his cell. When the 10 minute move was announced the inmate pushed the staff member when he was exiting the unit. The staff member hit his body alarm and staff responded and cuffed the inmate. The inmate was brought to the lieutenants office and the officer wrote a 200 series incident report on the inmate. But due to overcrowding in SHU the inmate was released back to the same unit that he had assaulted the staff member in from the LT’s office. The union did not know about this incident until Tuesday,OCT 14th 2008. When we spoke to the Captain about this he stated that he did not know that a staff member was assaulted. He then had the inmate that assaulted the staff member placed in SHU.

At approximately 8:17 a.m., as SHU staff were retrieving the food trays from the cells, an inmate, attempted to grab staff through the open tray slot. At this time, another inmate, blocked the tray slot with a blanket. As staff attempted to secure the tray slot, one of the inmates threw a liquid substance, which smelled like urine, through the tray slot, striking two staff members in the facial and chest areas. During this time, two additional staff members were struck by urine as they assisted in securing the tray slot. A Use of Force Team was authorized to place both inmates into ambulatory restraints due to their disruptive behavior. Confrontation avoidance was initiated and proved successful. Both inmates submitted to restraints, moved to another Range holding cell, and placed into hard ambulatory restraints. Both inmates were medically assessed without further incident. One staff member was medically assessed and referred to an outside hospital for a possible strained back. The FBI was notified and an investigation continues.
 
**At 12:15 a.m., Special Housing Unit staff observed two inmates in SHU had covered their cell door window preventing staff from completing the 12:01 a.m. institution count. The Lieutenant arrived at the SHU at 12:20 a.m. and ordered both inmates to uncover their window, in which they refused to comply. The shield was placed in front of the cuff slot which was then un-locked. At this time, one inmate forced the slot open and immediately threw a cup of urine through the slot, striking the shield. During the process of securing the cuff slot, three staff were struck by some of the urine thrown by the inmate. Staff secured the tray slot. Confrontation avoidance was initiated and proved to be successful. The staff members were medically assessed.
 
***At 7:02 p.m., staff radioed for assistance after observing an inmate fight in a housing unit. One inmate, was striking another inmate in the head and upper torso with a broken broom handle. The other inmate was swinging a towel and pushing at the inmate with the broom handle. Upon the arrival of responding staff, the one inmate ran from the incident and threw the broken broom handle and another item he had in his possession. As the inmate was running, staff had to physically place him on the floor in an effort to control his disruptive behavior. Both inmates were restrained, medically assessed, and admitted into the Special Housing Unit without incident. The FBI was notified and an investigation continues.

USP- LEE- OCTOBER 10, 2008

At approximately 9:15 a.m., an inmate, had the a unit range observation cell’s windows and walls covered in sheets of paper. Staff ordered the inmate to remove the paper from the cell windows and walls. He refused to comply with staff orders. Staff ordered the inmate to step to the cell door to submit to hand restraints in which he refused to comply. At this time, the inmate displayed a urinal and threatened to throw urine on staff. Due to his disruptive behavior, notifications were made and authorization was granted to assemble a calculated Use of Force Team. Confrontational avoidance was initiated and proved unsuccessful. The inmate was given a final order to submit to restraints in which he complied.

At approximately 10:11 a.m., an inmate approached staff and was observed with injuries consistent with being involved in a physical altercation. The staff member immediately activated their body alarm and secured the inmate. Responding staff secured the unit and initiated upper body searches. A review of the VICON CCTV video surveillance identified two inmates as being the assailants. Specifically, both inmates began striking the first inmate in the face with their fists. One inmate then began to swing a lock attached to a belt at another inmate, striking him in the head. All of the inmates involved were restrained, medically assessed, and placed into the SHU without further incident. The FBI was notified and an investigation continues.

FCI- OXFORD- OCTOBER 8, 2008
 
At 12:20 pm, the Control Center announced an inmate fight in the paint shop area. Specifically, facilities staff observed two inmates in a physical altercation. Responding, the staff immediately contained and isolated the incident. Both inmates were separated, restrained, and escorted to the Lieutenant's office. Health Services staff conducted medical assessments on both inmates. The first inmate was treated for a 2" laceration to the right cheek, a minor laceration to the forehead, and minor abrasions throughout the body. The other inmate was treated for a minor laceration to the back of the left hand, and a superficial laceration to the left forearm. There were no staff injuries reported during the incident. Both inmates were placed in the Special Housing Unit. An investigation is on-going. The FBI has been notified.

At approximately 8:15 a.m., a food service staff member activated his body alarm when inmate ****, attempted to strike him in his face.  When the staff memeber attempted to place some distance between him and inmate ****,  the inmate lunged towards him and grasped his jacket.  Responding staff were able to place inmate **** on the floor with no further incident.  No injuries were sustained by either staff or inmate ****.

 One inmate was stabbed and has a severed liver and was
Airlifted to a trauma Hospital. No Staff injuries to report. Also 3 Gangs squared off in the middle of this in Food Service but was stopped by responding staff.

From interviews thus far, an inmate owed a $15 gambling debt. Inmate was approached by another inmate and was told that if the debt was not paid, they were going to stab the inmate that owed the money.  The inmate said he paid the $15 already to avoid any drama. Later that evening, the inmate who owed the debt, went to the other inmates and confronted them about the threat and cursed them out.  Stating "you guys were going to run some steel for a $15.00 debt". (obviously in a
disrespectful manner) At approximately 12:00 p.m., inmates approached inmate in aUnit, and confronted him about what he said to the inmate who owed the debt.  Inmate said that the inmate who did most of the speaking for ** was armed
with a homemade weapon.  The inmate stated he told them he advised the inmate to stop gambling, he and the inmate then started fighting, and that is when other inmates joined the fight using fists and broom handles.

Later, at approximately 12:32 p.m., several inmates began
fighting with weapons in a Unit.  The inmates involved in this incident, are from the initial incident earlier.  At this time all inmates have been returned to there units, the institution
remains on lock down, SIS staff are conducting key interviews with targeted inmates. Only one inmate sustained injuries requiring outside medical attention, and he has been transported to an outside medical facility for treatment of the non life threatening injuries.  There were no staff
injuries reported at this time.

At approximately 5:50pm,  a fight involving 4 inmates broke out. One inmate was bitten in the ear and received medical attention. No other serious injuries are reported.

FCI- LEWISBURG- OCTOBER 4, 2008
 
At approximately 5:38 a.m., the J-Block Officer heard noises coming from the third floor of the block.  Upon investigating, the block officer observed inmates ****, and ****, throwing closed fist punches at one another.  Inmate **** initially refused all orders, continuing to punch inmate **** and knocking him to the floor.  Inmate **** then began to kick inmate **** in the head and upper torso.  Upon the arrival of additional staff, inmate **** ceased his actions.    Inmate **** was found to have sustained swelling and several abrasions to the head, 2 half inch laceration to the scalp, a laceration to the bridge of the nose, and laceration to the left hand which required 11 stitches.  There was a weapons involved(Batteries in sock and razor).

An institution was placed locked down following the completion of the 4PM count.  Earlier in the week, we had received a bus load of inmates.  Several of them were placed on the compound.  We had problems in the past with the ** and **.  Throughout the day on Saturday, all of the other inmates on the compound requested protective custody.  As a result, the administration decided to lockdown the USP as a precaution.  The lockdown was lifted Monday.

At 10:15 a.m., staff observed an inmate with injuries consistent with being involved in a physical altercation. As SIS staff reviewed recorded video surveillance, it revealed two inmates striking one another in the head, face, and upper torso area with their fists while working in the serving line of Food Service. Both inmates were escorted to Health Services and medically assessed. Both inmates were admitted into the Special Housing Unit without further incident. An investigation continues.

At approximately 10:11 p.m., as Special Housing Unit staff conducted the count on a Range, they observed two inmates striking one another in the head and upper body with their fists. Staff activated their body alarm and ordered the inmates to cease their disruptive behavior. Upon the arrival of responding staff, the inmates complied with staff orders and submitted to hand restraints. Both inmates were placed into separate recreation cages and medically assessed without further incident. An investigation continues.

Medical Emergency sent to a Unit where an Inmate was unresponsive on his toilet with vomit on his shirt. Medical assessed in HSU and stated it was possibly narcotics related. A search of his cell was conducted and revealed a Syringe and a homemade pipe.The Inmate was placed in the special housing unit for possession of narcotics

An incident occurred just after the Housing Unit had been toured. This shows that we should have never let the Inmate Population up from the Saturday incident. The inmates are retaliating from the Saturday rival between other gang inmates. Institution placed back on lockdown.

An incident involving two inmates has resulted in the death of one of the pair. A news release from the prison states that staff responded to an incident between two inmates at approximately 10:21 a.m. and immediately separated the two inmates. The injured inmate was immediately transported to Lee Regional Medical Center, where he was pronounced dead at approximately 11:51 a.m. No staff were injured during the incident. The institution was placed in lockdown status as a result of the incident.

At approximately 1:00 p.m., inmate **** was observed in a block in a stairway in a fighting stance; with a padlock tied to a torn sheet in his hand.  Staff called for assistance and inmate **** was restrained.  Video surveillance revealed inmate **** assaulted inmate ****, and inmate **** with the homemade weapon and closed fists.  The inmate who committed the assault was out of SHU approximately 3 minutes.

At approximately 5:30 p.m., staff called for assistance below a tower. Staff arrived and found an inmate on the ground face down. The video surveillance recordings was reviewed and determine that , the inmate had fought with another on the walkway between the metal detector building and the east corridor food service entrance. The video showed one inmate strike the other in the face with his left fist, the other falls to the ground and stops moving. The first inmate continues to kick the other inmate in the face, head and body, then departs toward the metal detector building, while the one inmate remained on the ground . Inmate first inmate was located in a housing Unit and escorted to Special Housing , medically assessed and placed in Administrative Detention. The other inmate received medical attention, then escorted to Special Housing and placed Administrative Detention. the investigation continues.

At approximately  9:00 a.m., inmate ****was escorted into the Lieutenant’s Office after refusing to be pat-searched in a Corridor.  Once in the Lieutenant’s Office, inmate **** took an aggressive stance, threw his coat and lunged at the Lieutenant.  Inmate **** was then placed on the ground by responding staff and escorted to the Special Housing Unit without further incident. The Lieutenant sustained soreness to his left shoulder and the inmate did not sustain any injuries..

At  7:05 p.m.,  an inmate was placed in SHU for possession of a weapon.  

 

** At 7:31p.m., a fight was announced on the Recreation Yard. Eight inmates were locked up due to the altercation.

**7:58 PM a Fight was announced in a unit by a unit officer. Two inmates were locked up due to the altercation. One staff member injured during response from Training Center. An ambulance was called for the inmate.

At approximately 7:10 a.m. staff approached a cell in the Special housing Unit (SHU) occupied by an inmate to retrieve the breakfast food trays. Upon opening the tray slot the inmate threw his trays out of the slot striking an officer in the left foot. The inmate then became verbally aggressive stating: "You stupid cracker." Staff secured the tray slot without further incident.

At approximately 7:56 pm, the Tower Officer called for assistance due to observing six inmates, involved in a physical confrontation on the recreation yard. The Tower Officer activated the automated voice annunciator ordering the inmates to get on the ground. The inmates refused the warning, and two Towers subsequently fired three ALS hard bang rounds, causing the inmates to lay on the ground. Responding staff restrained all involved inmates and escorted them to Administrative Detention. All inmates were medically assessed.

At 9:30p.m., an Inmate was sent to SHU for insolence towards staff,, stealing government property, and possession of gambling paraphernalia.

At approximately 7:08pm, a unit officer announced a fight in front of his unit. He observed three inmates involved in a fight. Two inmates were holding another inmate by his shirt and striking him with his closed fist in the chest and stomach area. The one inmate was striking back with both left and right closed fists. Responding staff arrived, the inmates were separated and placed on the ground. As one inmate was being placed on the ground, staff saw him with a metal weapon approximately 4 inches in length. The weapon was secured, inmates restrained and escorted to medical. One inmate was taken to medical where a piece of metal was in his abdomen. The other was taken to an outside hospital to remove the metal object and kept for observation. The other inmates were medically assessed and placed in Special Housing. An investigation continues.

At 3:24 p.m., staff called for assistance upon observing an assault in the common area of a housing Unit. Specifically, staff observed two inmates, striking another inmate, with their fists to the head and body. Staff ordered the inmates to cease their behavior, in which they complied. Upon the arrival of responding staff, the inmates were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**An inmate was assaulted by several other inmates. His back was broken, broke facial bones and has swelling of the brain. He was airlifted to San Angelo,Texas Hospital and is in ICU.
 
** While evening mainline was in progress a fight broke out in a housing unit.  After the incident was contained, the institution returned to normal operations.  No staff were injured during the incident.

While counting at the 12:00 am count two staff were assaulted by an inmate when they removed a sheet he had put up. They were trying to see the inmate for count. One staff suffered an ankle injury and the other staff had minor injuries.

At approximately 3:05 p.m., visiting room staff called for assistance. Specifically, an inmate visit had concluded and he was escorted to the shakedown room to be searched. During the visual search, staff noticed he had an object in his right hand. The inmate placed his right hand behind his back and staff grabbed his wrist to secure the object. The inmate pulled away from staff and attempted to put the object in his pants. As responding staff arrived the inmate was placed on the wall, restrained, and a visual search was conducted. During the visual search, the object fell out of his pants. The object was cylindrical object and wrapped in silver tape. The object was retrieved and secured by SIS staff. The inmate was medically assessed and placed on Dry Cell status. The FBI was notified and an investigation continues.

At about 4:15 p.m. there was a fight called in unit **. One
inmate was transferred to an outside hospital then was life flighted to a trauma center where he remains in critical but stable condition. Both inmates were drinking intoxicants.

** The inmate was sent to U.T for a solid month after being assaulted.

At approximately 6:20 pm, the Tower Officer called for assistance due to observing three inmates fighting on the recreation yard. Specifically, the inmates were striking one another in the head and upper torso area with their fists. The Tower Officer activated the automated voice annunciator ordering the inmates to get on the ground. The inmates refused the warning and continued to fight. As responding staff arrived, the inmates got down on the ground and hand restraints were applied. The inmates were medically assessed and admitted into the Special Housing Unit without further incident. An investigation continues.
 
**At 9:50 p.m., SHU staff were escorting an inmate back to his assigned cell. Staff removed the hand restraint from his left hand and started to remove the restraint from his right hand, when the inmate attempted to grab the hand restraint and keys. Staff attempted to close the wicket. The inmate kicked the wicket with his foot, striking the staff member in the left hand and wrist. An assisting staff member attempted to secure the wicket and had his right thumb caught between the wicket and the door. The wicket was secured without further incident. The inmate and the two staff were medically assessed. The FBI was notified and an investigation continues.

At approximately 10:20 a.m. a Tower  officer observed an assault on the recreation yard, he activated the verbal warning system and the inmates did not respond, he fired 2 blast dispersion rounds over the area and they continued to assault the inmate. He then fired 1 fin stabilizer into the area,the inmates stopped assaulting the inmate and lay down on the ground. No weapons were found on the scene. No staff injuries. While staff were responding to this incident, in corridor * inmate ** became verbally aggressive towards staff while being interviewed in Psychology. He left the area and became insolent towards responding staff. Staff observed him being insolent towards staff and he was ordered to get on the wall to be cuffed, resistance occurred and the inmate was placed on the floor to regain control. As staff were now splitting up to respond to the corridor * incident, a body alarm activation occurred in one of the housing units. Now staff are running in 3 directions. Thankfully the body alarm was false.

Institution is on lock down. At the Friday meal some inmates refused to put their trays up and depart the chow hall. The inmates expressed discontent as the Warden made comments about how long the feeding was taking for certain participants in SHU.

Approximately 11:30 am two inmates engaged in a fight in the  housing unit. Moments after staff broke up the fight a second group of twelve inmates began fighting. This was gang related. During the fight an inmate approached a female staff member that was attempting to break up the fight; with a homemade shank. The female staff member kicked the inmate in the groin area subduing him and she was able to retrieve the weapon from the floor. The unit was placed on lockdown and the lockdown was lifted the next morning on 09-19-2008.

USP- LEE- SEPTEMBER 17, 2008

At approximately 9:30 a.m., an inmate was ordered to submit to restraints in order to receive a cell mate, in which he refused to comply. At 11:30 a.m., the inmate was ordered once again to submit to restraints, in which he refused to comply. Notifications were made and authorization was granted for a calculated use of force team. At 1:20 p.m., confrontation avoidance was initiated; however, was unsuccessful. The inmate then covered his window and broke the sprinkler head in his cell as the #1 Camera man arrived. Upon the arrival of the use of force team, the inmate was ordered to submit to restraints, in which he refused to comply. The inmate then blocked the tray slot with his mattress. Less than lethal munitions were used to gain compliance of the inmate who was removed from the cell, medically assessed, and decontaminated due to the use of chemical agents. The inmate was placed into a housing range Observation cell and placed into ambulatory restraints without further incident. The use of force concluded at 1:56 p.m. No staff injuries were reported.

He was assaulted by 3 other inmates. 5 staff members were called in to subdue the inmates. Unfortunately, these staff were injured in the process. Institution was placed back on lockdown.

At approximately 3:29 p.m., an Officer responded to a duress alarm in a cell in the Unit. As staff approached the cell, a noise was heard consistent with someone fighting. An inmate, was standing with his shoulder to the cell door to prevent staff from having a clear view into the cell. Staff radioed for assistance and secured the cell door upon witnessing three inmates, fighting with one another in the cell. Upon the arrival of responding staff, the inmates ceased their behavior. The inmates were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. An investigation continues.

In the Mental Health lock unit, an Inmate assaulted a staff member during his cell rotation, no staff injuries was reported.  The inmate was placed in four point restraints and received injuries to his hand and foot.

** At 5:57 pm a body alarm sounded for a cell fight with a weapon. Possibly related to all other incidents above.

Responding staff separated the inmates and placed them in hand restraints. The inmates were medically assessed, and escorted to the Special Housing Unit, pending further investigation.

At approximately 11:43 AM, the control center announced “officer needs assistance on *-Block”. As the Lieutenant arrived on site, he observed two inmates engaged in a mutual altercation with each other, as the Lieutenat got closer he observed an inmate with a weapon on his hand. Inmate **** and inmate **** were given orders to stop fighting but they both refused. Since a weapon was observed and both inmates refused staff orders to stop a two second burst of OC was dispersed. After the OC was dispersed both inmates were separated and both were taken to health services for a medical assessment and decontamination. An officer received a cut to his right thumb, he was seen by the on duty PA and was sent to the outside hospital for further treatment.

At approximately 12:50 p.m., the Lieutenant ordered an inmate to leave the Lieutenants’ Office and he refused. The inmate continued to refuse to leave and swung his fist at the Lieutenant, just missing him. Staff immediately attempted to gain control of the inmate and he resisted staff by swinging fists and kicking at them. The inmate was placed onto the ground to gain control of his disruptive behavior. While staff were attempting to place restraints on the inmate, he kicked a staff member, striking them on the left leg. Once the inmate was controlled, restraints were applied. He was medically assessed and taken to the Special Housing Unit without further incident. The FBI was notified and the investigation continues.
 
**At 5:28 p.m., staff called for assistance due to fight. Staff observed an inmate enter the corridor from the recreation yard, followed closely by another inmate. Upon entering into the corridor, one inmate struck the other inmate in the face with a closed fist. They immediately began throwing closed fist punches. Staff immediately placed both inmates on the ground to control their actions. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident.

At approximately 5:30 pm, an Officer was confronted by an intoxicated inmate who was holding a 8" shank. The inmate refused to give up his liquor. The institution was placed on lockdown, however, the next day inmates were released.

At approximately 9:55 p.m., The a unit Officer radioed for assistance. Specifically, inmate was ordered by staff to go to his cell. The inmate then began following the staff member waving his arms and speaking loudly acting as if he was intoxicated. Staff again ordered the inmate back to his cell. At this time, the inmate stepped within inches of the staff member and grabbed the staff member’s right arm. Upon the arrival of responding staff, the inmate was ordered to place his hands on the wall, in which he refused to comply. The inmate was then placed on the floor for staff to gain control of him . He was then placed into hand and leg restraints, and he was escorted to the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 8:05 a.m., the SHU Recreation Officer called for assistance upon observing an assault taking place. Specifically, staff observed inmates ***** and ***** strike inmate ****** with closed fist punches, causing inmate **** to fall to the ground. Once on the ground, inmates **** began kicking inmate **** about the head and facial area. Staff ordered the inmates to stop and submit to restraints, which they complied without further incident. Inmate **** was examined by medical and treated for minor injuries such as lacerations, and hematomas to the head, and facial area.

**At approximately 12:10 p.m., during the lunch meal,  staff observed inmate **** strike inmate **** with a closed fist in the back of the head. Staff responded, separated, and escorted both inmates to Health Services for medical assessments. Inmate **** and inmate **** were medically assessed and it was discovered they both did not sustain any injuries and were escorted to the Special Housing Unit without further incident.

**At approximately 5:10 p.m., during the Evening meal,  staff observed inmate **** strike inmate **** over the back of the head with a Walking Cane. Staff responded, separated, and escorted both inmates to Health Services for medical assessments. Inmate **** was medically assessed and it was discovered he had a laceration to the back of his head. Inmate **** did not sustain any injuries and they were both escorted to the Special Housing Unit without further incident.

** At 9:50 pm, A body alarm sounded, an inmate became disruptive and destroyed several inmate computers with a broom handle because he was mad at an officer.

At approximately 5:15 p.m, while making rounds on a range in the Special Housing Unit, inmate **** in cell *****, was banging on the door, stating he and his cell mate, **** got into a fight.  Inmate *** was observed with bruising on the right eye.  Both inmates were placed in hand restraints and separated without further incident.

Shortly after 4:00PM, an inmate was stabbed with a pencil, about 3-1/2 to 4 inches of the pencil was broken off inside his left calf. 5 to 6 inmates where locked up over the incident and lockdown was only implemented during the regular count time.

At approximately 8:00 a.m., staff observed seven to eight drops of blood in the secure corridor near UNICOR. A review of recorded video surveillance revealed at 7:49 a.m., two inmates assaulted another inmate. Specifically, two inmates began striking another inmate with their fists to his head and upper torso area. All three inmates were removed from their assigned housing units, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At 3:56 p.m., staff called for assistance in a Unit upon observing an inmate strike another inmate, three times with closed fists to the face and upper torso. The inmate being assaulted ran from the other. Staff grabbed the inmate doing the assaulting around the waist and pushed him back down the range away from the other inmate. Staff ordered the inmates to cease their actions, in which they complied. Upon the arrival of responding staff, the inmates were separated and restrained. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

***At 7:11 p.m., staff called for assistance in a housing unit upon observing an inmate striking another inmate in the upper body repeatedly with a homemade weapon. Upon the arrival of responding staff, staff had to physically separate the two inmates taking one to the floor and placing the other on the wall. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 7:20 p.m., a tower Officer observed inmate **** strike inmate ****. Both inmates had closed fists and were hitting each other in the face and torso area.  As staff approached, inmate **** ran into a crowd of inmates.  Responding staff secured inmate ****, and the recreation yard was cleared of all inmates.  Inmate**** was identified by recreation staff and he was taken to Health Services for assessment.  Inmate **** sustained a minor abrasion to the temple area, and a minor laceration to the nose. Inmate **** sustained no injuries.  Both inmates were placed in the Special Housing Unit pending further investigation.

At 7:15 p.m., staff called for assistance due to an assault in progress on the recreation yard. Staff observed four inmates striking another inmate about the head and upper body area with their fists. The Tower Officer activated the bi-lingual annunciator ordering the inmates to the ground; however, the inmates disregarded the order and continued to assault the inmate. The one tower Officer fired one, ALS hard bang round from his designated less lethal Remington Model 870, 12 gauge shotgun, to gain compliance; however, the inmates continued to assault the one lone inmate. Fearing for the safety of responding staff and the inmate being assaulted the Tower officer fired two additional ALS hard bang rounds. Upon the arrival of responding staff, the inmates were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

Both inmates were medically assessed and it was discovered Inmate **** sustained a laceration to his left pointer finger, treated for his injury and placed in the Special Housing Unit without incident.  Inmate  **** did not sustain any injuries and was placed in the Special Housing Unit without further incident.

At approximately 11:29 a.m., the Yard Officer called for assistance upon observing inmates involved in a physical altercation on the sidewalk in front of the metal detector shack. Specifically, both inmates were striking one another to the head and upper torso area with their fists. Both inmates were given several orders to cease their actions, in which they refused to comply. Due to the inmates disregard for staff orders and their continued disruptive behavior, both inmates had to be placed onto the ground for responding staff to gain control. Both inmates were separated, restrained, and medically assessed. Both inmates were admitted into the Special Housing Unit without further incident. An investigation continues.
 
**At 2:47 p.m., while SHU staff exchanged laundry in the Special Housing Unit, staff observed inmates striking one another to the head and upper torso area with their fists. Staff immediately activated their body alarm and announced a fight in the Special Housing Unit. Upon the arrival of sufficient staff, the cell door was opened. Responding staff had to physically separate and restrain both inmates. Both inmates were medically assessed and placed into separate cells without further incident. An investigation continues.

At 5:53p.m., a fight was announced in the food service area. Two inmates were sent to SHU for fighting.

At approximately 9:25 a.m., the fire alarm was activated in the Special Housing Unit due to an inmate, setting a fire on the range inside his assigned cell, and directly outside his cell door. SHU staff immediately extinguished the fire. The inmate then covered his window and barricaded himself inside of his cell and stated he had a weapon in his possession. Notifications were made and authorization was granted for a use of force team. Upon arrival of the use of force team, confrontational avoidance was initiated in which the inmate agreed to submit to hand restraints. He was medically assessed and was placed back into his. The FBI was notified.

**At 12:55 p.m., as staff conducted rounds in Special Housing Unit, they approached a cell, which was occupied by an inmate. Staff observed the inmate take a razor blade and cut his left forearm. The inmate was bleeding from the self inflicted wound. Staff ordered the inmate to relinquish the razor blade and submit to hand restraints. He complied and was escorted to the SHU medical observation room. Medical and Psychology staff were immediately notified. The inmate was medically assessed and treated for his injuries.

***At 6:13 p.m., the Tower Officer called for assistance upon observing an assault on the recreation yard. Specifically, staff observed several unidentified inmates assaulting another inmate in the face, head, and upper body area. The Tower Officer activated the bi-lingual annunciator ordering the inmates to the ground; however, the inmates disregarded the order and continued to assault the inmate who was later identified. Fearing for the safety of responding staff and inmates, the two Tower Officers each fired one ALS hard bang round from their designated less lethal Remington Model 870, 12 gauge shotguns. At this time, some of the unidentified inmates disappeared into the crowd. Upon the arrival of responding staff, the inmates involved were identified, restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 6:15 a.m., a Officer observed inmate **** with injuries to his head and face. The inmate was escorted to Health Services while the remaining inmates in the Block were locked down.  Video surveillance revealed inmate ****, and inmate **** engaged in a fight on the second floor ot the block.  Inmate **** did not sustain any visible injuries and inmate **** sustained two small lacerations to his left eye, contusions to his head, face, and jaw, as well as abrasions to his knees and chest, requiring outside medical treatment.

At approximately 6:31 a.m., as a Unit Officer conducted rounds throughout the unit, he heard sounds of a struggle coming from a cell. As he arrived at the cell door, he observed two inmates striking one another with their fists to the head and upper body. The unit officer radioed for assistance and ordered both inmates to cease their actions in which they refused to comply. Upon the arrival of responding staff, both inmates were separated and restrained. Both inmates were medically assessed and were admitted into the Special Housing Unit without further incident. An investigation continues.

**At approximately 11:45 a.m., the fire suppressant system was activated in the Special Housing Unit due to the inmate tying a bed sheet around the sprinkler head and breaking it. Notifications were made and authorization was granted to place the inmate into hard, four point, restraints due to his continued disruptive behavior along with his documented history of manipulating restraints. The FBI was notified.
 
**At 7:15 p.m., staff called for assistance due to physical altercation on the a Housing Unit stairwell outside of the unit. Staff observed an inmates striking another inmate, in the head and upper body with his fists. Upon the arrival of responding staff, the inmates were separated and restrained without incident. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. An investigation continues.

At approximately 2:46 p.m., staff called for assistance upon observing a fight on the recreation yard at the basketball court. Specifically, staff observed two unidentified inmates engaged in a physical altercation. Specifically, both inmates were striking one another in the face, head, and upper bodies with their fists. One inmate had a lock attached to a belt, swinging it at the other inmate. The Tower Officer activated the bi-lingual annunciator, ordering inmates to the ground; however, the inmates continued to fight. Fearing for the safety of responding staff, the Tower Officers fired four hard bang rounds, from their designated less lethal shotguns. The two inmates fell to the ground and continued to grapple with one another until responding staff arrived. Both inmates were separated, restrained, and medically assessed. After further review of the closed circuit video surveillance, the inmates were identified. Both inmates were admitted into the Special Housing Unit without further incident. An investigation continues.

At approximately 6:28 p.m., staff responded to a call for a fight after another Officer observed inmates ****, and inmate **** in a physical altercation. ATower officer activated the Emergency Announcement System, then discharge one (1) T-471 magnum Ultra Flash Stun Munition at the north base of the Tower, which provided the desired effect. All inmates went to the ground and the incident was concluded without further incident. Medical  conducted  assessments and treated inmate *****, for superficial abrasions to left & right elbows, knees. ****for superficial abrasions to left & right elbows, knees and swelling to right hand. Both inmates were placed in administrative detention without further incident.

At 1:24a.m., There were twenty one inmates from the bus that refused to come out of the holding cell.  The captain was then called and the twenty one inmates from the bus was complied to hand restraints . Seven of them were escorted to SHU and fourteen were escorted to regualr holding cells.

At 3:37 p.m. a unit Officer responded to a duress alarm in a cell. Upon arriving at the cell the officer observed two inmates assaulting another inmate, by striking him with closed fists to the head and upper body area. Responding staff separated and restrained the inmates and escorted them to medical, where they were medically assessed. The inmate also identified another inmate, as being involved in the assault. A review of the video surveillance identified an additional inmate who silenced the inmate duress alarm from the main panel. The inmates were placed into Administrative Detention and the FBI was notified.

At approximately 9:30 PM a DAP staff member working overtime as the E/W officer in a housing unit was jumped from behind by an inmate while attempting to enter the officer's office. He was punched twice in the face breaking an eye socket in two places and chipping a tooth. The staff member attempted to summon assistance but his radio/body alarm battery was dead. The inmate ran back upstairs after the assault. The staff member managed to go out in the main corridor where a Lt. finally noticed him in distress. The staff was taken to the outside hospital where he was treated and released at about 2 AM. The institution was NOT placed in lockdown as a result.

Specifically, staff observed two inmates, striking one another in the face, head, and upper bodies with their fists. The Tower Officer activated the bi-lingual annunciator, ordering inmates to the ground; however, both inmates continued to fight. Fearing for the safety of responding staff, the Tower officers fired two ALS hard bang rounds from their designated less lethal shotguns. Both inmates then fell to the ground and continued to grapple with one another until responding staff arrived. Both inmates were separated, restrained, and medically assessed. At approximately 6:23 p.m, the compound officer observed several unidentified inmates fighting with another inmate in front of the commissary. A Tower officer activated the emergency announcement system, instructing the inmates to get on the ground and fired one concussion round from the shot gun.  As responding staff arrived inmate *** was observed lying on the ground with multiple puncture wounds to his back area. The inmate was placed in hand restraints, medically assessed and escorted to the Special Housing Unit, pending further investigation.

At approximately 8:20 p.m., two inmates, broke the fire suppression system within their assigned cell, in the Special Housing Unit. Due to their disruptive behavior, their cell and a portion of the range was flooded. Notifications were made and authorization given to assemble a calculated use of force team. Additionally, a tee shirt was placed over the inside of the cell door window, denying staff the opportunity to visually determine the status of either inmate. Confrontation avoidance was initiated and was successful. The inmates removed the tee shirt from the window. Staff observed both inmates huddled together in the cell’s shower along with their personal property. Both inmates submitted to restraints, removed from the cell, and placed into ambulatory restraints to stop their disruptive behavior. The inmates were placed into a cell without further incident. The investigation continues.

At approximately 12:18 p.m., staff gave an inmate a lunch tray with two cups of water. The inmate then threw a single cup of water into a Officer’s face. A medical assessment of the inmate and Officer was conducted. No injuries were sustained.

An Officer was conducting PM census in housing unit, the officer advised an inmate to get off the phone. As he was walking and talking to that inmate, the inmate suddenly moved and attacked him with his fists. The officer activated the body alarm. Another staff member heard the commotion and called for assistance. Two other inmates in the meantime attacked the officer, there is a possibility that a fourth signaled the inmates.

**At approximately 9:40 a.m., a use of force team was assembled due to an inmate refusing to submit to hand restraints and covering his cell window along with barricading himself in a cell in the Special Housing Unit. Upon arrival of the use of force team, confrontational avoidance was initiated. The inmate immediately removed the window covering and brandished a razor blade. Inmate was ordered to drop the razor blade and submit to hand restraints. He then began to cut his left wrist with the razor. The inmate was ordered to drop the razor and submit to hand restraints in which he complied. He then was escorted to a holding cell where medical treatment was administered. Psychology staff was notified. The inmate was placed into ambulatory restraints due to his attempted suicide. Incident reports were written.

**At approximately 7:05 p.m., a Tower Officer observed three inmates grappling with one another in the court yard. Two of the inmates assaulted another inmate hitting him on the upper body and head area with closed fists. The Tower Officer activated his bi-lingual warning annunciator ordering the inmates to the ground; however, both inmates refused to comply and continued fighting. Fearing the safety of responding staff and inmates, the Tower Officer used his designated less lethal, firing two ALS hard bang rounds. Both inmates separated and laid down on the ground. Both inmates were restrained, medically assessed, and admitted into Special Housing without further incident.

At approximately 6:50 p.m., as a housing unit Officer was conducting rounds, he observed the door window of a cell was covered with card board. The officer opened the door and observed an inmate, sitting on his toilet, attempting to look away. The inmate was observed to have injuries to his head and facial area. A call for assistance was announced. Upon the arrival of responding staff, the inmate was escorted to Health Services for a medical assessment and admitted into the Special Housing Unit. The unit was secured and upper body searches were initiated with negative results.  An investigation continues.

At approximately 5:28pm. An officer called on the radio that there was a fight in the hallway close to the commissary. Three inmates assaulted one inmate with closed fists. Staff responded and were able to take control of the fight. All four inmates were restrained and taken to the institution hospital and then to SHU. No staff were injured.

At approximately 8:00 pm, an Officer was doing his 30 min checks in the CMU unit on an inmate. Upon arriving at the cell the inmate had blocked the front of his cell with his mattress. When the officer pushed the mattress out of the way to see the inmate, the inmate through a cup of liquid at the officer. The liquid hit the officer on the right arm and  upper torso. The lieutenant was notified. The officer went to the institution hospital. Nothing was done with the inmate and nothing was taken from the inmate.

An officer was making rounds and went inside a cell performing a cell search. The officer was attacked by an inmate who was intoxicated. The officer was able to get outside the cell. Once outside another inmate joined in the attack of the Officer. Both inmates were able to get the Officer on the ground. The inmates were able to gain control of the Officers handcuffs and began hitting the Officer in the head with the handcuffs.  A responding Officer who was in the unit next to the incident was the first to respond. The Officer was able to tackle one of the inmates. That inmate then was able to kick that Officer in the head.  Additional responding staff  were able to subdue both inmates who were intoxicated. The first Officer who was working in the unit where the incident occurred sustained a broken eye socket. This Officer also had staples placed on his head. The second Officer that additionally responded sustained a concussion.

At 8:30 p.m., an inmate was observed by staff, ripping up his bed sheet into strips in his assigned cell in the Special Housing Unit. The inmate tied one end of the cloth strip around his cell’s fire suppression system, forcibly pulling the strip, causing the fire suppression system to break, flooding his cell, and the range. Notifications were made and authorization was given to assemble a calculated use of force team. Confrontation avoidance was initiated and the inmate agreed to submit to restraints. The procedure began at 9:55 p.m. and concluded at 10:23 p.m., without incident.

At approximately 5:00 pm, a fight broke out on the Rec. Yard. The disturbance was gang related. 2 inmates sustained life- threatening injuries. They are both in the hospital on life support. The institution was placed on lockdown.

On August 16 and 17,  staff  found over 50 gallons of Hooch on the  compound. Laundry carts were filled up to the top with all Hooch that was  found.  Pictures were taken of the Hooch in the cart.

At approximately 8:00pm, an Inmate who has a history of PSY Problems, punched an officer in the face , the Officer did not have time to activate his body alarm. The inmate immediately  got on top of the officer. Two other Officers were fortunately in the unit retrieving mattresses, and saw the commotion, and they immediately responded and grabbed the inmate. Once additional staff arrived and restrained the inmate, 3 staff  members had to be treated and taken to the hospital , one officer sustained a broken nose and several facial lacerations, a second officer sustained a broken toe and torn knee ligaments, and the third Officer sustained several facial lacerations. The Institution was not locked down after this incident occurred.
 
While staff were being treated in medical , a separate inmate fight broke out in a unit.  Two intoxicated inmates were fighting.  Once this situation was secured, an inmate in * unit refused to go into his cell for the 10:00 pm count and had to be restrained by staff. All the above incidents took place within an hour of one another.
 
The Duty Officer recommended lock down status for the weekend.  Normal Operations  resumed the next day.

At 7:14 am, morning after a significant disturbance with no lockdown.Two body alarms within one minute were sounded.  One alarm for three inmates attacking one inmate in a housing unit. Second body alarm for multiple inmates fighting with weapons in a separate unit and a corridor.  Institution finally placed on Lockdown.

Several inmates fighting in different areas of the unit. Chemical munitions needed to bring the unit under order. 8 Inmates sent to SHU. After incident, many inmates in other units would not lock down until additional staff responded to each unit. No Lockdown

At 4:52pm, an Officer heard a commotion in his stairwell, upon looking, 3 inmates were fighting, 2 on 1 in the stairwell. An assistance call was put out and responding staff restrained the 3 inmates and 1 inmate was taken to Medical with head injuries. No staff was hurt during this isolated incident.

**At 1:02 pm, a Staff member was Assaulted In a corridor, a unit officer is waiting for additional staff to arrive to escort a disruptive inmate to the Lieutenant's office, the inmate punched the officer in the face. The inmate was restrained, but before he could be escorted, he spit in the officer's face.  No Lockdown.
 
** At 1:30pm, an Officer was punched in the face by an inmate. While taking the inmate to the ground the officer was spit on in the face. USP Hazelton is NOT locked down due to this incident.
 
**USP Hazelton is locked down again. At 8:55pm, an officer called for assistance, inmates fighting with weapons. Staff arrived and chemical munitions were administered. According to the officer working the unit, no staff were injured. It is believed this incident was gang related.

At approximately 5:35 p.m., an inmate stopped staff at his assigned cell, as they conducted rounds on the range in the Special Housing Unit. He notified staff his cellmate, had a razor blade in his possession. During this time, one inmate punched the other inmate in the left side of the head with his closed fist. Staff immediately radioed for assistance. Staff observed both inmates striking one another in the head and upper torso with their closed fists. Upon the arrival of responding staff, both inmates complied with orders to cease their actions and submitted to hand restraints. Both inmates were restrained, placed into separate recreation cages, and medically assessed without further incident. An investigation continues.

An inmate homicide occured. An Inmate walked up and stabbed another inmate several times in the neck while playing cards. Then dropped the knife and waited to go with staff. Staff had no problems. There was another fight earlier, but they don't know yet if that was connected.

**At 7:16 p.m. Multiple fights broke out throughout the institution involving two rival gangs. Operations ordered the institution locked down and an emergency recall of staff in the local commuting area. A total of twenty-two inmates were placed in Administrative Detention. Five of the inmates involved received multiple puncture wounds believed to be from handmade shanks during the altercation. No Staff were injured. All inmates were medically assessed without further incident. The FBI was notified and an investigation continues.

**At 7:40 p.m., as staff were escorting inmates from the recreation yard to Health Services, an inmate began to pull away from escorting staff in an attempt to assault another inmate. As staff attempted to regain control of the inmate, he struck a staff member with his knee. Staff were able to gain control of the inmate and escorted him to the Special Housing Unit without further incident. The FBI was notified and an investigation continues

**At 8:35 p.m. a tower  Officer called for assistance on the recreation yard due to three inmates assaulting an inmate by striking him with closed fist to his head and torso area. The  Officer activated the bi-lingual verbal warning ordering the inmates onto the ground. The inmates failed to comply and continued their action. Perceiving a threat to responding staff and inmate safety, another tower Officer fired two ALS hard bang rounds; the other tower Officer fired two ALS hard bang rounds, another tower Officer also fired two ALS hard bang rounds (all warning shots). Upon the arrival of responding staff, the inmates were separated and restrained. A razor blade type homemade weapon was found in one inmates hand. A six inch plexiglass homemade weapon was found approximately 15 feet from the incident. All inmates were medically assessed and placed in the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At 9:20 p.m. an inmate purposely brushed up against a female recreation specialist 's chest and shoulder. The inmate was restrained and placed in Administrative Detention. Both staff and inmate were medically assessed, with no reported injuries. An investigation continues.

USP-VICTORVILLE, AUGUST 5, 2008
 
During the morning shift, eight staff members were assaulted by several inmates who are gang related. Three staff members sustained injuries which required outside medical care, and five others were treated for minor injuries. Preliminary intelligence indicates the assault occurred after staff attempted to search an inmate. According to investigators, inmate refused to be searched and handed an unknown object to another inmate. Staff requested assistance after several inmates approached them in an aggressive manner. Staff ordered the inmates to disperse and return to their cells; however, they refused their commands. Inmate ** then struck one of the officers with his fist prompting the other inmates to join in on the assault. Upon the arrival of additional staff, the inmates were separated and restrained without further incident. The institution was locked-down as a precautionary measure, and the incident remains under investigation.

At approximately 9:50 p.m., while reviewing video in a housing Unit, staff observed at 3:35 p.m., in the common area, a inmate strike another inmate with a closed right fist to the head, who then fell to the ground and continued to receive blows with closed fist to the head and upper body area. All inmates were medically assessed and placed in the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

Five staff members were assaulted in Unit **. The Lt. called for more staff to respond to the unit. The institution was placed on Lockdown!!! 

FCI-TEXARKANA (LOW), AUGUST 3, 2008
 
A major riot almost broke out in the low. Two groups (gang related)chased each other around on the Rec. yard. One group ran into the Modular unit and grabbed the pool sticks off the tables, broke them, with the intent of using them for weapons. The institution was locked down and visiting was suspended.

USP-TERRE HAUTE, AUGUST 3, 2008
 
At 8:17 a.m., a fight erupted on the recreation yard involving approximately forty inmates. Several of the combatants were armed with various prison-made weapons. A The Tower Officer activated the Emergency Inmate Notification System ordering the inmates to lay on the ground; however, the inmates ignored the commands and continued fighting. The Tower Officer then deployed several rounds of less-lethal munitions toward the crowd with little effect. Upon the arrival of additional staff, they were able to quell the situation and secure the combatants. Nine inmates were transported to an outside hospital for additional medical treatment. The institution was locked-down, and mass inmate interviews were initiated. Following a search of the recreation yard, staff discovered thirty-seven weapons believed to have been used in the disturbance. Prison investigators believe a group of white inmates claiming to be members of a group called the ** initiated the attack on the various inmates.

USP TERRE HAUTE AUG 3, 2008
 
Institution was placed on lockdown following a major incident on the recreation yard.  At approximately 8:00 am, two inmates were involved in a fight in Unit **.  Following this incident, an incident broke out on the recreation yard at 8:17 am, involving multiple inmates w/weapons.  Approximately 80 inmates were involved. At 8:19am, additional assistance was requested from the FCI.  DCT & SORT teams were also activated. A tower officer fired nine less lethal munitions.  No live ammunition was fired. A Lt. pulled staff back until firearms and munitions were available. Staff then responded to the incident with lethal and less lethal weapons. As staff were removing involved inmates from the area, another fight broke out on a different section of the recreation yard involving Unit ** inmates.  Nine ambulances responded to the institution, which led to the news media showing up shortly thereafter. Nine inmates were taken to the local hospital for treatment of stab wounds.  Five of the nine inmates remain at the local hospital, with one inmate in critical condition.  Another twenty inmates were placed in the SHU. The institution hospital looked like a MASH unit, with bloody inmates all over the place. Thankfully, no staff were injured.  The incident was precipitated by an incident that occurred on Saturday, August 2nd, in which an intoxicated  inmate entered into a housing unit & became involved in a dispute with two other inmates. The two inmates beat him almost to death. He remains in ICU at the local hospital with a skull fracture, broken jaw, & other injuries.  The Sunday incident was apparently retaliation for the Saturday incident.

*At 6:40am, an inmate fight broke out  behind a Unit in the Corridor with weapons. Responding staff broke up the
fight with no staff hurt.

*At 4:55pm, an inmate assaulted an Officer by walking into his office and slapping him across the face, The officer hit his Body Alarm and responding staff attempted to lock  the inmate in his cell, in which it took 15 minutes to lock the inmate back into the cell due to not wanting to go to the cell.  It was intense and we almost lost the unit. The unit is locked down and will  be coming up tomorrow.

At approximately 6:46 p.m., staff radioed for assistance on the Recreation Yard upon observing four inmates involved in a physical altercation by the covered exercise area. A review of recorded video surveillance revealed three inmates assaulting one inmate. Specifically, the three inmates punched and kicked the one inmate in his upper torso and head area. Responding staff ordered the inmates to cease their behavior and lay on the ground in which they complied. The inmates were restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 5:20 a.m., while processing inmates
for the bus, inmate ** became non-compliant, pulling
away from staff. The Bus  Lieutenant utilized immediate force in order to regain control of the inmates actions , placing the inmate on the floor. Inmate ** continued to resist staff while attempting to place the inmate in restraints. Inmate ** was escorted to the Special Housing Unit and medically assessed by Health Services staff. The inmate was treated for a minor laceration to the top of the head. There were no staff injuries
noted during the incident. An investigation is on-going.

Approximately 4:20pm, an Officer got assaulted by an Inmate striking him in the face with closed fist. The Inmate had just arrived at the institution.

Last week an inmate left the dining hall,during the noon meal, with his food tray in hand. He began complaining about the new menu to the executive assistant. When he did not get satisfaction, he turned and threw his tray against the main corridor wall splattering food all over staff who were standing main line. After he was locked up,staff who were securing his property found a 9" shank. Information later revealed that this inmate was openly discussing assaulting a staff member.
Yesterday, there were three separate fights between inmates in the housing units. One of those housing units was vacated.

At approximately 1:30pm an inmate was being escorted to the
Lieutenant's office by a staff member for being in the wrong Unit. The Unit officer called for another Officer on the radio to give some assistence with the inmate. The inmate became arguementive with the staff member and stated he was not locking up and turned away from staff member and tried to
go back to the unit. The staff told the inmate to get against the wall and placed a hand on his back. The inmate stated, "Get your fucking hand off of me and swung his arm back and hit the staff member with his right forearm. Several other staff members responded. Two officers restrained the inmate against the wall and placed cuffs on the inmate behind his back. While escorting the inmate to SHU, the inmate lunged at another staff member and the other officer took control of the inmates head to take him to the floor and in the process the inmate fell on top of the officer's right leg slamming his knee on the floor. Another officer placed leg irons on the inmate, because he continued to resist all the way to SHU causing another officer to get injured. The officers that were injured where taken to Hospital Emergency Room. The other staff member was seen by the institution PA. One officer was placed on lite duty because of the knee injury. The other officer was given a couple of days off to recover.

At 7:01 a.m., a unit Officer was conducting a pat search on an inmate. During this time, the inmate verbally threatened the officer and attempted to turn on him in an aggressive manner. A call for assistance was made. The officer and responding staff placed the inmate on the ground to gain control and stop his disruptive behavior. The inmate was restrained, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At Aprox 9:00 AM An assistance call went out from Special Housing, Two fights at the same time in two different rec pens. Staff responded and broke up the fights. No weapons were seen and no staff injuries were reported.
 
**At Aprox 8:30 PM An assistance call went out from Special Housing, Cell fight, Staff responded and after the one of two I/M's cuffed up, staff were able to remove the I/M that was assaulted, They had Health Services examine they I/M that was assaulted and had a bed board and a neck support applied to move the I/M  to Health Services. A check of both I/M's did not require an out side hospital, The inmates where separated and the investigation continues. No staff injuries were reported.
 
**At Aprox 10:00 PM count another I/M in Special Housing was acting out because of the above cited action and covered his cell door windows so that staff could not see him. A cell move team was assembled to extract the I/M, At the time that this report was made, no other info was available.

At 8:04 p.m., as a  staff member was exiting with the tool cart, an inmate approached him and became argumentative. The staff member ordered the inmate to exit the area in which he complied. As the inmate walked away, he began yelling obscenities towards the staff member and grabbed the door and aggressively slammed it in the direction of the staff member, causing the door to slam into the staff members right hand, pinning it between the door and the tool cart. Staff escorted the inmate to the Lieutenant’s Office without further incident. Medical assessments were conducted. The FBI was notified and an investigation continues.

At approximately 3:24 p.m., staff radioed for assistance in a housing unit upon observing two inmates involved in a physical altercation on the second tier. Responding staff located an inmate in a cell, bleeding from apparent stab wounds to the head. A review of the video surveillance system revealed inmates entered the cell and began to fight. Both inmates exited the cell and one inmate is seen striking another on the head and upper body with his right hand. As staff responded, one inmate departed the area. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 9:20 PM, a unit officer called control and operations about a possible assault.  At 9:45 PM one inmate was escorted to SHU for involvement in the assault with minor injuries and again at 11:00 PM, a second inmate involved was escorted to SHU after being medically accessed with minor injuries.

At 9:20 p.m., staff called for assistance after he responded to a duress alarm in a cell in a housing Unit. Staff observed two inmates exiting a cell. Upon arrival to the cell, staff observed an inmate lying on the cell floor covered with blood. The unit was secured and upper body searches were initiated. Two inmates had injuries consistent with being involved in a physical altercation. Additionally, they had blood on their clothing. A review of recorded video surveillance revealed two exiting the cell after staff’s response to the duress alarm. The assaulted inmate had no injuries; however, video surveillance confirmed his involvement in the incident. All four inmates had a strong smell of homemade intoxicants. The inmates were restrained, medically assessed, and placed into to the Special Housing Unit without further incident. An investigation continues.

*At aprox 5:15 PM, An assistance call went out from Food Service, fight in the mess hall. Two Inmates were fighting. No weapons were seen. Staff responded and broke up the fight. No staff injuries were reported. Both Inmates were placed in Special Housing.
 
*At aprox 9:17 PM, Special Housing reported a cell fight, Staff responded and after enough staff were present they broke up the fight. One Inmate had to be taken to the outside Hospital, No report on his condition was known at this time.  A home made weapon was found, and the attack may have been gang related. Investigation continues. No staff injuries were reported.

 About 7:00 PM a body alarm was activated in the shu after a inmate removed his hand cuff while being escorted back to his cell. The inmate placeds one staff member in a bear hug while kicking at other staff. The inmate  also hit a staff member in the face. All 3 staff member were taking to the hospital with injuries. A team was put together to remove the inmate from his cell gas was used when the inmate refused to cuff up. All 3 staff are doing well at this time.

At approximately 9 p.m., a fight occurred in the laundry room.  Staff responded to the incident, placed the unit on
lock down and began Inmate searches. Two of the inmates involved were located and taken to medical for assessment of their injuries. One inmate received lacerations to the back of his head and upper back area. He was treated by medical staff and placed in Special Housing. The other inmate received puncture wounds to his right arm and was transported to an
outside medical facility for treatment.  The following day 7/26/08 a review of  the camera recording revealed there was a third inmate involved. That  inmate was assessed for injuries and placed in the Special Housing Unit. The  inmate that was transported to an outside facility was returned to the
institution on 7/30/08. Investigation continues in this matter.

The inmates were given several orders to cease their actions and submit to hand restraints in which they refused to complied. Responding staff separated both inmates and hand restraints were applied. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. An investigation continues.

At approximately 6:39 p.m., the Lieutenant was summoned to the Special Housing Unit by staff due to two inmates refusing to submit to hand restraints for a cell rotation. Upon arrival to cell on, the Lieutenant ordered both inmates to submit to hand restraints. The inmates began to argue with one another, at which time one inmate brandished a razor blade and aggressively stepped towards the other inmate. The Lieutenant called for assistance via radio. Upon arrival of sufficient staff, the cell door was opened and both inmates were physically placed onto the ground. The razor blade was recovered and secured by the Lieutenant. Both inmates were restrained and escorted from the ange for medical assessments. The FBI was notified and an investigation continues.

At Aprox 1:30 - 2:00 PM, An assistance call went out, Aprox 5 on 1 Inmates were fighting. Several of the Inmates had home made weapons. The Inmate being attacked was stabbed several times. Staff responded and broke up the fight. No staff injuries were reported. All Inmates involved were taken to Health Services, screened and then taken to Special Housing. The cause of the attack is still under investigation. Even after we were locked down for three weeks over another stabbing, the administration resumed normal after several hours.

Staff members report that they monitored a telephone call placed by an inmate who talks about how he made arrangements to have the words “Sworn In” trademarked, andhe’s in the process of printing law enforcement T-Shirts for police officers, fire fighters, and military personnel. During the conversation, the inmate claims another inmate (identity unknown) at USP Victorville is responsible for creating the design (two beams of light which portray the fallen New York Twin Towers). Prison investigators are attempting to verify this information and are attempting to obtain a copy of the drawing.

Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The F.B.I. was notified and the investigation continues.

At approximately 9:26 p.m., staff were conducting rounds in Special Housing when an inmate asked for a bed sheet. He then held up a bed sheet which staff observed was blood stained. The inmate and his cell mate, were ordered to submit to hand restraints and both complied. The inmates were escorted from their cells and secured in recreation cages pending medical assessments. Medical staff conducted assessments which determined each inmate had injuries consistent with involvement in a physical altercation, though this was neither observed nor heard by staff. Investigation continues.

At approximately 4:41 p.m., staff attempted to place an inmate into a Special Housing cell. Upon opening the cell door, the inmate stepped forward, abruptly stopped, and rapidly backed into the SHU Lieutenant. Specifically, the inmate struck the SHU Lieutenant in the upper body with his right shoulder. SHU staff maintained control of the inmate and placed him into the cell. At this time, the other inmate in the cell approached the first inmate who then backed into the shower, tripped, and fell to the shower floor. Although the second inmate walked towards the first inmate in an aggressive manner, he did not strike him. The second inmate was ordered to submit to hand restraints in which he complied. Both inmates were removed from the cell, medically assessed, and placed into separate cells without incident. The FBI was notified and an investigation continues.

The inmates were given an order to cease their actions and submit to hand restraints in which they reluctantly complied. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. An investigation continues.

There were 136 inmates who stayed on the Rec. yard protesting, pretty much encouraging a group demonstration. Once the DCT team and 30 percent team were activated all inmates complied with staff orders and were removed off the Rec. yard with no incident and no injury to staff or inmates. Institution still on lockdown.

Upon the arrival of responding staff, the inmates were ordered to submit to hand restraints in which they complied. Both inmates were medically assessed and placed into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At 1:05pm, an inmate jumped on top of a Officer's desk and began stomping her feet on the desk and screaming at the Officer at the top of her lungs threatening him. The Officer hit the triple deuce phone alarm fearing for his safety. Other staff responded to the emergency and as they entered the housing unit, the inmate jumped off the desk and attempted to assault the Officer, as the other staff members attempted to restrain her she continued to fight with staff. Ultimately they had to take her to the ground where she continued to resist.  After a fifth staff member assisted they were able to restrain her and move her to Special Housing.

At 2:25 p.m., A Unit Officer was making security rounds and detected a strong odor of intoxicants and discovered inmate clothing laying on the floor covered with blood in a cell. The Operations Lieutenant was notified and the unit was secured. Upper body searches were initiated and it was determined that five inmates, were found to have injuries consistent of being involved in a physical altercation. Upon reviewing the video, four inmates were identified as being involved in a physical altercation inside of a cell. Specifically, all inmates enter the cell momentarily and rapidly exit with inmates two inmates striking one another with closed fist to the head and upper torso area. The inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

At approximately 5:30 p.m., staff called for assistance on the main walk in front of Building Two as staff witnessed two inmates assaulting another inmate. Specifically, the two were striking one inmate with their fists to the head and upper torso. As the one inmate fell to the ground, the two inmates continued striking with their fists to the head and upper torso. The inmates were separated by responding staff and placed in hand restraints. A responding staff member was medically assessed and referred to an outside hospital for evaluation. The inmates were medically assessed and admitted into the Special Housing Unit. The FBI was notified and an investigation continues.

An inmate reported to the Lieutenants Office and stated he was assaulted by another Inmate in recreation.  Both inmates were medically assessed and placed in the SHU.

An inmate was stabbed by 2 inmates.  This was not discovered until the next day on 07-05-2008 when this same inmate was stabbed again as he did not go to Medical or check in to SHU when stabbed the first time.  Several inmates were locked up over this.   No staff injured and no lockdown was ordered.

 at approximately 7:45 pm an officer requested assistance as a inmate approached him with injuries consistent with fighting.  Responding staff secured the inmates in their cells and conducted upper body checks. FOUR Inmates were observed through video surveillance entering the upper ten man cell and assaulting another inmate. It was later determine there was a weapon(s) involved but they were never recovered.  The institution was recalled and all inmate secured in their cell, upper body searches were conducted on all inmates.  The institution remained secure until the next morning when it was determined this was an isolated incident.

A fight broke out on Evening Watch in a housing unit.  It started off with 2 inmates - (gang related).  Several other inmates joined in as well, approximately 6-8 inmates total involved.  These inmates were placed in SHU.  No staff were injured and no lockdown was ordered.

At 1:15 p.m., while escorting an inmate to a Special Housing Unit cell, he began threatening staff. As staff began to place the inmate into the cell, he attempted to pull away from staff, striking a staff member on the left side. Due to his disruptive behavior, the inmate was placed on the ground to gain control of his actions. He was placed in another cell without further incident. Medical assessments were conducted. The FBI was notified and an investigation continues.

At 12:44 pm, after beginning the first meal in Food Service under modified lockdown conditions, an Officer observed an Inmate that was not properly dressed and was attempting to leave his unit to go to mainline. The Officer ordered the Inmate to submit to a pat search, and just as he noticed an unusual lump in the Inmate's sock, the Inmate turned and commenced an assault on the officer, striking him in the head as the Officer attempted  to subdue and control the Inmate. The Officer was struck several times in the head before responding Staff  arrived and restrained the inmate. A Staff member also suffered torn ligaments responding to the emergency.

*USP 1:  An Inmate became aggravated and refused the orders of the officer.  When the Inmate was ordered to submit to a pat search, the Inmate then charged the Officer.  While being restrained the Inmate continued to be combative.  Once the Inmate was restrained he made verbal threats against the Officer by stating he was going to have something done to the Officer. The Officer received minor injuries to his knee. 
 
*USP 1: An Inmate became aggravated during his unit disciplinary hearing. The Inmate made some comments of what a Staff member could do to him. The Inmate then grabbed the the Staff member by the arm and attempted to pull the Staff member into him. The Inmate was restrained by responding staff and placed in the special housing unit.   
 
*USP 1: An Inmate threw hot water in the face of an officer and began to punch the Officer in the face. The officer received minor burns and bruises to the facial area. The Inmate was restrained and placed in the special housing unit.

at 5:04 p.m., an Officer activated his body alarm after he responded to a duress alarm in cell ***, Specifically, he observed an inmate assaulting an other inmate with closed fists and kicks to the head and upper torso area. Simultaneously, upon arrival to the to the cell by the unit officer an other unidentified inmate fled from the cell. Recorded video surveillance was reviewed and revealed both inmates exiting the cell after staff’s response to the duress alarm. Upper body searches were conducted and revealed the inmate had fresh injuries consistent with being involved in an altercation to include fresh blood on his clothing and breathing heavily. All of the inmates involved were restrained, medically assessed, and placed into to the SHU without further incident. The FBI was notified and an investigation continues.

*at approximately 9:10 a.m., two inmates were located in SHU, Cell ***, when they broke the sprinkler head in the cell. A calculated use of force team was authorized by the Warden and at 10:56 a.m., the use of force was initiated. Confrontational avoidance was successful and the inmates submitted to restraints. The inmates were escorted to a holding cell and placed in ambulatory restraints to stop their disruptive behavior of destroying a life saving device without incident. Medical assessments were completed with no injuries noted to staff or inmates. The use of force was completed at 11:25 a.m.

 

*at approximately 11:58 a.m., staff called for assistance on the compound due to an inmate assaulting staff with a weapon. Specifically, staff observed an inmate acting suspiciously while on the walkway in front of a building. As staff approached him and ordered him to submit to a search, he pulled two combination padlocks attached to belts and started swinging them at staff. The Tower Officer activated the verbal warning system ordering all inmates to lay on the ground and subsequently fired three warning shots due to the inmates continued attempt to assault staff. Another Tower Officer fired one warning shot in an attempt to stop the inmates action. As responding staff approached, he attempted to run into the courtyard area. He struck one staff member in the hand with a lock attached to a belt. The inmate was restrained and medically assessed and placed into SHU without further incident. The FBI was notified and the investigation continues.

*While conducting routine pat searches of inmates, an officer was assaulted by an inmate.  The inmate turned on the pat search and struck the officer on the left side of the face.  Minor injuries to the officer with bruising and swelling.
 
*An inmate assaulted another inmate by stomping and beating the other inmate. Injuries caused were mainly broken facial bones.

8:16 am During work call movement, body alarm activated in a unit. Approximately 15 Inmates belonging to a disruptive group rushed into a unit, produced weapons, and began attacking 5 other Inmates. It took responding staff several minutes to bring the unit under order, even with the use of chemical agents. At least one Staff member was directly threatened with a weapon when attempting to stop one of the assaults (there were assaults taking place in multiple areas of the unit). The 5 Inmates that were attacked also had produced weapons, and one was wearing makeshift body armor that protected him until he was stabbed through the back and into his lung.           One inmate sent to the hospital.  LOCKDOWN until 06-30-2008

*at approximately 8:46 a.m., staff called for assistance after observing an inmate approach an other inmate from behind and strike him in the upper chest area with a wooden ice pick type weapon. Staff ordered him to drop the weapon and he complied. Responding staff separated and restrained both inmates. The inmates were medically assessed, and placed into SHU without further incident. The FBI was notified and the investigation continues.

*at approximately 1:09 p.m., staff called for assistance after observing a fight with weapons in a unit. The staff member observed an inmate strike an other inmate in the head and face with a broom handle. After striking him several times the broom handle broke into several pieces. Both inmates continued to strike at each other, fell to the ground and continued to head butt each other. Once responding staff arrived, the inmates were separated and restrained. A review of the video surveillance system identified one inmate initially throw his boot, striking the other in the back, at which time he grabbed the broom handle and started striking him. Both inmates were medically assessed and placed into SHU without further incident. The FBI was notified and the investigation continues.

*at approximately 8:55 p.m., an Officer summoned for compound staff to the unit due to an inmate appearing to be intoxicated. Compound staff ordered him to stop so they could evaluate his well being. He became verbally hostile towards staff and continued to walk away. Staff pursued him, while ordering him to stop, and he reluctantly complied. Once staff approached him they smelled a strong odor of intoxicants. Staff ordered him to submit to hand restraints and he complied. While being escorted to the SHU he began to resist staff by attempting to aggressively pull away. Staff placed him on the wall to gain control of his actions and applied leg restraints. Once inside the corridor, he became noncompliant with staff by refusing numerous orders to cease attempting to pull away from escorting staff. He was placed on the wall in an attempt to calm him. As staff began to escort him to SHU, he again attempted to aggressively pull away from escorting staff. He was placed onto the ground to gain control of his continued disruptive behavior. he was carried by escorting staff to the SHU without further incident. The inmate was medically assessed. Incident reports were written and an investigation continues.

At 7:30 pm, several Inmates in SHU became disruptive, covering windows, refusing to relinquish food traps, destroying property etc. Several force cell teams had to be organized and chemical agents had to be used during several of the uses of force to gain the Inmates compliance.

An inmate assaulted a staff member by first spitting on the staff member and then she pushed the officer down onto the ground. During a review of the video three inmates were identified that had assaulted the inmate. All inmates involved were restrained, medically assessed and placed into the SHU without further incident. * at approximately 9:36 a.m., SHU staff announced staff needs assistance in the SHU recreation yard. Staff observed five inmates assault one inmate. The inmate was knocked to the ground and was kicked and struck with fists in the head and upper torso. They were given an order to cease their actions and step into the recreation cage sallyport to be placed in hand restraints in which they complied. Responding staff separated and restrained the inmates without further incident. Medical assessments were conducted. Due to the injuries the inmate received from the assault, he was taken to an outside hospital for further evaluation. The FBI was notified and an investigation continues.

* at approximately 8:25 p.m., SHU staff called for assistance when they observed two inmates in a heated argument in cell ***-*** of the SHU. Both inmates refused staff orders to submit to hand restraints. Staff observed one inmate strike the other inmate in the facial area with a right fist. Once enough staff were on scene, the cell door was opened and both inmates were restrained and medically assessed. One inmate continued to resist staff efforts to escort him and attempted to pull away. Staff placed him against the wall to stop his disruptive behavior and regain control of him. Both inmates were placed into separate cells without further incident. The FBI was notified and the investigation continues.

 *at 1:43 p.m. the Tower Officer called for assistance on the recreation yard due to four inmates fighting. All of the inmates involved were observed striking one another with closed fists to the head and upper body. The Tower Officer activated the bi-lingual verbal warning ordering the inmates onto the ground. The inmates failed to comply and continued to strike one another. Perceiving a threat to staff and inmate safety, the Tower Officer fired three warning shots; the other Tower Officer fired eight warning shots, and another Tower Officer fired three warning shots. Upon the arrival of responding staff, one inmate attempted to depart the area; however, he was located and restrained. The remaining inmates were separated and restrained by responding staff. As staff attempted to place one of the other inmates in restraints, he struck a staff member on the right side of his face with a closed fist. All inmates were medically assessed and placed in the SHU without further incident. The FBI was notified and an investigation continues.

* at approximately 9:00 p.m., an Officer summoned compound staff to the unit. Upon arrival, an inmate smelled of a strong odor of intoxicants. Staff ordered him to exit the unit in which he reluctantly complied. Once outside the unit, hand restraints were applied. While being escorted to the SHU he began to resist staff by attempting to aggressively pull away from staff. Staff gave him numerous orders to cease his actions in which he refused to comply. He was placed onto the ground to gain control of his actions. Once on the ground, leg restraints were applied. Staff assisted him to his feet and initiated the escort once again. Once inside the Corridor, he became verbally aggressive while attempting to pull away. He was placed onto the wall in attempt to calm his actions. As staff began to escort him to SHU, he became hostile by yelling obscenities and attempted to aggressively pull away from escorting staff. Staff gave him several orders to cease his actions in which he refused to comply. He was once again placed onto the ground to gain control of his continued disruptive behavior. He had to be carried by staff to the Special Housing Unit. The inmate refused to submit to a breathalyser test. He was medically assessed. Incident reports were written. An investigation continues.

All of the inmates involved were restrained and medically assessed. The inmates were breathalyzed with two providing a positive reading for intoxicants. One refused the breathalyzer test. All inmates were placed in the SHU without further incident. The FBI was notified and an investigation continues.

 at 7:30 p.m., SHU staff ordered an inmate to submit to hand restraints for the purpose of accepting a cellmate. He refused to comply with staff orders and barricaded himself in his cell. Confrontation avoidance was initiated and was unsuccessful. Notifications were made and authorization was granted to assemble a calculated Use of Force team. As the Use of Force team approached his cell door, he was ordered to submit to restraints in which he complied. As a result of his disruptive behavior, he was placed into ambulatory restraints. He was medically assessed and placed into a holding cell without further incident. The investigation continues.

A fight between  inmates occurred and before the inmates all returned to the housing units,  One officer was assualted.  He was injured in the face or head. Reports are that the officer had been transported to the hospital for treatment. It was later discovered that his jaw had been broken in 3 places. Surgery was scheduled.

* at approximately 3:20 pm, an Officer called for assistance. As he was conducting rounds he smelled intoxicants coming from cell ***. As the officer opened the door he observed an inmate laying on the bottom bunk, with blood coming from his head and an other inmate sitting on the chair. Both inmates were intoxicated. They were escorted to medical and assessed. The inmate sustained two inch laceration cut on left side of head. An investigation continues.

* at 8:45 p.m., An inmate was secured in the SHU Law Library. He picked up the typewriter, located within the Law Library, and repeatedly threw it against the walls and windows. He then used a piece of the broken typewriter to destroy the library’s fire suppression system, causing the library and ranges to flood. Notifications were made and authorization was granted to assemble a calculated Use of Force team. Confrontation avoidance was successful and he was removed from the Law Library. As a result of his disruptive behavior, he was placed into ambulatory restraints. He was medically assessed and placed into a holding cell without further incident. The investigation continues.

*6:15 AM, we had a cell fight in Special Housing, Inmates were separated and moved to other cells.
*6:25 AM, an assistance call was made, two inmates were fighting. No weapons were seen and no staff injuries reported. Inmates were placed in Special Housing Unit, Investigation continues.
*12:12 PM, A fight between two inmates started in our East Corridor, as the two inmates were fighting when a third inmate joined in. It was a two on one fight as staff responded. No weapons were seen, No staff injuries reported. All three inmates were placed in Special Housing Unit. Investigation continues. At that time we went into lock down. Inmates were locked on the block with no Inmate movement. But were allowed to move about the block.
During this time 6 Inmates from various incidents were put into ambulatory restraints in Special Housing Unit.

It was also determined two other inmates were also involved in the assault. All of the inmates involved were restrained, medically assessed, and placed into to the SHU without further incident. The FBI was notified and an investigation continues.

* at approximately 9:45 a.m., an inmate, destroyed the sprinkler head in a cell in the SHU. Due to his disruptive behavior, authorization was granted to assemble a Use of Force team. Confrontation avoidance was initiated and proved to be successful. He was ordered to submit to restraints, in which he complied. While he was being placed in ambulatory restraints, he made numerous threats to kill the President of the United States and the Warden upon his release from prison. The inmate was medically assessed and placed into a holding cell without further incident. 

* at approximately 10:58 a.m., an Officer called for assistance via radio when he observed two inmates striking one another to the head and upper torso with fists in cell ***. As staff responded , the inmates were given a order to stop fighting in which they complied. The inmates were restrained, medically assessed, and placed in the SHU without further incident. An investigation continues.

* at approximately 7:04 p.m., an Officer activated his body alarm when he observed two inmates striking one another to the upper torso with closed fists. The Officer ordered them to stop fighting; however, they refused and continued to strike each other with fists to their upper torsos. As responding staff arrived, they were physically separated and restrained. The inmates were medically assessed, and placed in the SHU without further incident. Staff reviewed recorded video surveillance which depicted one approaching an other inmate at approximately 7:00 p.m. He used both of his hands to grasp his neck and choked him. Recorded video surveillance him coming to the aid of the inmate being choked at which time both of them began to physically fight. One inmate smelled of intoxicants and subsequently refused a breath test. The FBI was notified and an investigation continues.

As responding staff were escorting both inmates from the unit, one attempted to break free and lunged towards the other. He was placed on the ground and restrained due to his disruptive behavior. Both inmates were medically assessed and placed into the SHU. The FBI was notified and an investigation continues.

* at approximately 1:46 p.m., an Officer activated his body alarm when he observed two inmates, striking one another to the head and upper torso with fists. As staff responded and the inmates were given several orders to stop fighting in which they refused to comply. Responding staff had to physically separate both inmates. The inmates were restrained, medically assessed, and placed in the SHU without further incident. An investigation continues.

 *At approximately 9:21 a.m., Control Center announced staff needs assistance in the Special Housing Unit, cell ***-***. Specifically, as staff restrained one inmate for recreation, his cellmate began to strike him in the head and facial area with his closed fists. The inmate was given an order to stop his actions and lay face down on the bed, in which he complied. Upon the arrival of sufficient staff, the cell door was ordered open by a staff member. He was placed into hand restraints and removed from the cell. Both inmates were medically assessed and placed into separate holding cells without further incident. The FBI was notified and an investigation continues.

* At approximately 3:09 p.m., Control announced a fight on the recreation yard. Staff observed two inmates striking one another in head and facial area with their fists. One inmate fell to the ground and the other inmate began to kick him in the head and facial area with his shoes. A tower officer fired four warning shots to stop the inmates disruptive behavior. Another tower officer fired two warning shots. The inmates ignored the warning shots and continued to fight. A tower officer fired one warning round into the designated pit and another tower officer fired three more warning shots due to the inmate continuing to kick him in the head as he lay unresponsive. Upon arrival of responding staff the inmates were restrained, medically assessed, and placed into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

* At 8:23 p.m., staff were conducting cell rotations in the Special Housing Unit. Staff placed an inmate in cell ***, and opened the food slot to remove his hand restraints. At this time, he turned and spit out of the food slot, striking the staff member in the right knee. The food slot was immediately secured. An incident report was written. The FBI was notified and an investigation continues.

There was a fight in one of the housing units involving multiple inmates with weapons.  Several inmates received injuries and one inmate was taken to the outside local hospital.  No staff were injured.  The institution came off lockdown status on Monday.  Shortly after the institution came off lockdown, another fight occurred in a housing unit involving three inmates.  This was followed by a staff member being assaulted in another housing unit.  The staff member was not injured.  The institution was again locked down and remains locked down.

At approx. 8:10 pm an assault of 3 inmates on 1 inmate took place in front of a Unit.  The 3 Inmates were kicking and punching the other Inmate in the head and lower torso area.  Inmates complied when staff arrived and were all taken to Special Housing.  No staff were injured.

FCI- FLORENCE, MAY 30, 2008
 
At approx. 6:35 am staff observed an Inmate outside of the Medical Dept. bleeding from the top of his head.  The Inmate had injuries to the face and head consistent with being involved in a fight.  The Inmate stated that he had fought with his cellmate.  When the cellmate was interviewed he admitted to the fight and both Inmates were put in Special Housing.  No staff were injured.

At approximately 10:30 a.m., an inmate destroyed the sprinkler head in a holding cell in the Special Housing Unit. Due to the inmates disruptive behavior, authorization was granted to assemble a Use of Force team. Confrontation avoidance was initiated and proved to be successful. He was ordered to submit to restraints, in which he complied. He was placed in ambulatory restraints, medically assessed, and placed into a different range of holding cells without further incident.

At approximately 1:20 p.m., Special Housing Unit staff observed an inmate kick an other inmate in the left leg and knee area with his left leg. Staff ordered him to cease his actions and he refused to comply. The cell door was opened and both inmates were separated. While separating the inmates, the inmate lunged forward and attempted to head butt staff. Inmate was placed onto the wall to gain control of his disruptive behavior. Both inmates were removed from the cell and medically assessed without further incident. The FBI was notified and an investigation continues.

At 8:25pm, Recreation Tower called out a fight on the Recreation Yard, 10 inmates were involved in the fight with weapons. Responding staff broke up the fight with no staff injuries to report. We are locked down at this time.

At approximately 11:30 am an Officer was walking with an inmate to the bathroom for some type of search.  The inmate charged into the Officer knocking him into the door frame.  Before the Officer could recover, the inmate flushed an  unknnown object down the toilet contained in a plastic bag.  The Officer instructed the inmate to stay where he was at however; the inmate pushed past the officer and left the area.  Additional staff  arrived and secured the inmate.  The Officer and inmate were examined by medical staff.  Inmate was placed in Special Housing Unit pending further investigation.

There was an assault in a housing unit during the daywatch shift, 2 inmates on 1,  The one inmate was stabbed several times and needed to be taken to an outside Hospital for medical treatment, the housing unit was locked down and the other 2 inmates were placed in SHU, the rest of the institution ran as normal.

At 3:46 pm Medical Emergency needed in a unit.  Inmate was found laying in cell with multiple stab wounds and serious blood loss. A tourniquet was needed to keep inmate from bleeding out.  Six inmates were sent to SHU as a result.  LOCKDOWN until 06-02-2008.

At approximately 1:40 p.m., the Tower # * Officer announced a fight on the yard. Staff observed an inmate striking an other inmate with closed fists to the head and upper torso area. The Tower # * Officer activated the automated voice system ordering the inmates to get down on the ground. The inmates refused to comply and continued their disruptive behavior. The Tower # * Officer fired four warning shots. As responding staff arrived, the inmate continued to strike the other inmate in the head with his closed fists. Tower # * then fired two more warning shots and Tower # * fired two warning shots. Tower # * observed a group of inmates moving toward staff and fired three warning shots. Responding staff separated and restrained both inmates. Both inmates were escorted off the recreation yard, medically assessed ,and placed in the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

The unit officer called for assistance and as responding staff arrived, the unit was locked down. Staff arrived at cell #* and found a inmate hiding under the lower bunk, with abrasions to his hands. Both inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At approximately 9:16 a.m., the Special Housing Unit Recreation Officer observed two inmates assaulting another inmate by striking him in the head and upper torso area with their closed fists. The officer immediately called for assistance. The inmates were ordered numerous times to separate; however, they refused to comply. Upon arrival of sufficient staff, the recreation door was opened and the inmates were separated and restrained. Medical assessments were conducted and the inmates were placed back into their assigned cells without further incident. The FBI was notified and an investigation continues.

**At approximately 11:58 a.m., staff announced call for assistance on the compound. Staff observed three inmates punching each other in the head and torso with their fists. Staff ordered the inmates to stop, they refused to comply and continued to punch each other with their fists. Due to their disruptive behavior and failure to comply with staff orders, the inmates were taken to the ground by responding staff and restrained. The inmates were medically assessed and escorted to the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At approximately 12:15 p.m., staff observed numerous inmates on the recreation yard adjacent to the soccer goal on the south end. Staff observed a heated argument between two inmates. One inmate attempted to walk away from the incident and the other inmate threw an apple, which struck him in the back between his shoulder blades. He continued to walk away from the area and the other inmate attempted to kick him. The inmates were separated and restrained. Both inmates were medically assessed and escorted to the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At approximately 1:46 p.m., staff announced a fight on the recreation yard. Staff observed three inmates striking an other inmate in the upper torso area. The Tower # * officer activated the voice alarm ordering the inmates to get down on the ground; however, the inmates refused to stop their assault. Tower # * shot one warning shot to stop the inmates assault on inmate *. The inmates refused to comply. The Tower # * and Tower # * officers fired a total of seven warning shots to stop the inmates disruptive behavior. As responding staff arrived, the inmates were separated, restrained, and escorted off the yard. The inmates were medically assessed and placed into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At approximately 3:35 p.m., the tower # * Officer notified the compound officers he saw an inmate strike another inmate. Staff observed one inmate head butt an other inmate in the facial area. Upon the arrival of responding staff, the inmates were restrained, medically assessed, and escorted to the Special Housing Unit without further incident. The FBI was notified and the investigation continues.

At approx. 9:20 am, a inmate was given a order to report to the Lts. office. The inmate refused that order and  proceeded to walk to his cell.  The order was repeated, at which time the
inmate walked toward the Unit door, and in a aggressive manner stated, " I  am not leaving the Unit".    He then turned on staff; in what was perceived as a threatening stance. At this time, staff noticed the inmates  from this particular inmates area watching and crowding toward the area of this incident. 3 staff members  gained control of the inmate by forcing the inmate into the Sallyport of the Unit, out of the crowd of inmates whom were gathering in the unit.  Immediate Use of Force was initiated at this time, taking the inmate  to the ground to control his actions.  During this entire time, the
inmate was combative, verbally abusive, verbally defiant,  and striking  at the unit staff.  The inmate was restrained.  Upon additional staff  arriving at the scene, the inmate was escorted to the Lts, office. 1 staff member sustained a knee injury, a 2nd was kicked in the side of the head, and the 3rd staff member sustained cartlige injuries to a  hand during this assault on staff.

At 8:44 a.m. Inmate assaulted a staff member by Bitting Him. The inmate Struck another Officer in the face causing a large cut above his eye. Inmates were not put on lockdown, left out on Recereation yard. A recall was done only to count,  then inmates relased for chow and normal operations.

At approximately 6:22 p.m., the Tower # * Officer activated the bi-lingual warning system, ordering all inmates to the ground. Staff called for assistance upon observing two inmates fighting at the middle table on the recreation yard. Tower # *, perceiving danger to responding staff, fired one warning shot however they continued to fight. Responding staff assumed physical control of both inmates and removed an approximate 6.5" in length, sharped wooden weapon from the right hand of one inmate. They were removed from the recreation yard, medically assessed, and admitted into the Special Housing Unit without further incident. The FBI was notified and the investigation continues.

At approximately 7:30 pm an  Officer was punched in the face by an inmate. This happened about an hour after an inmate was stabbed in a housing unit. The officer was not communicating with the inmate that punched him.  The
officer was standing alone watching other inmates.  There was no disagreement, the inmate walked up to the Officer and just punched him.

At approximately 10:28 a.m., a staff needs assistance call in a Unit was called via radio by Control. As responding staff arrived, a staff member had an inmate restrained on the floor in the Unit outside of cell ***. A review of the incident revealed the inmate entered cell ** while staff was conducting a cell search. During this time the inmate attempted to remove items from his cell. Staff ordered him to drop the items and attempted to retrieve the items from him. The inmate began striking Staff repeatedly to the head, facial, and throat areas. The Staff member pushed the inmate out of the cell in an attempt to stop inmate and his attack. The inmate was placed on the ground with the minimal use of force necessary and was placed in hand restraints. Then escorted to Health Services for a medical assessment and was placed into Administrative Detention without further incident. The FBI was notified and the investigation continues.

Institution locked down due to a disturbance on the recreation yard between gang related inmates. No staff were hurt, a few inmates received minor injuries, weapons were involved.  Immediately after restraining the inmates on the yard, there was a call for assistance, due to another fight in a housing unit between different inmates of the same gangs involved in the earlier fight, involving weapons.  Institution remains locked down.

During evening watch there was a fight on the rec. yard with weapons.A Tower  officer fired his non-lethal weapons. inmates did not respond. The Tower officer saw an inmate getting stabbed, he fired his M-16 warning shot, to which the inmates finally obeyed all commands to cease  and disist activity. This incident caused a lock down of the insitution
until further notice.... During the week of lockdown many homemade  weapons were found. 15 shanks were found on one shift..

**At approximately 3:00 p.m., a Unit Officer observed an inmate waiving his arms and yelling obscenities. The Officer observed blood on the inmate’s head and immediately radioed for assistance. Upon the arrival of responding staff, the Unit was secured and upper body searches were initiated. An inmate was identified as having injuries consistent with being in a physical altercation. The surveillance tape was reviewed and it was determined that six inmates were involved in the incident. The inmates were medically assessed and admitted into the Special Housing Unit without further incident. The FBI was notified and an investigation continues.

**At approximately 6:35 p.m., staff radioed for assistance in the Special Housing Unit. Staff observed two inmates fighting in a recreation cage. Both inmates were striking one another in the upper torso and facial area. Responding staff separated and restrained both inmates. Both inmates were medically assessed and placed into separate cells without further incident. The investigation continues.

When the officer attempted to feed inmate ***, the inmate grabbed the Officer by the right hand and in an aggressive manner proceeded to pull the officer into the cell through the food  slot. The Officer immediately pulled his arm from the food slot and secured it. Both the Officer and inmate were medically assessed with no injuries noted. An investigation continues.

At approximately 11:55 a.m., an inmate was being escorted to the Lieutenant’s Office from the metal detector building. As staff approached the exit door of food service, the inmate started to run towards the housing units and staff radioed for assistance. Tower # * activated the bi-lingual warning system to order all inmates onto the ground. The Tower # * officer then fired two warning shots, fearing for the safety of staff and in an attempt to get the inmate to stop. Tower # * observed numerous inmates in the immediate area of the assistance call, and fired one warning round into the designated pit. The inmate changed direction and headed towards the east corridor entrance door. He was observed by staff placing something into his mouth. Responding staff placed him on the ground and ordered him to spit out the item he had placed into his mouth. He spit out two clear plastic wrappers which contained 15 small packages of suspected narcotics. He was restrained, medically assessed, and placed into the Special Housing Unit without further incident. The FBI was notified and the investigation continues.

At approximately 12:23 p.m., Tower # * called for assistance on the recreation yard. Specifically, Tower # * notified Recreation Staff that there appeared to be an inmate on a ball court sharpening a weapon. Recreation staff confronted the inmate at which time he refused to comply to a pat search three separate times and began to run away toward the toilet area. Tower # * initiated the bi-lingual voice command and fired five warning shots to get the inmates attention. Staff gave the order for him to stop but the inmate refused to comply. Tower # * fired one warning shot into the pit due to the amount of inmates on the recreation yard. Fearing for responding staff safety, Tower # * fired two warning shots. Staff observed the inmate flush an item wrapped in blue down the toilet. Immediate force was used on the inmate due to his disruptive behavior towards staff and to gain control of him. He was immediately medically assessed and escorted to Special Housing without further incident. FBI was notified. An investigation continues.

The inmates were located in outside recreation. The officer immediately called for assistance. The inmates were ordered numerous times to separate and refused to comply. Upon arrival of sufficient staff, the recreation door was opened and the inmates were separated. Hand restraints were applied to the inmates and they were escorted to separate holding cells. All of the inmates involved were medically assessed and placed back into their assigned cells without further incident. The FBI was notified and an investigation continues.

**At approximately 12:50 p.m.,  an Officer announced a fight in front of a Building. Staff observed an inmate continuously strike an other inmate in the face several times with his closed fists. Upon the arrival of responding staff, the inmates were restrained, medically assessed, and escorted to the Special Housing Unit without further incident. The FBI was notified and the investigation continues.

There was an Assault Unit *.  There was 1 inmate who was stabbed.  He was taken to an outside hospital and required a total of 10 stitches to three different injuries.  Unit * was locked down for just under 24 hours.  The rest of the institution operated as normal as possible.

Inmate ** was escorted to the Lieutenants Office where he stated he had been assaulted by another unknown  inmate. Inmate ** was escorted to the Special Housing Unit, where he was medically assessed, and photographs were taken. A review of recreation cameras revealed inmate ** being assaulted in the recreation multimedia room at 5:55 p.m. by inmate  **. During the interview of inmate ** he admitted to assaulting inmate **. Inmate **was escorted to the Special Housing Unit, where he was medically assessed, and photographs were taken.

**At approximately 3:40 p.m., the Yard Officer was at the Compound Detector, monitoring inmate movement during institution re-call, when the officer observed an inmate walking on the side walk, from a Building towards the Compound Detector. The Yard Officer intercepted the inmate upon observing he was walking the wrong direction and noticed that he had recently sustained facial injuries. He was escorted to the Lieutenant’s Office where during an initial interview he refused to provide accurate information explaining the origin of his injuries. Inmate was photographed, escorted to Health Services for an assessment and treatment of minor injuries, and then placed into Special Housing without incident. SIS staff reviewed recorded surveillance tape which depicted him engaged in a physical fight with two other inmates at cell *** at approximately 3:32 p.m. He left the Unit where moments later he was intercepted by the Yard Officer. As a result of their actions and behavior, all three inmates have received incident reports for a violation of Code 201, Fighting.

**At 12:00 p.m., staff was feeding the noon meal in the Special Housing Unit. Staff opened the food slot on cell ***, giving an inmate his noon meal. The inmate threw his food trays out of the food slot striking the officer in the left hand. The food slot was immediately secured. The staff member was medically assessed with no injuries. The inmate was medically assessed with no injuries noted. A incident report was written. The FBI was notified and an investigation continues.

**At approximately 8:13 p.m., a Tower Officer activated the bi-lingual warning device, ordering inmates onto the ground, and then summoned assistance, via radio, upon observing inmates facing each other and assume a fighting stance and began fighting while standing at the recreation yard. Staff responded quickly and assumed physical control of one inmate by placing him onto the ground. He briefly resisted staff’s efforts to secure him using hand and leg restraints; however, after applying minimal force, He was secured using hand and leg restraints causing all resistant behavior to cease. During the process of assuming control of him, the other inmate blended in with other inmates and he was not identified until approximately 30 minutes later, via a review of recorded surveillance tape. Though the Tower Officer stated he saw each inmate swing their arms at the other he did not see any physical strikes. However, it was later determined the first inmate had drank intoxicants and he provided two breath analysis tests, producing results of .134% and .145%. They were both medically assessed and neither had evident injuries. Subsequent information revealed they were on the Rec. Yard, socializing with other inmates when one inexplicably became agitated and aggressive, most likely worsened by his use of intoxicants. Efforts to calm him failed, resulting in him swinging, and then missing. Due to his actions and behavior he has received an incident report for a violation of Code.

**At 7:17pm ,Control center announced a fight in ** with weapons. No information on inmates involved.

**At 12:15 p.m., while feeding in the Special Housing Unit, staff approached a cell. Staff placed a food tray on the wicket. As the staff member turned to retrieve more trays, an inmate threw the tray out onto the range, striking the Officer in the right thigh area leg. The wicket was immediately secured. The staff member was medically assessed with no injuries. The inmate was medically assessed with no injuries noted. A incident report was written. The FBI was notified and an investigation continues.

**At approximately 12:43 p.m., staff announced assistance on the recreation yard. Responding staff observed three inmates fighting by hitting each other with closed fist to the head and upper torso. Responding staff separated and the restrained the inmates. The inmates were escorted to medical for an assessment and then to Special Housing Unit without further incident. An investigation continues.

At 12:31 pm a fight broke out,Two inmates involved.

*6:18 pm Body alarm sounded for a fight. Inmate was knocked out and then repeatedly stomped on by 2 other inmates. Two inmates taken to hospital one with closed head injuries.

 

*7:15 pm Assistance call in a unit, second fight related to above incident.  Two inmates involved.

At approximately 8:24 p.m., a Unit Officer used his radio to announce a fight in a Unit. Specifically, the Officer observed an inmate walking backwards, away from and while facing another inmate, on the first floor of the Unit, near the Officer’s Station. They stopped and both of them faced each other and assumed fighting stances and began fighting. The Unit Officer ordered both to cease and separate and they complied. Responding staff applied hand restraints to both inmates and they were escorted from the Unit without incident. The two inmates were cell mates, and their cell was searched, during which blood was discovered on the south wall between the upper and lower bunk and the bed sheets of the lower bunk.

**At approximately 11:50 a.m., Compound staff announced a fight on the compound. Staff observed an inmate, began to fight with an other inmate, by repeatedly striking him in the head and chest area with their fists. A Tower activated the voice command system to order the inmates to get on the ground. The inmates did not comply. The Tower officer fired two warning shots. The inmates did not comply with orders. Tower officer fired two more warning shots, pausing in between each one to give the inmates time to comply and cease fighting. However, the inmates failed to comply with staff orders and continued their disruptive behavior. Concerned for the safety of responding staff and other inmates, a seperateTower fired one warning shot, into the designated area. The group of inmates next to the entrance door to mainline complied and got on the ground while responding staff separated and restrained the inmates. The inmates were medically assessed and placed in the Special Housing Unit without incident. An investigation continues.

 

**At approximately 3:24 p.m., a Unit officer announced a fight in front of a Building by observing two inmates, striking another inmate, in the head and torso area several times with their fists. All three inmates immediately separated and two of them complied with staff orders. The other threatened staff when given orders to get on the ground and refused to comply. Responding staff restrained and escorted both inmates to Health Services and the lieutenant’s office. The other was placed on the ground with minimal amount of force to gain control. The inmates were medically assessed and placed in the Special Housing Unit without incident. The FBI was notified and the investigation continues.

USP- TUCSON, MAY 3, 2008
 
Institution was just removed from lock-down status on April 30, 2008 just to return to lock-down status 3 days later. Staff found a letter that stated "they" (inmates) were going after an inmate and they did. Approximately 30 inmates were involved in the incident. An inmate was stabbed during this disturbance. Inmate was taken to local hospital.
